in Nursing Course by the Selection Committee, Directorate of Medical Education

shall fall within the jurisdiction of Chennai only.

Applications can be downloaded from the websites www.tnhealth.org and

www.tn.gov.in and the filled application along with necessary enclosures and 2 self

addressed envelopes measuring 23 x 10 cms sent to the Secretary, Selection

Committee, Kilpauk, Chennai -10 before the last date along with a Demand Draft for

200 /- towards the cost of application form drawn in favour of The Secretary,

Selection Committee and payable at Chennai. The name of the candidate and

the name of the course should be written on the reverse of the Demand Draft.

The template with requisite details available for download should be printed and

pasted on the cover. SC/ SCA and ST candidates who are natives of Tamil Nadu are

exempted from payment of the cost of the Application Form.

AR Number will be assigned by the Selection Committee.

Candidates can track their applications on the above websites by entering their names

as entered in the application and the Plus Two Examination Register Number.

Eligible candidates can download their call letters from the websites www.tnhealth.org

/ www.tn.gov.in by entering their AR No allotted by the Selection Committee and their

Date of Birth and attend the counselling as per the date and time mentioned in the

counselling schedule which will also be available on the websites . Candidates are

requested to paste their photographs on the call letters which is to be attested by a

Gazetted Officer. ( The photograph must be a copy of the same one pasted on the

Application)

Prospectus for admission to Diploma in Nursing Course 2013-2014 session as per G.O(D)No.799 Health & Family Welfare (PME) Department dated: 29.07.2013 and as amendedfrom time to time.

1. GENERAL INSTRUCTIONS:-

a) Candidates seeking admission to Diploma in Nursing course should submit theapplication form with necessary enclosures which should reach:-

The Secretary,Selection Committee,No.162, E.V.R. Periyar High Road,Kilpauk, Chennai – 10.on or before 23-08-2013 by 5.00 p.m.

b) The candidates shall ensure that the completed application form with all particularsand enclosures reaches the Selection Committee within the date and timespecified. Applications received by Speed Post/Courier or any other means afterthe last date & time will not be accepted irrespective of the date of booking.

c) The Selection Committee shall not be responsible for any delay in the receipt orfor the loss in transit of application form etc.

d) Request for extension of time for submission of documents called for, after thespecified date and time will not be entertained.

e) Any supporting documents received after due date or application will not be accepted.

f) Candidates claiming special category (Orthopaedically Physically disabled EminentSportsperson, Children of Ex-Servicemen and Children of Freedom Fighter) shouldproduce the supporting documents along with the application. If supportingdocuments not produced they will be considered only under the General Category

g) No oral enquiry would be entertained.

h) Photocopy/ Fax copy of the application will not be accepted.

i) Both upper limbs, vision and hearing should be normal for all candidates includingfor candidates applying under physically disabled category.

j) Candidates selected under General Category will not be allowed to claim the verysame status as the candidates selected under Special Category and vice versa.

2. ELIGIBILITY :-The candidate should be a citizen of India.

3. Nativity :

i. Candidate should be a Native of Tamil Nadu

ii. Candidates who have studied from VIII to XII standards in schools in Tamil Naduneed not enclose the “ Nativity Certificate”

iii. Candidates who are native of TamilNadu but studied from VIII to XII Std eitherpartly or completely in schools situated outside Tamil Nadu should producethe “Nativity Certificate.

iv. The Certificate of Nativity in Tamil Nadu should be issued by the competent authorityonly in the specified format available in the Prospectus and supported by acopy of Passport/ Ration Card / Birth Certificate. If the copy of the supportingdocument is not produced, then the application form will be rejected. NativityCertificate submitted later will be summarily rejected.

v. Applications without a proper Nativity Certificate will not be considered.

vi.Permanent Residence Certificate will not be accepted.

Note:

a) Nativity Certificate should be produced as per Annexure XII a&b- See Instructionsfor Nativity Certificate.

b) Producing false nativity certificate if detected at any time later during the course ofstudy will result in expulsion of the candidate and also initiation of criminal proceedingsagainst the candidate as well as the Parents as per law.

