Workflow: Accessing the Patient Chart Setup Window

1. Access the Patient Chart Setup window by clicking the gear icon at the upper right of the patient’s chart.

Alternately, you can access the Patient Chart Setup window from the Intergy EHR menu by selecting Setup, and then selecting Patient Chart.

2. From the Settings For drop-down list, select the chart layout you want to change. Based on security settings, you may be able to modify only your personal chart layout. However, you might have access to modify the Practice Default Chart layout.

Note: If you modify the Practice Default Chart layout, the changes you make will apply to everyone who uses the default layout. For more information about assigning security permissions for modifying the Practice Default chart layout, refer to the topic “Setting the Security Permissions for Patient Chart Layout” in the Intergy EHR online help or the Intergy EHR Setup Guide.

Configuring Chart Navigation Buttons

You can configure which navigation buttons display in the Nav Bar on the left side of the patient’s chart in Intergy EHR. You can also reorganize the buttons to display in the order you want. For example, you could move the Immunizations button higher up on the list, or move the Docs button farther down.

With this enhancement you now can configure the Web Applications button, which previously was not movable.

In addition, the Medications and Administered Meds buttons have been separated, and now can be configured independently.

Similarly, the Recalls, Charge History, Diagnosis History, and Appointments buttons have been separated, and now can be moved and configured independently.

With these enhancements, the only button that cannot be moved or configured is the Info button, which always remains at the top of the Nav Bar.

Workflow: Configuring Chart Navigation Buttons

1. On the Patient Chart Setup window, select the Chart Navigation tab. The Chart Navigation tab displays.

The chart summary sections are listed in the current order that they display in the Nav Bar of your Intergy EHR system.

2. Do the following to configure the buttons of the Nav Bar:

If Then

You want a button to display in the Nav Bar

Check the box beside the button. The button will display in the Nav Bar. If the Nav Bar contains more buttons than can be displayed, the additional buttons can be accessed through the More option on the Nav Bar.

You want to remove a button from the Nav Bar

Clear the box beside the button. The button will not display in the Nav Bar.

3. When you are done configuring chart navigation options, click the Save button to save your changes. All the changes you have made on each of the tabs on the Patient Chart Setup window are saved.

If Then

You want a section to display in the Chart summary

Check the box beside the section. The section will display in the Chart summary.

You want to remove a section from the Chart summary

Clear the box beside the section. The section will not display in Chart summary. For example, if you do not want Recent Images to display in the Chart summary, or you do not want Tasks to display in the Today panel, clear those boxes.

You want to move a section to a different position in the Chart summary

Drag and drop the section in the list to the location where you want it to display. For example, if you wanted to move the Recent Cardio Results section to the third position in the summary, drag and drop it there from its current location in the list.

3. When you are done configuring the Chart summary and Today panel, click the Save button to save your changes. All the changes you have made on each of the tabs on the Patient Chart Setup window are saved.

Configuring Encounter Detail Sections

The Encounter tab of the Patient Chart Setup window allows you to control what information displays in the Encounter Detail in Chart. For example, if your practice is a cardiology clinic that does not perform immunizations, you could choose not to display information about immunizations.

You can also specify whether each section displayed in the Encounter Detail should be displayed in its normal width or in Wideview.

Note: You display Encounter Detail by selecting the Pt Enc button in the Nav Bar, then selecting an encounter from the list of encounters that displays.

The workflow below describes how to configure Encounter Details sections.

Check the box in the Wide column beside the section. This provides more space on the page to display the information. For example, if you want medications to display in Wideview, check the box in the Wide column beside it. The other sections will remain their regular size, while the Medications section will span the width of the Encounter Detail.

You want a section to display in normal width

Clear the Wide check box for that section.

You want to move a section to a different position in Encounter Detail

Drag and drop the section in the list to the location where you want it to display. For example, if you wanted to move the Recent Cardio Results section to the third position in the detail, drag and drop it there from its current location in the list.

