V. Reports of Officers/Staff/Forums

President

A. Mercatante reported that the Public Health Advisory Council (PHAC) met Friday,

August 10 and three subcommittees have been formed: accreditation, funding, and Health

in All Policies. There are concerns PHAC members have been appointed to the funding

subcommittee with no knowledge of local health department operations or funding. It was

requested that S. Hall, as the incoming president of MALPH, be appointed to that

subcommittee. Questions were asked about the FY 2019 Essential Local Public Health

Services dollars distribution. There is new money, and the old formula will be used;

however, local health departments have not received notice of allocation, which is

concerning. M. Polverento has asked for a few volunteers to discuss and define

“population health”.

Secretary/Treasurer

N. Derusha reported the budget remains on target. The FY 2019 proposed budget will be

available for the October meeting. Motion by R. Burns, support by K. Hughes to accept

the financial report.

Executive Director

M. Swain reported the quarterly leadership discussions with each department. The October

board meeting will be in Bay City on Tuesday, October 9 at 11:00am. State partners will

be invited to discuss PFAS. There was a PFAS webinar, and that will be the basis for

discussion at the board meeting. She shared that local hospitals may contact local health

departments to assist in a water management plan to address Legionella exposure.

Maternal Infant Health Program (MIHP) is seeking two health officers and two nurse

administrators to perform a time and cost study analysis. M. Swain reported that NACCHO

leadership will be discussing filing an amicus brief in the cases against Dr. Eden Wells and

Nick Lyon. The election for MALPH’s executive committee members ends September 19,

2018. M. Swain will be attending the Public Health Law Conference from October 3-6th.

Lobbyist

No report at this time.

Forum Reports

Health Education and Promotion (HEP): J. Weisbrod reported they continue to work

on the Public Health Endorsement for undergraduate and graduate students. There will be

10 modules to complete. A survey has been developed and will be distributed to HEP

forum members, along with Public Information Officers to review the modules.

Public Health Finance and Administration: J. Dean reported that the forum is working

on an Egrams timeline and working with MDEQ about the functionality of Egrams, as they

are considering using it. She also reported that the Admin Conference will be the end of

the week, with a billing component to increase hearing/vision Medicaid billing.

Environmental Health (MALEHA): K. Schweighoefer reported that PFAS has been the

focus of many local health departments. They are also continuing to monitor HB 5752 and

HB 5753 (statewide sanitary code). MALEHA is working on a vapor intrusion toolkit that

will be discussed and distributed at the Environmental Health Director’s Conference. In

addition, they are monitoring the recreational marijuana ballot initiative and impact on

(adulterated) food. They are expecting to see work completed on the Food Code and Food

Law and work with MDARD.

Public health nursing (NAF): D. Reed reported that they worked on NAF’s strategic plan

during the last meeting. Also, they are getting educated on PFAS.

Management information systems: There was no report at this time.

Public health physicians (MAPPP): T. Frankovich reported that MAPPP has drafted a

letter to Rep. Brandt Iden regarding Tobacco 21 (HB 4736). They are doing final edits

on gun violence resolution. They also discussed the PFAS webinar.

VI. Adverse Childhood Experiences Presentation

N. Britten, MPH presented the Adverse Childhood Experiences, equity, and resilience and

the role of the local health department.

VII. Public Health 101 Presentation

Jeanette Ball and Jessie Jones of the Michigan Public Health Institute presented on the

Public Health 101 project they have been working on as part of a (Southeast) Cross

Jurisdictional Sharing grant.

VIII. Reports from State Departments

Department of Agriculture and Rural Development (MDARD)

S. Dunleavy reported that they are working on the impact of PFAS on Michigan’s

agriculture. He also reported that they are concluding the fair pilot project for the second

year. They are looking at vitamin shops that are making smoothies and the role of the local

health department to inspect. They continue to meet with LARA regarding medical

marijuana. They are currently using Cottage Food guidance, but this will address high risk

foods. They continue to build a FAQ sheet. They expect to resume work on the Food

Code including 64 items.

Department of Environmental Quality (MDEQ)

D. Debruyn reported that the department has worked on Septic Smart Week promotional

materials. www.michigan.gov/deqsepticsmart. They are also working with local health

departments to schedule annual evaluation in the non-community water supply program

(January through March). There will be well construction training for local health

department staff. They continue to work on PFAS testing and notification.

Department of Health and Human Services (MDHHS)

M. MacQueen reported that while Hepatitis A incidence has slowed, the CHECC is still

operational. She stated that the situation/vaccine grid is due September 20. They are also

collecting end of year deliverables report. They continue to do San Bernadino follow up

regarding active shooters and monitoring opioid surveillance.