4. AGE LIMIT :-

Minimum age will be 17 years and maximum age limit is 35 years as on 31st December2013

5. QUALIFICATION:-

i. Candidates should have passed in all the subjects of the qualifying examination ofthe Higher Secondary Certificate Examination (Academic) conducted by the TamilnaduState Board /any other equivalent Board with any group of subjects.

ii. Candidates who have already completed Diploma in Nursing Course are not eligibleto apply

6. MINIMUM ELIGIBILITY MARKS:-

i. OC, BC, BCM and MBC Candidates - minimum educationa) 10 +2 class passed preferably science with minimum 40% of marksb) 10 +2 class passed in any group with minimum of 40% of marks

ii. SC, SCA, ST candidates minimum educationa) 10 +2 class passed preferably science with minimum 35% of marks.b) 10 +2 class passed in any group with minimum 35% of marks.

iii. Candidates should have studied Tamil as the first language and English as the secondlanguage.

iv) Normalization Of Marks: - As per the Tamil Nadu Act No 3 of 2007, dt 05.03.2007and G.O( Ms) No 77 HE(J2) Dept , dt: 05.04.2007, the marks obtained by the studentsin the relevant subjects in the qualifying examination conducted by the various Boardsor Authority shall be equated with the marks obtained by the students in the samesubjects in the qualifying examination conducted by the State Board of Tamil Naduby adopting the method of normalization.

Explanation : -

Under the method of normalization the highest mark obtained by the students ofvarious Boards in each subject will be equated to the highest mark obtained by thestudents of State Board in that subject and the relative marks obtained by otherstudents in that subject will be determined accordingly.

Illustration:

If the highest mark secured by the student of State Board in Physics is 100 and thehighest mark secured by a student of any other Board in the same subject is 90, boththe highest mark will be considered to be equal to 100. If a student of the Other Boardsecures 60 marks in Physics when the first mark in Physics in the same board is 90, the60 marks will be considered to be equal to 66.66 marks as arrived at below:-

100 x 60 = 66.66% 90

After normalization of marks in the relevant subjects in the qualifying examinationconducted by different Boards, the qualified students of different Boards will be mergedinto a common merit list.

v) In case of candidates who have qualified from National boards such as CBSE and ISCEetc. the highest marks obtained by the candidates at the National Level alone will betaken into consideration for normalization.

PROCEDURE FOR FILLING AND SUBMISSION OF APPLICATION:-

7. a. Before filling up the application form and OMR sheets, read all theinstructions carefully.

b. Each application form bears a unique number which is printed on the Applicationform and the OMR. Candidates are requested to check the number and ensurethat the same number is printed on the Application form, OMR, Outer &Inner covers and Acknowledgement Card. In case any discrepancy is noted, itshould be immediately brought to the notice of the Selection Committee before thelast date for the receipt of applications. However the serial number of the OMRwill be considered as the final number for correspondence purposes

8. Fill up the application form and OMR sheets legibily, neatly and correctly. The informationwill be processed by using a computer. Any mistake committed by the candidate willprejudice the selection

9. Do not fold OMR sheets under any circumstances. Making a hole, using a pin or astapler on the OMR sheets should be avoided. Cutting, Overwriting and erasing in theApplication form and in the OMR sheet or Scrutiny Form should be strictly avoided.

10. (a) Application with incomplete or insufficient particulars or without enclosures or thosereceived after the last date mentioned will be summarily rejected.

(b) Filled up applications without the signature of the candidates will be rejected.

11. The Government orders issued and to be issued from time to time pertaining to any ofthe matters contained in the Prospectus should be read as part and parcel of thisProspectus and such Terms and Conditions in the Government Orders are deemed tohave been incorporated in the Prospectus.

12. Candidates should affix recent passport size photographs taken with name and date(as shown in the Instructions- Annexure X) in the places provided in the applicationform and the OMR.The photograph pasted in the common application form alone shouldbe duly attested by Group A or Group B officer of the Government of Tamil Nadu orCentral Government or the Head of the Institution last studied. The photograph pastedin the OMR need not be attested.