Chart Patient Summary Enhancements

Patient Summary Enhancements

The following enhancements have been made to the Patient Summary in Chart:

• The patient banner now displays the patient’s primary insurance.

• The new Care Team section is available on the Patient Summary.

• The Recent Labs, Cardio Results, and Images sections in Patient Summary now show the last five entries for each. This lets you view the most recent results for those sections, even if they occurred before the patient’s last visit.

Showing/Hiding the Patient Picture

As in previous versions of Intergy EHR, you can set up Intergy EHR parameters so that you can view an available photo for the patient, birth date, and Social Security number in the Patient Banner section of the Patient Summary. For patients without a photo on file, the generic photo icon displays as blue for male patients, magenta for female patients, and indigo for those patients whose gender has not been recorded.

In version 9.10 of Intergy EHR, a collapse icon has been added to the bottom right of the Patient Banner when the patient’s picture is visible. Clicking the collapse icon hides the patient’s picture, name, and date of birth.

If the Patient Banner is minimized, then the expand icon displays instead. Clicking the expand icon displays the patient’s picture, name, and date of birth.

Orders/ChargesAuthorization Number in Orders Group Details

Authorization Number in Orders Group Details

The Orders Group Details dialog box now contains the new Authorization # field, which specifies the identifying number associated with the order. The authorization number might be provided by the patient’s insurance or a referring provider.

This new field allows you to include the authorization number on the order. Then, when the order is sent via HL7 to another practice or facility, the number will be on the order that is received. Once it is on the order, it is included on other items, such as the appointment that is created.

For more information about the authorization number and HL7 orders and results, see “Sending Outbound Orders (from Intergy EHR)” on page 33.

Orders/Charges Changes to Diagnoses and Orders

Changes to Diagnoses and Orders

Intergy and Intergy EHR now support encounter-level diagnoses for billing. This allows providers to select as many diagnoses as needed to associate with an encounter on a claim; however, only four (4) diagnoses can be associated with a charge. Beyond the four charge-associated diagnoses, the remaining diagnoses will display on the claim as additional encounter diagnoses. The additional diagnoses will be limited to a number specified by the insurance payer.

Along with this enhancement, the Selected Diagnoses and Orders panel of Orders/Charges has been renamed the Orders/Charges Cart.

As in previous versions of Intergy EHR, when a diagnosis or order is selected, it displays in the bottom panel of the Orders/Charges window in the Orders/Charges Cart list. The diagnoses are listed first, and then each order displays under the primary diagnosis with which it is associated. You can add diagnoses for an encounter in the Orders/Charges Cart.

Diagnoses can be added from the encounter note or from Orders/Charges. The diagnoses are displayed in numerical order. You can reorder the diagnoses in the orders tree by dragging and dropping the diagnoses.

The maximum allowed diagnoses is the maximum number supported by the patient's primary insurance, which is displayed in the Orders/Charges Cart header. If the Orders/Charges Cart list contains more diagnoses than the number supported by the insurance, a warning will be displayed when you attempt to save. Diagnoses added to the cart will be

Orders/ChargesChanges to Diagnoses and Orders

added to the encounter, and diagnoses removed from the cart will be removed from the encounter.

The icon that displays in front of the order indicates its type: prescription renewal, new prescription, lab request, task, order, or charge. Icons also display for follow-up information and facility information. Any diagnoses selected for the patient in Encounter Note display automatically in the Orders/Charges Cart list. The text “From Note” displays in parentheses after these diagnoses to distinguish them from diagnoses selected in Orders/Charges.

Diagnosis Tree ModificationsThe following modifications have been made to the diagnosis tree in the Orders/Charges Cart:

• The No Diagnosis node now is always the first in the tree. You can no longer drag and drop it to other locations

• Unresolved diagnosis nodes (such as for multiple ICD-10 codes) are listed after the No Diagnosis node, if it exists. If the No Diagnosis node is not present, then unresolved diagnosis nodes are listed first.

Note: You can no longer drag and drop an unresolved diagnosis node to another location in the list. However, after a diagnosis is resolved it is numbered and can be moved.