O. Todd reported that they continue to work on crisis communication regarding PFAS. He

also provided an update on the Hepatitis A outbreak. He reminded members of the

USALEEP release today. Local sharing grant reports are due soon, and there will be a pre-

conference session at the Michigan Premier Public Health Conference. He is aware that

all LHDs will be impacted by PFAS in some way. An MPART invite will be sent to all

local health departments. He reported that school data is coming in from PFAS testing.

MDHHS staff (toxicologists, Kory Groetsch, and Steve Crider) are all available to help

LHDs. MDEQ is to develop a plan for communication between the two state departments

and local partners. He extended the deadline for the MDHHS Director’s Award. He

reported that they are still working on the Essential Local Public Health Services formula.

By February 2019, MDHSS is to submit to the legislature the formula they intend to use

for the new monies for FY 2020.

Health (BOH) to order at 8:45 a.m. on Wednesday, September 26, 2018 at the Gratiot County Branch Office of

the Mid-Michigan District Health Department (MMDHD), Ithaca, Michigan.

A. Public Comment – None.

B. Consideration to Go into Closed Session for Purposes of Labor Negotiations

B. Kellenberger requested a roll call vote to go into closed session at 8:45 a.m. C. Partlo called the

roll: Sam Smith – Yes, Dwight Washington – Yes, Betty Kellenberger – Yes. Motion carried 3-0.

C. Michigan Nurses Association (MNA) Contract Negotiations

B. Kellenberger indicated that an agreement had been reached between the MMDHD and MNA.

Motion made by B. Kellenberger and seconded by D. Washington to recommend that the full

BOH approve the MNA Contract for the period of October 1, 2018 through September 30, 2021 as

presented. Motion carried.

Motion made by B. Kellenberger and seconded by S. Smith to recommend that the full BOH

approve the wage increase for FY 18/19 for the non-union employees as proposed. Motion carried.

E. Health Officer’s Employment Agreement (Tabled from August Regular Meeting)

Motion made by B. Kellenberger and seconded by D. Washington to recommend that the full

BOH approve the Health Officer’s Employment Agreement for FY 18/19 as proposed. Motion

carried.

order at 9:04 a.m., on Wednesday, September 26, 2018, at the Gratiot County Branch Office of the Mid-

Michigan District Health Department (MMDHD), Ithaca, Michigan.

M. Cheatham stated that Representative Moolenaar acknowledged the work being

done by the Clinton Substance Abuse Prevention Coalition through a Health

Resources and Services Administration (HRSA) grant.

a. MMDHD’s Expenses for August 18 through September 14, 2018

Motion made by T. Lindeman and seconded by B. Kellenberger to approve

payment of the MMDHD’s Expenses for August 18 through September 14, 2018,

totaling $460,286.47. Motion carried.

b. MMDHD’s Monthly Balance Sheet, Revenue and Expenditure Report for August

2018

Motion made by T. Lindeman and seconded by B. Kellenberger to approve and

place on file the MMDHD’s Monthly Balance Sheet, Revenue and Expenditure

Report including the Amended Budget for August 2018. Motion carried.

c. Transfer of Funds from the Ingham Health Plan Corporation

M. Cheatham explained that MMDHD had contracted with the IHPC to do contracting

and billing for the Community Health Worker (CHW) Mid-Michigan Pathways

Program stating that the IHPC was unable to obtain remunerative contracts with the

health plans; therefore, the relationship with them has been severed. He said that the

agency was now running the Mid-Michigan Pathways Program on its own. He asked

the BOH to request the remaining fund balance from the IHPC.

Motion made by T. Lindeman and seconded by B. Kellenberger to approve the

transfer of remaining MMDHD funds from the IHPC ($631,989) that was utilized

for the Mid-Michigan Pathways Program. Further, the BOH approved

restricting the funds for the Mid-Michigan Pathways Program. Motion carried.

d. Document Destruction Services

M. Cheatham said that the topic was informational. He said that C. Partlo oftentimes

finds ways for the agency to save money and found a new vendor for the agency’s

document destruction services saving over $631 annually.

e. Retirement System Annual Report

M. Cheatham reported that the Michigan Department of Treasury thought that

MMDHD had an underfunded Other Post-Employment Benefit (OPEB) and asked us

to fill out a request for a waiver. He explained that the BOH eliminated the benefit

November 1, 2016. He said that the BOH was required to review and approve the

waiver request.

Motion made by T. Lindeman and seconded by S. Smith to approve the

Retirement System Annual Report as presented. Motion carried.

f. Montcalm County Appropriation

T. Lindeman indicated that the Montcalm Board of Commissioners approved the

health department appropriation at approximately $38,000 less than requested. M.

Cheatham said Bob Clingenpeel, Montcalm County Administrator, had earlier

indicated that Montcalm County would pay the full appropriation this year; however

they included the contracted funds for the Resource Recovery Program as part of the

appropriation resulting in the deficit. He said that the agency is in the same position as

last year and requested the BOH approve EH surcharges for Montcalm County only,

the same as last year to cover the appropriation deficit.