13. All the entries in the application form should be written legibly by the candidate in herown handwriting. The candidate should ensure that all information including the marksin the qualifying examination furnished by her in the application and the enclosuresare correct. The candidate is informed that if after proper scrutiny of her applicationthe marks furnished by her in the application are not correct or any other informationis found to be incorrect, then ;

i. She has to forfeit the admission no matter at what stage of the course, she will beat that time.

ii. She will be debarred from pursuing any studies for a period of three years

iii. Legal action will be instituted against the candidate and the parent/guardian forfurnishing incorrect information.

iv. Criminal action will be initiated against her for furnishing false Mark sheets falseCommunity certificate/false nativity certificate.

14. No Documents enclosed with the application will be returned. Candidates are requiredto enclose only the attested Xerox copies of the following certificates along withthe application. Attestation should have been made by Group A or Group B Officer ofthe State Government or Central Government or by Head of the Institution where thecandidate studied.

(i) Mark sheet(s) of H.S.C. examination/any other equivalent examination duly endorsedby the competent authority. Xerox copy of both the sides of the mark sheets shouldbe enclosed.

(ii) Transfer certificate obtained after the completion of H.S.C. (Academic) or equivalentcourses.

(iii) Nativity certificate (wherever necessary).

(iv) Permanent Community Certificate Card (if applicable)

(v) Candidates seeking Admission against Special Category seats should submitnecessary certificates obtained from appropriate authorities in the prescribed formand submit them along with the application to the Selection Committee. (Please seeformat in the Annexures II , III & IV.)

(vi) X standard Mark Sheet .(vii) Certificate for proof of study from VIII standard to XIIstandard.(viii) H.S.C Hall Ticket

Note:

i. The candidates belonging to Scheduled Tribe should produce Community Certificateobtained from Revenue Divisional Officer. An attested Xerox copy or certified copy ofthe permanent community certificate card should be produced.

ii. Producing a false community, Nativity or any other certificate if detected later at anypoint of time during the course of study will result in expulsion of the candidate andalso criminal proceedings will be initiated against the candidate as well as the parentsas per law.

iii. As number of bogus community certificates are being encountered off late and in orderto curb such practices, the authentication of the above certificate has to be verified andconfirmed by the concerned authority, before the individual completes 1st year Diplomain Nursing course.

The concerned candidate and parents should take necessary steps to thateffect. Unless proof of authenticity of community certificate is received, thecandidate will not be allowed to continue the 2nd year.Community Certificatesobtained from other states will not be considered and such candidates will be treatedas belonging to the open category

METHOD OF SELECTION AND ADMISSION : -

15. Selection will be made on merit basis of total marks of the subjects (excluding languages)obtained in +2 examination calculated to the base of 100 marks by the eligible candidates.

DETERMINATION OF RANK OF CANDIDATES OBTAINING EQUAL MARKS :

16. In cases where two or more candidates obtain equal total marks of the subjects (excludingTamil and English) for 100, the rank of such candidates shall be determined on thebasis of the following order :-

i) Percentage of marks in Nursing as Vocational/ Academic Subject.ii) Marks in Biology or Botany and Zoology taken together in +2 Examinationiii) Marks in Physics and Chemistry taken together in +2 Examination.iv) Number of attempts in Higher Secondary Coursev) Date of Birth - Weightage will be given to Seniority in Age.vi) Computerized random number assigned. Higher value of random number will be

taken into consideration.MERIT LIST:-

17. The Secretary, Selection Committee will draw a merit list and publish the same at thenotice board of Selection Committee, Directorate of Medical Education and on the website:http://www.tnhealth.org. www.tn.gov.in

18. The Candidates will be called for counselling as per their merit and they may exercisetheir option as per their eligibility norms mentioned in the Prospectus.

COUNSELLING PROCEDURE (SINGLE WINDOW SYSTEM):-

19. The selection will be made on merit basis and by Counselling applying the rule ofreservation by the Government of Tamil Nadu as follows:-

Open Competition - 31 % Backward Class - 30% Most Backward / Denotified Community - 20 % Scheduled Caste - 18% Scheduled Tribe - 1 %

Within 30% reservation for BC, 3.5% reservation will be provided for BC Muslimsand within 18% of reservation for SC candidates, 16% of seats out of 18% quotaearmarked to SC shall be alloted to the Arunthathiyar community

BC, BCM, MBC, DNC, SC, SCA & ST are eligible for selection under open competitionas per merit in addition to the reservation made for them.