• Re-ordering the nodes in the Orders/Charges Cart affects their order in the encounter diagnoses, and will affect their position on the claim when billing.

• Additional diagnoses added in labs, charges, and so forth, are added to the tree in the Orders/Charges Cart. However, they are only saved if you click Save in the cart. Note that this is only applicable to labs ordered via the Lab Ordering window.

User Password Enhancements

The following Intergy user password enhancements have now been implemented for the Intergy and Intergy EHR systems:

• Security Questions for Changing a Password at Logon

• Setting Up Your Security Questions

• Option to Change Password at Logon

These enhancements are described in the following sections.

Security Questions for Changing a Password at LogonSecurity questions are now included for answering when you change your password at logon. This provides greater security for your Intergy system by requiring additional authentication of users.

Previously, security questions were not used with Intergy passwords.

Now, a user is required to set up security questions for changing passwords at logon. When attempting to change his or her password at logon, a user is required to answer the security questions. Additionally, when a user is forced to change a password, the user will be required to reset his or her security questions.

Setting Up Your Security QuestionsTo set up your user security questions and answers, the new Select Security Questions dialog box has been added for selecting your user password security questions and entering your answers.

The Select Security Questions dialog box displays on the first logon after upgrade for users to set up their security questions.

Practice and System Setup User Password Enhancements

You can select three security questions and enter your answers or you can skip the security questions for this logon.

To Do this

Select security questions and enter answers

1. For 1. Question, 2. Question, and 3. Question, select a security question, and then enter your answer in the field below the question. Answers are not case sensitive.

2. Click the Save button.

Skip security questions for this logon

Click the Skip for now button. You will be prompted to select questions and enter the answers the next time you log on.

Option to Change Password at LogonYou now have the option to change your password from an Intergy system (Intergy and Intergy EHR) logon window, if the user permission to change your password had been set by the system administrator. This allows you to reset your password and log on if you are locked out or have forgotten your password. Additionally, this provides you with the ability to change your password at logon.

Previously, passwords could be changed only after logon from within the Intergy system, if the user permission to change your password had been set by the system administrator. If you had forgotten your password, there was not a method available for you to reset your own password and log on, which was done by support personnel.

Now, you have the ability to reset or change your password at logon, if the user permission to change your password has been set by the system administrator. The new Forgot Password? link has been added to all LogOn windows for the Intergy and Intergy EHR systems.

Additionally, the new Security Questions dialog box has been added. The Security Questions dialog box displays when you click the new Forgot Password? link on an Intergy system (Intergy and Intergy EHR) LogOn window. This allows you to answer your password security questions in order to change the password.

Workflow: Changing the Current User Account Password at Logon

If your user account has security permission to change your password, you can now do so from an Intergy system (Intergy and Intergy EHR) logon window. This permission is configured from the Users and Security window in either Practice Administration or System Administration.

1. In the Intergy system logon window, enter your user name.

2. Click the Forgot Password? link. The Security Questions dialog box displays.

Note: If your current user account has not been set to allow you to change the password, you will be prompted to contact your system administrator.

3. In the Security Questions dialog box, enter an answer to the three security questions and click OK. If the three questions are all answered correctly, the Change Password dialog box displays.

4. Enter the current password.

5. Enter the new password.

6. Enter the new password again in the Confirm new password field. This verifies the new password is entered correctly.

7. Click the OK button. You are logged in to the Intergy system. The new password will be in effect on the next logon to your account.

PrescriptionsPharmacy Notes

Pharmacy Notes

The Pharm. Notes field has been added to the Rx Details dialog box. This field lets you provide additional notes for the pharmacist that are separate from the patient instructions.

Quick text notes for the Pharm. Notes field can be defined in the Intergy desktop in Quick Text Maintenance for the quick text type 'RXPHARMACYNOTES'.

In addition, the new Report Builder field RTMemo[2] has been added to enable you to modify your Rx Forms to use the new Pharm. Notes field.