Motion made by T. Lindeman and seconded by D. Washington to adopt an EH

surcharge of $45 on select new construction services in Montcalm County and a

$10 surcharge on all other services (excluding temporary food licenses for non-

profits and fees for registered septic installers) effective October 1, 2018. Motion

carried.

2. Personnel Committee – Betty Kellenberger, Chair

a. Update on Michigan Nurses Association (MNA) Contract Negotiations

M. Bowerman stated that the MNA ratified the Contract; however, they will need to

sign it first, so the Personnel Committee can sign the Contract next month.

Motion made by B. Kellenberger and seconded by D. Washington to approve the

MNA Contract for the period of October 1, 2018 through September 30, 2021 as

proposed. Motion carried.

b. Non-Union Employees’ Wage Proposal

Motion made by B. Kellenberger and seconded by S. Smith to approve the non-

union employees’ wage proposal as presented for the period of October 1, 2018

through September 30, 2019 and including the change in the credentialing policy

to mirror the language in the MNA contract starting October 1, 2018. Motion

carried.

c. Health Officer’s Employment Agreement

Motion made by B. Kellenberger and seconded by G. Bailey to approve the

Health Officer’s Employment Agreement for FY 18/19 as proposed. Motion

carried.

3. Program Committee – Bruce DeLong, Chair – No report.

4. Mid-Central Coordinating Committee – Tom Lindeman, Vice Chair – No report.

5. Well and Septic Stakeholders Committee – Marcus Cheatham

a. Public Hearing for Septic Management Ordinance, Monday, October 15, 2018, 6 p.m.

at the Ithaca Branch Office

M. Cheatham reminded the BOH that a Public Hearing has been scheduled for

October 15, 2018, 6 p.m. at the Ithaca Branch office to seek public input on the Septic

Management Ordinance. He reviewed the process stating that the BOH must hold a

Public Hearing, and then the Counties can hold a public hearing if they choose, or

consider the ordinance in a regularly-scheduled meeting. G. Bailey mentioned that

Gratiot County Commissioners approved the language at their last meeting; however,

would need to adopt the ordinance at their next meeting.

F. MEDICAL DIRECTOR’S REPORT: Jennifer E. Morse, MD, MPH, FAAFP

1. Shiga Toxin Producing E.coli (STEC)

Dr. Morse reported on E.coli stating that there have been a few cases within the Central

Michigan District Health Department and District Health Department #10. She explained the

different strains, risk factors and how to prevent getting ill. She mentioned that persons,

especially children are particularly susceptible and should wash hands after touching animals

(farms, petting zoos, fairs), and do not swallow water from waterparks. She also said that

produce should be washed well and unpasteurized cider should be avoided.

2. Flu Shots Offered

The BOH demonstrated their support of seasonal influenza prevention through vaccination by

receiving the vaccine from Lisa Mikesell, Public Health Nurse.

Dr. Morse reviewed her BOH Monthly Healthy Living Recommendations for October and asked the

BOH to adopt them:

1. While most E. coli are normal and harmless residents of our colon, some strains can cause

serious, even deadly, illness.

2. Children are very susceptible to serious illness due to STEC and HUS. They also do many things

that put them at risk. Teach them the habits that will keep them safe.

Motion made by G. Bailey and seconded by D. Washington to adopt the BOH Monthly Healthy

Living Recommendation for October as proposed. Motion carried.

G. HEALTH OFFICER'S REPORT: Mark W. (Marcus) Cheatham, Health Officer, Ph.D.

1. Local Public Health Salary Survey

M. Cheatham reported that as part of negotiations with the MNA, the membership

requested the Administration compare MMDHD nurse’s salaries to others. M. Cheatham

offered instead to ensure that the agency conduct a comprehensive salary survey for all

positions. He mentioned to the BOH that the agency could work with MALPH to conduct

an analysis of public health wages. He mentioned that this project would be incorporated

into the agency’s Strategic Plan.

2. Citizens Research Council of Michigan, An Ounce of Prevention: What Public Health

Means for Michigan, August 2018

M. Cheatham reviewed An Ounce of Prevention: What Public Health Means for

Michigan, stating the report says that the State has invested little more than what was

needed to draw down Federal funding for public health. The report says Michigan public

health funding has dropped by 16% from 2004. The report also calls out Michigan’s

fractured public health system. He reported that the Governor’s Public Health Advisory

Committee is considering recommending the reunification of State departments. He

summarized that the report was provided for information only.

H. OLD BUSINESS:

1. PFAS Update

L. Braddock provided an update regarding the PFAS contamination stating that sampling has

been completed in all of the three counties. Gratiot County’s test results are now being

received. She reported that the Ithaca Sanitary Landfill residential well sample results should

be received in 3 to 4 weeks. L. Braddock said that a public meeting announcing the test results

is planned.