a). Any remaining unfilled seats for want of adequate number of eligible candidatesbelonging to SCA community shall be filled by candidates belonging to SC communityother than Arunthathiyar as per merit and vice versa. (GO (Ms) 55 P & AR Departmentdated:08.04.2010)

b). Any remaining reserved seats in ST Quota for want of adequate number of eligiblecandidates belonging to ST community shall be allotted to candidates belonging toSC Community as per merit. (GO (Ms) No.77 Health & Family Welfare Department(MCA1) dated 24.02.2011)

c). Any remaining unfilled seats in BCM Community for want of adequate number ofeligible candidates after exhausting the merit list shall be filled up on merit basis bycandidates belonging to BC community without exceeding the quota for BC communityas a whole(G.O.Ms.No.30/H&FW Dept(MCA1)/dated 18/01/2012)

d). Even after filling up of the required seats reserved for Arunthathiyars on preferentialbasis, if more number of qualified Arunthathiyars are available, such excess numberof candidates of Arunthathiyars shall be entitled to compete with the Scheduled Castesother than Arunthathiyas in the inter-se merit among them (As per G.O.Ms.No.65,P &AR(K) Dept. dated:27.07.2009).

20. Counselling schedule will be available on the websites. Candidates who have notreceived the Call Letter in time can also attend the counselling as per schedule andas per their merit with call letters downloaded from the websites (www.tnhealth.organd www.tn.gov.in)

21. a. Counselling for the Special Categories will be done before the General Category.

b. There will be no reallotment / change of college allotted for the Special Category

22. One of the Parents will be allowed to attend the counselling. They should make theirown arrangements for attending the counselling session. Individual intimation will besent for regular counselling through call letter.

23. (i) Option once exercised during counselling will be final and cannot be changed.

(ii) (a) Candidates who fail to attend the counselling at the specified date and timewill forfeit their right to admission and their name will not be considered later.

(b) Due to unforeseen reasons, if a candidate could not attend the counselling onthe specified date and time she can attend on a subsequent time/date duringthat counselling and allowed to take the course and college available at thetime of counselling when she actually attends.

(or)

She can authorize her parent on her behalf and the parent’s decision will be binding onthe candidate. The authorization letter must be in the prescribed format in theAnnexure XI a & b.

24. If a candidate desires to choose a second (Non-Stipendiary) category seat, when firstcategory (stipendiary) seats are available, she has to give an undertaking in writingthat she is not willing to take up the first category (Stipendiary) seat and will not claimthe same in future.

25 a) At the time of counselling candidates can choose the training centres of their choiceson merit basis and the same will be allotted as per the availability of seats.

b) Candidates can exercise their option for the course and the college of their ownchoice at the time of counselling and allotment will be subject to availability of seats

c) They are eligible to attend the counselling for reallotment of Government colleges.The date of such counselling will be published in the website and also will be informedto the Deans of respective Government Colleges. No individual intimation will besent to the candidates.

d) The movement of candidate from one category to another category( Non Stipendiaryto Stipendiary) and from one college to another college will be based on the meritfollowing the rule of reservation.

e) Individual request for transfer of college or change of course will not be entertainedafter counseling is over.

f) Mutual transfer will not be entertained. The selection for Diploma in Nursing course isonly provisional. The selection and admission is liable to be cancelled at any time asand when the eligibility conditions are found to be defective in addition to revelationof suppressed and overlooked facts.

26 a Those who get Retotalled / Revalued marks can also attend the counselling on theday and session according to their new marks. If the candidate produces theRetotalled / Revaluation marks after the counselling is over and within the cut offdate then his/her name, will be considered in the existing vacancies available and he/ she can not claim any preferential rights for allotment according to merit underRetotalling /Revalued marks. Candidates who submit the Revaluation / Retotallingmarks after the administrative cut-off dates fixed by Government will not be considered

for selection

b) All communications will be sent only to the mailing address given in the envelope filled

by the candidate. Candidates are requested to intimate any change of address,immediately to the Selection Committee. The intimation/call letters will be sent eitherby Certificate of Posting, Registered Post, Speed Post, Courier Service or by Telegramwhichever is feasible. Selection Committee is not responsible for any Postaldelay or non receipt of call letters.

c) Any change or modification and relevant information pertaining to this admissionprocess will be made available immediately on the websites.