Note: Pharmacy notes are sent electronically without any setup being necessary. However, if you want notes to show in printed Rx, please log a Support case through the Greenway Customer Community.

Rx Details Dialog Box Modifications

This section describes the modifications that have been made to fields on the Rx Details dialog box. Several fields have been moved or renamed, and the Sig. field now has enhanced functionality.

First, to make room for the new Pharm. Notes field, the DAW (Dispense As Written) field has been moved up, so that it is now below the Units field. In addition, the following fields have been renamed:

In addition, the Sig field now features enhanced functionality. When you make a selection in the Sig field, the Intergy EHR system now displays those instructions in the Pat. Instruct. field. You can leave those instructions as they appear, or you can edit them.

Note: Only the text in the Pat. Instruct. field is sent to the patient. This ensures that the patient receives only one set of instructions, rather than possibly receiving conflicting instructions from two different sources (fields) in the Intergy EHR system.

Prescriptions Secondary Pharmacy for Patients

Secondary Pharmacy for Patients

In Intergy and Intergy EHR, patients now can have both a primary pharmacy and a secondary pharmacy. (Previously, only a single pharmacy could be entered for each patient.) Being able to enter both a primary and a secondary pharmacy is very useful when a patient has both a local pharmacy and a mail-order pharmacy that are used in different situations, such as a mail-order pharmacy for chronic conditions and a local pharmacy for acute conditions. In addition, some patients might use more than one pharmacy based on where they can get the best price on prescriptions. Other patients might use one pharmacy most of the year, but use another pharmacy seasonally, such as if they spend the winter or summer in a different area.

The primary location for entering a patient’s pharmacy information is your Intergy system. However, you can also add, edit, or delete a patient’s pharmacy in the practice information section of the Patient Info List in your Intergy EHR system.

• If a secondary pharmacy exists for the patient, then that secondary pharmacy becomes the new primary pharmacy and is displayed in the patient’s information.

• If the patient does not have a secondary pharmacy, then the message “No Default Pharmacy” displays in the patient’s information.

Viewing Transmission Notes for a Medication

When viewing a patient’s medications on the Meds tab, you now can view transmission notes for prescriptions that were transmitted electronically. For example, you can view the times that transmission messages were sent and received, or see if any errors occurred during transmission.

If you need additional information about a note, you can click the link in the Note column to display technical information about the transmission message. This information can be helpful when troubleshooting any problems that might have occurred during the electronic transmission of the prescription.

Following is an example of the Meds tab for a patient, showing the Transmission Notes link in the Pharmacy section:

Prescriptions Viewing Transmission Notes for a Medication

When you click the Transmission Notes link, the Transmission Notes for Prescription dialog box displays.

The Note column describes each action that occurred during electronic transmission of the prescription, such as transmission messages being queued, sent, and received.

To view more information about a note, click on the blue link in the Note column. The Activity Notes message box displays more technical information about the transmission message, such as error codes when applicable. The additional information can be helpful when troubleshooting problems that might have occurred during electronic transmission of the prescription.

When you have finished viewing the Activity Notes, click the Close button.

Vitals WHO Growth Charts

WHO Growth Charts

You can now view World Health Organization (WHO) growth charts for infants and children from birth to 2 years of age (0-24 months). This allows you to use WHO growth standards for infants and children from birth to 2 years of age (0-24 months) as recommended by the Centers for Disease Control and Prevention (CDC).

Previously, only CDC growth charts were available for infants and children from birth to 2 years of age (0-24 months). Now, both CDC and WHO growth charts are available.

The new WHO growth charts have been added to the Growth Charts window. If the current patient is from birth to 2 years of age (0-24 months), the WHO growth charts are displayed by default.

Additionally, WHO and CDC radio buttons have been added in the lower portion of the window below the growth chart. This allows to you to select whether to display WHO or CDC growth charts for a patient who is from birth to 2 years of age (0-24 months). If the

Vitals Percentiles

You can now view percentile values for the height, weight, and head circumference vitals measurements for pediatric patients in the Today’s Vitals section of the Patient Summary.

