The packet you now have in your hands is an overview of our program. There are four major components of
this packet:
Instructions for registering with our World Headquarters offices
Instructions as to how to develop your 5 year Ordination Goal and The 5 Year Contract

Overview of our 5 year program
What you must do at this Phase of your journey: Carefully review and consider the entire packet. If you have questions please ask your pastor
or contact the district for assistance.
Register with the Wesleyan Headquarters’ department of ministerial development
Complete the 5 year contract
Submit the contract to the district office by February 28th to be registered in the upcoming
year’s program
Once the district office receives your 5 Year Contract we will send to you an intake packet which must be
completed by March 31st to be enrolled in the upcoming year’s program.
Instructions for Enrolling with Headquarters
As a prospective student you MUST enroll with our Wesleyan Headquarters office in Indiana. This is a vital
step in your ministerial preparation as they are the clearinghouse for all your academic course work and pro-
vide us with your academic transcripts as well as register you in the official student database. If you are not
registered with Headquarters we are unable to accept you into our program.
Creating a student profile and registering is very simple. Just click (or enter) this link and you will be taken
right to the page where you can complete your online headquarters' registration process:

You will need a payment method as they charge a $20 enrollment fee.
BIG TIP: While this office tracks your transcripts you need to be personally responsible to track your progress
as well. While they do an excellent job, it must be kept in mind that they are working with thousands of stu-
dents and from time to time records can get confused or mixed up. Each semester you finish course work keep
a hard or electronic copy of your semester grade report showing your completed courses, your professor's final
email on your completed course, or your semester transcript. This way you have a hard copy and proof of your
course work in case a complication arises down the road.
Instructions for “The 5 Year Contract”
What is the 5 year contract all about? 5 years is a fairly reasonable time to complete your ordination journey. Our entire system is set-up on this 5
year plan. While you can register with headquarters and begin taking classes at any time, we do not bring you
into our district program until you are within 5 years of your ordination. The reason for this is that while you
are finishing your educational preparations we want to begin walking with you in this journey. You will be
assigned a cohort group that will make this five year journey with you. As a cohort you will be working on a
very specific skill and development phase each year to help you prepare for ministry. We want you to enjoy
all 5 years and they are not meant to be repeated. We also want to see you graduate (or be ordained) with your
cohort class.
How to establish your five year target ordination date: Review the following Overview of our 5 year district program toward ordination
Review the following Academic Training Requirements of the Wesleyan Church
Register with the school of your choice (approved schools listed below)
Consult with your academic advisor as to a reasonable yearly course load
Use the Ordination Educational Worksheet (following pages) to help arrive at your goal date
Project course load out to make sure you can be done within the next five years
Consult with your mentoring pastor to make sure your plan is reasonable
Consult with your mentoring pastor to make sure he/she can facilitate your Supervised Ministry
Requirements
(The last two years of your preparation you must be in a ministry position (volunteer or paid)
within the local church unless you are in a qualified M-Div program. During these years you
will be assigned various pastoral duties and functions in which your mentoring pastor must
supervise and evaluate your activity. You cannot begin this portion of your training until you
have the 6 core classes completed and have moved into the ministerial student license category.)
Consult with the district intake team to make sure they believe you can complete your 5 year plan on time
IF YOU DO NOT BELIEVE you can complete this journey in 5 years, we suggest you register with Head-
quarters and begin coursework but wait until the next year to make application to our district program. There is
a $700 fee to begin our process. If you complete the 5 year contract and complete your ordination when you
take your first assignment preferably within the district, we will reimburse you the registration fee. If you fail
to complete the 5 year contract, you will forfeit the fee and may be reduced to beginning the process again.
Therefore, we ask that you give careful consideration to the 5 year contract. After reading through these mate-
rials, if you still need help establishing your 5 year target, we are more than happy to help.

Seek your local pastor for guidance on the call and he will give you:
A welcome letter from DMBD chairman
A Guide to Ministerial Application
An Initial Application to Ministerial Training from the Greater Ohio District
If not already, become a full member at your local church.
Complete the Initial Application
Schedule with your local pastor a meeting to review your application and discuss your call to ministry
Once you have met, have the pastor or lay representative (suggested vice-chair) sign your application

Fill out the 5 year contract and send it back to the district (contact information above)
(It is highly suggested that this contract be completed and post marked by
February 28th to have time to fully enroll in the program for the upcoming
year which begins in May.)
Comprehensive 5 Year Plan
Phase 3: Upon receipt of your 5 year contract (if you are within 5 years of your ordination)
You will be sent a complete Intake Packet
Personal Data Inventory Questionnaire (DBMD #1)
Professional References (with instructions and questionnaires) (DBMD #2)
Credit check and Criminal background check form
Legal Citizenship form
Complete a 2 page paper (APA styling) explaining your call to ministry
Complete entire Intake Packet
Send completed packet back to the district office by March 31 to qualify to be entered into our 5 year
program for the upcoming year (which begins each May)
Phase 4: Upon completion of Intake Packet the DBMD chair will set up a structured interview and initial
assessment with an “Intake Team”—Including some of or all of the following: DMBD chair, a
member of the DMBD counseling team, district superintendent and/or other DMBD members.
Complete a Myer’s Briggs Assessment
Set up with our counselor to take an MMPI (Minnesota Multiphasic Personality Inventory)
Sponsoring pastor must bring the candidate to the structured interview and sit for a reference discus-
sion following the student’s interview with intake team.
Pay, in full, the initial investment fee of $700 (based upon current pricing schedule—subject to
change). If you complete the entire process on time, this fee will be reimbursed to you once you are
ordained and take your first assignment, preferably within our district.
Phase 5: Pat yourself on the back because you just completed the hardest part of the paperwork toward
ordination.

(Student will not be admitted into our program until all requirements are met) Once a student passes initial intake assessment they will be:
Assigned to a cohort
(cohorts remain together for the duration of the process and are ordained together.)
Assigned a pastoral mentor (sponsoring pastor is preferred)
Cohort assigned a DMBD sponsor (Sponsor will also remain with cohort for their 5
year process)