(www.tnhealth.org / www.tn.gov.in ) The candidates are instructed to visit thewebsite from the date of application till the end of counselling without fail. Please donot wait for counselling information to be received by post, as all information includingthe date of counselling will be put up in the websites. Call letters can also bedownloaded from the websites. Candidates will be permitted for counsellingaccordingly.

d) An intimation or call letter calling for counselling does not guarantee admission sincemore number of candidates are called than actual number of vacancies.

e) The Selection committee is not responsible for non- receipt of call letter due to improper/incomplete address written by the candidate in the envelope.

CATEGORIES OF SEATS:-

27. Students will be admitted under the following categories on merit basis.

a) Category - I Stipendiary

b) Category - II Non-Stipendiary. The stipend will be paid to the Nursing students as follows:-

StipendRupees per mensum

First year 400.00Second year 440.00Third year 480.00

The rate of stipend is liable to be revised at any time without prior notice.

SPECIAL CATEGORIES : -

28. (a) Seats earmarked for the Special Categories will be allotted following the HorizontalReservation. The Seats will be allotted as per merit order in the concerned specialcategory and those seats will be deducted in the concerned compartment (OC / BC/ BCM / MBC / DC / SC / SCA / ST) and the remaining seats will be allotted to theGeneral Category

(b) If adequate number of eligible candidates are not available under SpecialCategory the earmarked seats will be reverted back to general category.

(i) SEATS RESERVED FOR CHILDREN OF FREEDOM FIGHTERS - 3

The candidates while applying under the special category mentioned above should

furnish the copies of the following documents along with the application.

a. Assistance Certificates / Freedom Fighter Pension Orders issued by the Central /

State Government to the freedom fighter.

(or)

Jail Extract duly recommended by the District Committee for the Freedom Fighters.

b. Particulars of the Freedom Fighter in the book containing Freedom Fightersname published by Government of Tamil Nadu.

c. Proof that the candidate is a daughter of the Freedom Fighter.

(or)

Tashildar Certificate to establish the relationship of the candidate to the FreedomFighter (or) Legal heir certificate.

Birth Certificate to ascertain the parentage of the candidate.

Grand Children of Freedom Fighters, adopted and foster Children are not eligibleto seek admission to Diploma in Nursing Course under this category

(ii) SEATS RESERVED FOR CHILDREN OF EX-SERVICEMEN – 57

The candidate should furnish a certificate as prescribed in the Format in Annexure-IIobtained from the competent authority.

The special committee from the defence authorities constituted for this purpose willscrutinize certificates submitted by the candidates and candidates will be selected onmerit basis.

(iii) SEATS RESERVED FOR EMINENT SPORTSMEN – 3Selection will be done by the Common Committee constituted for this purpose. Themode of selection will be made as per the guidelines given in Annexure III. Thecandidates are requested to arrange the certificates as per the serial number startingfrom June 2009 as given in the Annexure III and submit along with the applications .All the particulars are to be filled up as per the guidelines given in the Annexure – III.

(iv) SEATS RESERVED FOR ORTHOPAEDICALLY PHYSICALLY DISABLED:

3% of the total number of seats available in Government Institutions are reserved forthe Orthopedically Physically disabled candidates. The candidates with disability oflower limbs between 40% to 50% shall be considered. The other conditions foradmission into Diploma Nursing Course will be applicable as in the case of the generalcategory.

The candidates are required to produce a certificate from the District Medical Boardof the area concerned constituted for the purpose assessing the nature and theextent of disability in the format prescribed in the Annexure - IV (a) & (b).

The Certificate must have been obtained within three months prior tosubmitting the application for seeking admission under this category. If the

Certificate has been obtained earlier, the application will be rejected.

The Medical Certificate issued by District Medical Board of the area concerned forthis purpose is subject to the confirmation by the Expert Committee constituted bythe Director of Medical Education at the time of counselling.

The Candidate seeking admission under this category should produce a full sizephotograph exhibiting the deformity.