2. Update on Hepatitis A Outbreak Response

No report.

3. MMDHD Rebranding Update; Approval of New Logo

M. Bowerman provided an update regarding the agency’s rebranding efforts stating that the

Rebranding Committee selected a new logo for review and approval by the BOH.

T. Lindeman expressed concern regarding the multiple colors used in the logo, stating that a

six-color press would be needed to print documents outside the agency and extra costs would

be incurred. After discussion, the BOH requested that the Rebranding Committee check with

the vendor on if there would be extra costs for printing the logo on items and publications

produced outside the agency.

Motion made by G. Bailey and seconded by B. Kellenberger to approve the new agency

logo allowing the Rebranding Committee to select the best option pending the colors

chosen and the cost to print the logo. Motion carried.

4. Emory University PBB Reduction Research Study

M. Cheatham reported that Emory University visited Gratiot County to work on two studies

1) Orlistat, a drug to remove PFAS from people’s bodies; and 2) Father Study – a study to

determine if a grandfather exposed to PBB affects subsequent generations. He also said that

Emory received a large, five-year NIEHS grant to strengthen the PBB Registry. Dr. Morse

reported that Emory University is working on a CME Program for providers in the area to

educate them about PBB.

Emory University is also working with the University of Michigan School of Public Health to

design educational materials for legislators regarding PBB and PFAS. Central Michigan

University has funded work with the citizen’s action group in St. Louis to create oral histories

of those that lived through the PBB disaster.

M. Cheatham reported that Michigan Department of Health and Human Services (MDHHS)

now knows that Emory University has been funded for the five-year cohort study utilizing the

registry. He said that MDHHS will not allow Emory University to send mailings to those in

the registry and is requiring deceased members in the registry to be removed. MDHHS has

agreed to allow the Centers for Disease Control and Prevention (CDC) to render an opinion

about these issues regarding the PBB Registry.

I. NEW BUSINESS:

1. Michigan Breastfeeding Network Award

A. Tabor explained that the agency received the silver level award from the Michigan

Breastfeeding Network for being a breastfeeding friendly workplace. Additionally, she

reported on the International Board Certified Lactation Consultant® (IBCLC®) Program,

stating that services are beginning to pick up. Angie Martin averages about three visits per

week.

3. Emerging Issues

a. Type I Water Systems for Rural Villages

L. Braddock received a complaint from Riverdale regarding failed or nonexistent

septic systems are draining into a village or township drain. In order to find the failed

or nonexistent septic systems, the Michigan Department of Environmental Quality

(MDEQ) has asked the township to scope out the drain. L. Braddock reported that two

failures have been confirmed and are attempting to obtain access to another home to

confirm a failure. The MDEQ is coming back to Riverdale in October to investigate

the river to see if there are any other illegal discharges. She summarized that the

MDEQ was considering requiring Riverdale to establish a community septic system to

serve residents.

b. Marijuana Information

M. Cheatham reviewed the information provided on marijuana stating that the

graphics presented in the BOH materials are a part of a social media campaign to

educate the public about the consequences of unregulated access to recreational

marijuana if the ballot initiative passes in November. Educational materials are being

actively circulated in Clinton and Montcalm Counties. Dr. Morse was asked to

coordinate the campaign between the health departments (CMDHD, MMDHD,

DHD10). She reported that a press release has been distributed to the media, and

information shared with nurses and physicians. John Kroneck, from Cherry Health

Promotions, has sent a letter to school superintendents in hopes that it would be

distributed to parents.

Food Insecurity and Food Deserts

It is estimated that over 12% of U.S. households dealt with food insecurity at some point in 2016; in rural areas, over 15% were food insecure. According to the Life Sciences Research Office of the Federation of American Societies for Experimental Biology, food security and insecurity are defined as:

Food security: access by all people at all times to enough food for an active, healthy life, and includes, at a minimum: o the ready availability of nutritionally adequate and safe foods, and o an assured ability to acquire acceptable foods in socially acceptable ways (e.g., without resorting to

emergency food supplies, scavenging, stealing, or other coping strategies).

Food insecurity: limited or uncertain availability of nutritionally adequate and safe foods or limited or uncertain ability to acquire acceptable foods in socially acceptable ways.