Previously, only the measurements for weight, pulse rate, and blood pressure were displayed. Now, in addition to the measurements for weight, pulse rate, and blood pressure, the percentiles for the weight, height, and head circumference measurements of pediatric patients are displayed. Additionally, when you hover the pointer over the percentile section, the section changes to a View Growth Chart button. This allows you to open the growth chart for the patient by clicking the button.

The new percentiles section has been added to the Today’s Vitals section of the Patient Summary.

The height and head circumference percentiles are only displayed for patients up to 3 years of age.

For patients under the age of 2 years, the WHO growth charts are used to calculate the percentiles. For patients over the age of 2 years, the CDC growth charts are used. Note that the head circumference measurement applies for only patients 2 years old or younger.

When more than one set of vitals measurements is entered for today’s date, only the values for the most recent measurements and percentiles will be displayed. The percentiles panel only displays for pediatric patients (under the age of 18 years).

If a vitals measurement of height, weight, or head circumference is missing, a short line will be displayed under the heading to indicate that there is no measurement value for the percentile.

Medication History Consent

You can now enter and view the medication (Rx) history status of a patient’s consent concerning the practice’s use of protected health information (PHI) for treatment, payment, or operations (TPO).

This provides additional patient consent options and information for medication history.

Previously, only one patient consent status was available to be assigned and viewed for a patient.

Now, in addition to the main consent status, a medication history consent status is also available.

The new Rx Hx Consent field has been added to the Intergy Patient Registration Patient Demographics page. This allows you to enter the medication history consent status when registering a new patient.

On upgrade, the default value is based on the patient’s main consent status.

• If the patient’s main consent status is ‘No’, the Rx History consent status will be defaulted to ‘No’.

• If the patient’s main consent status is any other value than ‘No’, the Rx history consent status will be defaulted to ‘Yes’.

You can now show additional Pharmacy Benefit Manager (PBM) prescription (Rx) history information for a patient.

This allows you to view additional prescription information that is not displayed in the PBM Rx History window. For example, the prescription number is not displayed in the PBM Rx History section, but you can view the prescription number for a medication by showing the history details.

Person and Patient Preferred Name

You can now enter, edit, and view a preferred name for a person or patient.

This allows you to identify the preferred name for a person or patient and address the person or patient by the preferred name.

Previously, a preferred name for a person or patient was not available.

Now, a preferred name can be added for a person or patient and viewed so that staff can use the preferred name when addressing the person or patient.

The following enhancements have been made for the person and patient preferred name:

• The new Preferred field has been added in Patient Registration (Patient Demographics page). This field is used to enter the preferred name given by the person being registered as a patient that the person would like used when being called or addressed. The field is available when registering or quick registering a patient.

Social Security Number Masking in Patient Search

You can now specify the type of display of patient Social Security Number (SSN) in patient search dialog boxes in the Intergy and Intergy EHR systems.

This provides you with options for protecting the security and privacy of your patients’ SSNs by controlling the information that may be displayed on-screen.

Previously, there were no options for setting the display of patient SSN.

Now, you can select the SSN information that will be shown or masked in patient search dialog boxes, such as the Select Patient for Patient Information dialog box in the Intergy system and the Select Patient dialog box in the Intergy EHR system.

The new Display SSN in Patient Search parameter has been added to Practice Configuration in Practice Administration and System Configuration in System Administration. This allows you to specify the type of display of patient Social Security Number (SSN) in patient search dialog boxes in the Intergy and Intergy EHR systems.

You can select one of the following options:

• SHOWALL – Display the entire SSN. For example, a patient SSN will be shown as ‘123-45-6789’.

• SHOWNONE– Mask the entire SSN. For example, a patient SSN will be shown as ‘xxx-xx-xxxx’.

• LASTFOUR– Display the last 4 digits of SSN. For example, a patient SSN will be shown as ‘xxx-xx-1234’.

The parameter is shipped with a default value of 'LASTFOUR'.