Note:

i. The selected candidates for Diploma in Nursing course will be required to undergosecond Medical Examination by a Medical Board constituted for the purpose byDirector of Medical Education to ascertain and confirm the nature and extent of physicaldisability.

ii. Candidates with any other disability other than the locomotory disability of the lowerlimb will not be considered for admission into Diploma in Nursing Course under thisSpecial Category. Such applications will be summarily rejected without any intimationto the candidate.

iii. The decision of the Director of Medical Education in this regard will be final.

DURATION OF THE COURSE :-

29. Three and a half years including six months Internship (As per norms fixed by IndianNursing Council/ Government)

TUITION & OTHER FEE:

30. a) Non stipendiary candidates have to pay the tuition fee of Rs.200/- per year and

accommodation charge of Rs.20/- per mensum.

b) Candidates who have been selected under Orthopaedically disabled category are

exempted from paying the Tuition Fee and Special Fee.

c) All candidates should pay mess charges, etc.,

d) Students of SC, ST community, whose parents/guardian’s annual income is less

than Rs.2,00,000/- (Rupees two lakhs only) are exempted from payment of all

compulsory, Non refundable fee (Enrollment/Registration, Tuition, Games, Union ,

Library, Magazine, Medical Examination and such other fee compulsory payable by

the scholar to the Institution or University / Board) and also exempted from all

compulsory Non refundable fees fixed by the “Fee Committee”. The fees, so exempted

shall be claimed by the respective Institutions under the Scholarship Programme of

the Government of India (As per G.O, (Ms).No.6AD and TW Dept. dated 09.01.2012)

31. a) The selected candidates will be called for verification of certificates by the Head of

Nursing training centres concerned, Candidates will have to produce the original

documents at the time of verification. Selection will be cancelled if the candidate do

not produce all the original documents as required.

b) All the selected candidates should stay compulsorily in the Hostel during the entire

period of study.

32. The candidates have to undergo Medical examination by the Medical Board constituted

at the Training Centre to which they are allotted . only those candidates found Medically

fit will be admitted and be permitted to pay the fee.

33. Candidates selected for admission will have to sign the discipline declaration form

in Annexure-IX with her Parent / Guardian. Failure to sign the declaration will result

in cancellation of selection .

34. Nursing students who have successfully completed their training and obtained their

Diploma will be eligible for appointment as Nurses after registering with the TamilNadu

Nurses and Mid-Widwivies Council Chennai. Appointment cannot be guaranteed to all

or any of the successful candidates. Recruitment will be made as per rank list and in

accordance with the TamilNadu Medical Subordinate Service Rules. Stipendiary

candidates have to abide by the rules and serve the Government for the period as

specified in the Bond.

35. COMMUNICATION:-

All intimation for communication to the candidates will be sent only to the mailing

address given by the candidate at the time of submission of application. The Intimation

/ Call Letters will either be sent by Certificate of Posting or Registered Post or Speed

Post or Courier Service or Telegram or they can be downloaded from the Websites

www.tnhealth.org & www.tn.gov.in. as per instructions on Page 2 of this Prospectus

whichever is feasible depending on the availability of time before counselling. Selection

Committee will not be held responsible for Postal Delay and non viewing of website.

36. Hepatitis-B-Vaccination:-

All candidates selected for admission to first year Diploma in Nursing Course should

take the Hepatitis-B-Vaccine before admission and produce a certificate as in

Annexure – VI

37. Ragging : -

The candidates who join Diploma in Nursing courses should not indulge or participate

in any kind of ragging through out their study period. If they are found to have indulged

in ragging any time, the candidate will be removed from the roll of the institution at

whatever stage of study and criminal action will be taken against the candidate.

4. Open / Invitational Tournaments of any kind will not be considered.

5. For Tennis, the achievements of top three players by his / her position in the ranking

list published by AITA / TNTA based on the ranking rules and the final rankings of

previous four years, will be awarded marks for achievements and participation

(necessary forms from the Associations are to be enclosed).

6. For Cricket, the annual official championship tournaments conducted within the

country under the auspices of TNCA/BCCI at District / State / National level alone

will be taken into consideration for that year.

7. The participation / achievements of candidates with nativity in Tamil Nadu who

represented Tamil Nadu alone are eligible for marks under National Category.