A food desert is defined by the United States Department of Agriculture (USDA) as “parts of the country vapid of fresh fruit, vegetables, and other healthful whole foods, usually found in impoverished areas”. An area that has limited access to healthy options yet has no lack of access to unhealthy foods, such as fast food, convenience stores, and liquor stores, are referred to as food swamps. Food insecurity is associated with poor health and chronic illness. People that worry about feeding themselves or their families suffer from stress, anxiety, and depression. They are also more likely to be overweight, have poor sleep, chronic disease, and a worse quality of life. This may impact their ability to work, which adds to their struggle with food insecurity. Rural residents are known to have higher rates of chronic disease, disability, and obesity, as well as a lower life expectancy, as compared to urban residents. Approximately 6% of rural U.S. residents have to travel between 10 to 20 miles to get to the nearest supermarket. In rural areas, those that struggle with limited access to food feel that lack of transportation is their biggest barrier. This causes many to rely on more expensive and less nutritious foods, such as is available at nearby gas stations and convenience stores. Children, pregnant women, seniors, people with disabilities, and immigrants and refugees are at higher risk of poor outcomes due to food insecurity. Children dealing with food insecurity struggle with worse school performance, increased school absenteeism, worsened social skills, stunted growth, obesity, and deficiencies such as iron-deficiency anemia. Older adults dealing with food insecurity have high rates of physical and mental deterioration, while pregnant women have more excessive weight gain and gestational diabetes. There are many factors in addition to lack of transportation associated with a higher likelihood of dealing with food insecurity. These include low income, lower education, immigration, depression, anxiety, physical and mental disability, gambling, drug use, number of people in a household, available food storage and preparation areas, food cost, store location, and single-parent households. The food supply chain has many steps: production, processing, distribution, retail and market, consumption, and waste recovery.

Gaps anywhere in this chain will affect access to healthy food. Ways to improve and impact food production, processing, and distribution include:

Land Use Policies: policies offering financial credits to food producers can ease the burden of starting and maintaining farmland.

Food Hubs: processors and/or distributors for small farms, which can help them make a profit without expanding their production or land size.

Community Gardens: which provide residents with a space to grow food. School programs exist to help children and adolescents have access to healthy foods. Over 44 million meals are served in U.S. schools each day. School-based models include:

School Breakfast: These programs have been shown to increase attendance, decrease tardiness, and provide quality nutrition to students who lack food at home. Participating students have improved standardized test scores and lower behavior problems during the day. This program is federally reimbursed and many students qualify for free and reduced priced meals.

Afterschool Meals: provided after the school day but during a supervised, educational activity. Afterschool meals are normally implemented through the Child and Adult Care Food Program (CACFP) or the National School Lunch Program.

Farm to School Models: a national program focused on increasing the amount of local, fresh, and healthy foods in schools. The U.S. Department of Agriculture provides grants and funding to help implement Farm to School programs.

Backpack Food Program Models: This program helps to close the gap in meals for children between Friday lunch and Monday breakfast by providing food for children over breaks. Provide kids with whole grains, fruits, vegetables, and micronutrients and is typically implemented with assistance from a sponsor, such as a food bank or local business.

Summer Meals: Provides lunches to students at little or no cost during the summer break. This program increases access to food throughout summer months for students who rely on school meals. Summer food programs can be located at schools or community agencies such as libraries, faith-based organizations, or community centers.

School Food Pantries: schools devote space specifically for a food pantry to provide relief to hungry students and staff.

Retail-based models focus on providing or improving places where people buy food. Retail-based models include:

Food Assistance: This includes programs such as Supplemental Nutrition Assistance Program (SNAP) and the Special Supplemental Nutrition Program for Women, Infants, and Children (WIC).

Mobile Markets: These work similar to food trucks and have a set schedule and route where they make stops and deliveries. Transportation barriers are reduced by bringing a food market to rural and underserved areas. The USDA authorizes the use of SNAP and WIC benefits at mobile markets.

Healthy Corner Stores (or convenience stores): Helps small food retailers stock more fruits and vegetables. Programs that increase store capacity to sell and market healthy food and beverage items, provide training and technical assistance for store owners, in-store community nutrition lessons, youth education efforts in schools near targeted corner stores, and connecting store owners with community partners, local farmers, and fresh food suppliers to encourage healthy food sourcing.

Farmers Markets: organized, scheduled, recurring gatherings where producers can sell their food products directly to consumers. Participating farmers markets accept SNAP and WIC benefits.

Community Supported Agriculture: direct farmer to consumer market, similar to farmers markets. Consumers buy “shares” in the harvest and prepay before the growing season for a certain amount of produce and/or other farm products during the harvest season. Once the food is ripe or produced, the farmers deliver the food directly to the consumer or a predetermined pick-up site, or customers pick up their shares at the farm. Participating farmers can accept SNAP and WIC benefits.

Recommendations:

1. Food insecurity is more common in rural areas and is known to have many negative impacts on health. 2. Food insecurity is a complicated, multi-factorial issue; there are numerous programs and resources

available to address different aspects of the problem.