8. Only Tamil Nadu candidates are eligible to apply for admission under Sports Quota

and only the KVS/CBSE schools within Tamil Nadu are eligible for marks.

9. A Common Committee constituted for this purpose will rank the candidates based

on the marks assigned to the certificates enclosed along with the application.

10. All the achievement certificates are to be supported necessarily by the relevant

participation certificates and appropriate forms.

11. No further enclosures or certificates will be entertained after the last date for

submission of completed application form.

12. Required number of candidates will be called to appear for an interview before the

Common Committee for verification of the Originals followed by counselling after

ranking.

13. If any candidate fails to produce the requisite Original Certificates / Forms, her place

will be allotted to the candidate next in rank and so on. The consequential vacancy

at the end of the rank list will be filled up from among the additional candidates

according to their ranks.

14. Selected candidates will be counselled and allotment of branches and colleges

ANNEXURE XII aINSTRUCTIONS FOR NATIVITY CERTIFICATE :

1. This Certificate should be issued by an officer of the Revenue Department notbelow the rank of Tahsildar in the Taluk concerned.

2. This Certificate should not be issued by the Special Tahsildars, Deputy Tahsildarssuch as Loans, Land Acquisition, Election, Excise and HQs, Deputy Tahsildars, SpecialDeputy Collectors, Assistant Commissioner of Agricultural Income, Excise, Elections etc.

3. This Certificate should be signed also by the Village Administrative Officer.

4. The Certificate should bear the stamp of the office of the Officer signing the Certificate.

5. Any foreign national irrespective of the period of study in the State will not becomeeligible to apply for this course. No certificate of Nativity in the State of Tamil Nadu,should therefore be issued to Nationals of other countries for this purpose.

6. The certifying officers should insist upon clear proof of the Nativity of the Parent orGuardian of the candidate and satisfy themselves on the genuineness of theresidential qualification.

They should be particularly careful in the case of candidates whose mothertongue is not Tamil and whose place of birth is other than Tamil Nadu andwho have studied VIII to XII standards outside Tamil Nadu.

7. Residence Certificate will not be considered as Nativity Certificate.

8. The guidelines prescribed for the issue of Nativity Certificate and they are as followsas per Letter No.RA.V(B)16932/2000, dated 3-4-2000 of the Special Commissionerand Commissioner of Revenue Administration, Chepauk, Chennai-5.

(i) The parents/guardians of the applicants/students or the applicants themselvesshould have permanently resided continuously for a period of five years in Tamil Nadu.

(ii) Permanently residing for a period of five years should be supported bydocumentary evidence.

(iii) The family ration card, Electoral Roll, Census List if taken recently, documentslike sale deed, tax receipt etc. relating to the property owned by either of the parentsor by the applicant may be verified.

(iv) The Transfer Certificate issued by the School authorities where the applicanthad studied last may be verified to know whether she was in the State for five years.

(v) Enquiry in the village / place of residence of the neighbour / Village AdministrativeOfficers regarding continuous residing.

(vi) To ensure that wrong or incorrect address had not been furnished to obtain thecertificate.

(vi) The birth place of the present, place of residence of the parent / father, permanentassets, mother tongue, place of education, place of marriage of the applicant /parents, the period of stay in and outside Tamil Nadu can also be considered beforeissuing certificate.

1. APPLICATION CONSIST OF TWO PARTS: -

The Application Form contains 21 items. Sub group of the items of the application

is reproduced in the OMR Sheet.

2. METHODS OF ENTERING PARTICULARS IN THE APPLICATION: -

Particulars required by an item are to be entered in the Application in long hand or in

coded form depending on the item. “Address for Communication

“for example, is written in long hand. For certain items, like “ Medium of Instruction “,

several alternative responses are listed in tabular form. Each alternative is assigned

a code. You must encircle the code which will signify your response to that item. In

the case of certain items, like Native District, you respond by writing a code in the

box or boxes provided.

3. SUPPORTIVE DOCUMENTS: -

Documents / Certificates are required in support of the data furnished by the candidate

for the following :- Date of Birth, Community Certificate, if applicable, Marks in qualifying

examination, Nativity Certificate, if applicable and claim for Special Category seats,