Community Health and Education Division Programs

WOMEN, INFANT AND CHILDREN (WIC) NUTRITION PROGRAM: The WIC program is a federally-funded health and nutrition program designed to improve health outcomes for eligible women, infants and children. This is accomplished by providing nutritious food, nutrition education, breastfeeding promotion and support, and referrals to health and other services. Each year, the clients spend approximately $2.7 million worth of benefits in the local stores and food panties throughout our district. FLUORIDE VARNISH PROGRAM: The Fluoride Varnish program provides dental screenings and fluoride varnish applications to children 0-5 years of age. Fluoride varnish is a material that is painted on teeth to prevent decay. The services are provided in the WIC clinics, Head Start centers and Great Start Readiness Preschools. FAMILY PLANNING (FP) PROGRAM: FP services help women and men determine if and when they want to have a child. Services include screening for breast and cervical cancer, sexually transmitted infections (STI’s) and other health issues, in addition to providing a full range of birth control information and supplies. The program promotes maternal health through client education, counseling and referral to community resources for social and medical issues. Services are strictly confidential and provided on a sliding fee scale. CHILDRENS SPECIAL HEALTH CARE SERVICES (CSHCS): The Mid-Michigan District Health Department (MMDHD) serves as the local representative for the Michigan Department of Community Health CSHCS program. CSHCS are for children and some adults with special health care needs. Family-centered services include case management and care coordination to assist families in accessing and navigating complex medical specialty systems. Clients are assessed and referred to community resources. INTERNATIONAL BOARD CERTIFIED LACTATION CONSULTANT®: Providing expert breastfeeding support and education. Serving pregnant and breastfeeding women with or without insurance. Client can be seen in our office or the comfort of their own home. Lactation consultant has personal breastfeeding experience. IMMUNIZATION PROGRAM: The Immunization Program provides vaccinations for preventable diseases for children, teens and adults. The Vaccines for Children (VFC) program provides vaccines at no cost to eligible children, from birth through 18 years of age, who are on Medicaid, have no health insurance, or meet other qualifying conditions. ACUTE COMMUNICABLE DISEASE/TB CONTROL PROGRAM: The Communicable Disease (CD) program provides surveillance, reporting, controlling and preventing infectious diseases in the community. The TB Control program provides diagnosis, treatment and prevention of tuberculosis in cooperation with the family physician and or the MMDHD Medical Director. Medication is provided for treatment and prevention of TB.

SEXUALLY-TRANSMITTED INFECTIONS (STI)/HIV COUNSELING, TESTING and CONTROL PROGRAM: STI services include diagnosis, treatment, counseling and partner notification for STI exposures. STI prevention education is provided with the goal of decreasing costly complications from infections, and saving substantial public and private healthcare dollars. Services are strictly confidential. Anonymous and confidential HIV counseling and testing is also provided. Clients needing treatment or follow-up are referred to the regional provider of these services. HEARING SCREENING PROGRAM: By law, all children must be screened for hearing at least once between the ages of three and five years old, in kindergarten, second, and fourth grades. Early identification of hearing problems can prevent developmental, social and emotional delays. Children identified with hearing problems are referred to physicians for further evaluation. VISION SCREENING PROGRAM: Vision screening takes place at least once between the ages of three and five years old, first, fifth, and seventh grades. Early identification of vision problems can enhance academic achievement and prevent permanent loss of vision. Children identified with vision problems are referred for follow-up. PATHWAYS TO BETTER HEALTH: Home visits by a Community Health Worker to provide referrals to community services, link clients with primary care and preventive health care services, as well as reduce unneeded hospitalizations and emergency room visits. The goal is to connect clients to services to enhance health outcomes.

Environmental Health Division Programs

FOOD SERVICE SANITATION PROGRAM: Periodic inspections of food service establishments, and mobile food units for compliance with public health standards; includes recommendations for licensures, review of plans and specifications for new and remodeled establishments, educational activities, and inspections of temporary food service events. ON-SITE SEWAGE DISPOSAL PROGRAM: Assures the adequate and proper disposal of wastewater in areas not served by public sewage systems; accomplished through proper design, inspection and management of various methods of sewage effluent disposal. WATER QUALITY CONTROL (PRIVATE, PUBLIC, AND NON-COMMUNITY WATER SUPPLY PROGRAM): Protects sources of drinking water, assures proper plugging of abandoned wells, and assures private/public water supplies are constructed, maintained, and operated in compliance with state and federal safe drinking water supply standards. Inspections, consultations, and water system approvals are activities included within the program. Mid-Michigan District Health Department (MMDHD)

Grant Funding Activity

October 24, 2018

Michigan Department of Health and Human Services (MDHHS) – The MMDHD

participated in year three of a Cross-Jurisdictional Sharing grant awarded to District Health

Department #10 (DHD10). The agency was paid $2,155 for staff participation in a customer

service training held on August 3, 2018 in Big Rapids with Central Michigan District Health

Department, MMDHD, and DHD10.

Montcalm County Food Hub Grant – The MMDHD received funding in the amount of

$46,000 from the Greenville Area Community Foundation to improve the nutritional health

of Montcalm County residents by launching a non-profit organization called a food hub that

will buy fresh produce from local growers, bundle it together and sell it to institutions like

schools, restaurants, and hospitals. We plan to use part of this grant as local match to apply

for a $100,000 grant for planning for farm-to-school projects which could be District wide.

Clinton County Drug Free Communities (DFC) Grant – MMDHD was funded for year

five of this up-to-ten year grant in the amount of $68,000. This year the coalition has focused

on working with youth in the areas of suicide prevention, opioid abuse and the marijuana

ballot initiative. In year six, we become eligible to apply for the highest level of funding

allowed, $125,000.

Water Quality Improvements in the Flat River Watershed Grant – MMDHD pledged its

support valued at $30,000 for a grant proposal made by the Montcalm Conservation District.

MMDHD’s role would include mapping areas at high risk for septic system failure and

education and intervention with those property owners.

On-Site Review: In accordance with the historical local health department (LHD) review sequence, the Mid-Michigan District Health Department On-Site Review will occur the week of February 4, 2019. To prepare for the On-Site Review, you can access the Cycle 7 – 2019 Tool and materials on the Michigan Local Public Health Accreditation Program Website.

Pre-materials to MPHI: Your first step to prepare for the On-Site Review is to complete your Pre-materials (Health Department Information – Profile Information Form, Exit Conference requests, and the On-Site Review Schedule). Your pre-materials are due to MPHI on or before December 5, 2018, and can be completed in the Accreditation Web Module, please be sure to select Cycle 7 in the drop down menu in the ribbon along the top. You can find guidance on how to complete the pre-materials in the LHD Users Guide.

***Please complete the attached form and return to Kristy Medes at [email protected] no later than two weeks before your pre-materials are due, so that we can set up user accounts for staff from your health department. Once you complete the form and return it to Kristy, you and anyone else you indicate on the form will receive an email with a user name, and link to set your password for the Accreditation Web Module.

For reviewed programs, TA contact information is located on each page of the section-specific MPR Indicator Guide, as well as in Appendix VII of the Local Health Department Users’ Guide. Section 1 - discharge permit for any premises

1.1 The Health Officer shall require all sewage disposal systems in the District to operate under the

requirements of a discharge permit.

1.2 The Health Officer is authorized to require an inspection of any premises to ensure that it has a

sewage disposal system that is operating as designed and that prevents sewage from entering the

watersheds of the District. The Health Officer shall issue a discharge permit to any sewage disposal

system that meets the requirements of this code (Sections 3 and 7).

1.3 Application for Permits: An application for a discharge permit shall be made by the property owner

or authorized representative to the Health Officer at the following times:

a) When notified in writing by the Health Officer

b) Upon submittal of a septic system inspection report

or

c) As requested by the property owner

1.4 A discharge permit shall be issued automatically with the issuance of a final inspection of a sewage

disposal system permit.

1.5 Application Exemption: The Health Officer may exempt an owner from making an application for a

discharge permit if documentation can be provided that demonstrates the property has been vacant

or unused for a period of 36 consecutive months in a 5 year period.

1.6 A discharge permit shall be issued upon review and approval of a discharge permit application and a

septic system inspection report submitted with a pass designation by a Registered Septic System

Inspector as described in Section 3 of this chapter.

1.7 Transfer of Permits: Should the ownership for the property for which a permit has been issued

change, the permit maybe transferred to the new owner. Such transfer must be requested in

writing on forms to be provided by the Health Officer.

1.8 Rescinding of Permits: If conditions exist that indicate an imminent health hazard, the Health Officer

shall rescind a permit. A permit may be reinstated if corrective actions are completed and verified

by the Health Officer.

1.9 Denial of Permits: The Health Officer may deny the issuance of a permit under one or more of the

following conditions that has been identified by a Registered Septic System Inspector and reported

to MMDHD;

a) No sewage disposal system can be located that serves the dwelling

b) A portion of the sewage disposal system is not located on the property

c) The condition of the sewage disposal system is a hazard and a threat to public safety

d) Evidence of effluent discharging to the ground or in to any lake, river, waterway, county

drain or storm drain

e) Evidence of a sewage disposal field or tile system that will not effectively treat wastewater

f) Evidence of a direct connection between a septic tank and a waterway

g) Sewage failure exists at a location where public sewer connection is available

h) Imminent health hazard exists at a location where public sewer connection is available

i) The permit application contains false information that is material to the decision to issue a

permit

1.10 Correction Notice: The Health Officer may issue corrective orders where conditions exist that

prevent the issuance of a discharge permit.

1.11 Expiration of Permits: Any permit issued pursuant to the requirements of the preceding

sections shall be valid for the term of 10 years from the date of issuance, unless rescinded as

provided in Section 1.5 of these regulations. The Health Office shall issue a permit for a period of

less than 10 years if an inspection indicates marginal site conditions for supporting onsite

wastewater disposal or the system is at risk for failure.

Section 2 – System Maintenance

2.1 Pumper’s Reports: Licensed Septage Haulers shall provide pumper’s inspection reports to the

Health Department on a form provided by the Health Department when septic tanks are pumped as

part of periodic maintenance.

2.2 The Health Officer shall require a septic system inspection if a pumper’s report suggests a sewage

disposal system failure.

2.3 Alternative Sewage Disposal Systems: Existing Alternative Sewage Disposal Systems that continue

to meet Chapter 3, Section 5.2 of these Regulations shall not be required to obtain an additional

septic system inspection but will be required to apply for a discharge permit.

2.4 Properties that have a deed restriction indicating yearly maintenance requirements shall provide

copies of yearly maintenance to MMDHD. Maintenance reports shall be reviewed as part of the

property’s application for a discharge permit.

Section 3 – Inspection and Registration

3.1 Registration: All individuals performing work under this Regulation within the counties of Clinton,

Gratiot and Montcalm shall apply for and pay a registration fee to be registered with the Mid-

Michigan District Health Department as a Registered Septic System Inspector. Registration shall be

valid for 5 years from date of issuance.

3.2 The Health Department shall maintain a list of Registered Septic System Inspectors.

3.3 The Health officer may revoke the registration of a Registered Septic System Inspector when failure

to abide by Environmental Health Regulations occurs.

3.4 Persons seeking to become a Registered Septic System Inspector must demonstrate knowledge of

construction practices, operational standards and know the causes and indicators of On-site sewage

disposal system failures. Documentation of successful completion of a current training course

approved by the Health Department shall be presented to the Health Officer at time of Registration.

3.5 Candidates that meet the registration requirements will be required to attend a consultation

meeting with the Health Officer prior to the issuance of a Registration.

3.6 Periodically during each registration period, the Health Department shall conduct Quality Assurance

and Quality Control review of Registered Septic System Inspectors files and field work review.

3.7 Designated representatives of the Health Department who hold an RS or REHS credential or are

endorsed by the Health Officer may conduct septic system inspection as directed by the Health

Officer.

3.8 A Registered Septic System Inspector shall collect sufficient information through a site inspection to

determine whether a sewage disposal system is properly treating sewage effluent. The Health

Department will review this information and issue a pass or fail determination.

3.9 A Registered Septic System Inspector shall report the results of their inspection electronically to the

Health Department promptly following the date of inspection.

3.10 A Registered Septic System Inspector shall notify the Health Officer within 5 days if an imminent

public health hazards exists at a property being inspected.

3.11 A Registered Septic System Inspector who is also registered as a septic installer at MMDHD shall

not install a replacement septic system on a property without the homeowner or authorized

representative applying for a permit to construct, alter or extend a sewage disposal system

3.12 A Registered Septic System Inspector who is also registered as a Septic Installer at MMDHD shall

not install a replacement sewage disposal system on the same property in an effort to obtain a

discharge permit.

3.14 A Registered Septic System Inspector may perform limited work during an inspection at the

discretion of the homeowner with or without being a septic installer in MMDHD as long as such work is

limited to above ground or tank enhancement.

MID-MICHIGAN DISTRICT BOARD OF HEALTH

The Board of Health (BOH) approved the transfer of the remaining Mid-Michigan District Health Department (MMDHD) funds held by the Ingham Health Plan Corporation ($631,989) for the Mid-Michigan Pathways Program. The BOH further approved restricting the funds to that program.

The BOH approved the Retirement System Annual Report and waiver request for the Michigan Department of Treasury.

As Montcalm County approved their appropriation to the health department at approximately $38,000 less than requested, the BOH adopted surcharges on selected services in Montcalm County effective October 1, 2018.

The BOH adopted the following Monthly Healthy Living Recommendation for October 2018:

1. While most E. coli are normal and harmless residents of our colon, some

strains can cause serious, even deadly illness. 2. Children are very susceptible to serious illness due to STEC and HUS.

They also do many things that put them at risk. Teach them the habits that will keep them safe.

The BOH approved the Michigan Nurses Association (MNA) Contract for October 1, 2018 through September 30, 2021.

The BOH approved the non-union wage proposal for October 1, 2018 through September 30, 2018, including the change in the credentialing policy mirrored in the MNA Contract.

The BOH approved the Health Officer’s Employment Agreement for FY 18/19.

The BOH approved the new agency logo allowing the Rebranding Committee to select the best option pending the colors chosen and the cost to print the logo.
