(ii) IMPORTANT INFORMATION
1. For both in-service medical officers and private candidates: The
admissions for CHS CPS PG Diploma Courses for the year 2020 will
be conducted online.
(Note - Application forms will not be accepted if submitted by email.)
2. The candidates should read thoroughly the information bulletin
especially point no.4 concerning the eligibility criteria, to
ascertain their eligibility for in-service and private quota seats.
3. Two different merit lists will be prepared- one for in-service
medical officers and another, common merit list including private
candidates and in-service candidates since in-service medical
officers can opt to take private quota seat based on applications
received, NEET All India Rank and applicable incentives.
4. Constitutional reservation and reservation for PWD/SEBC/EBC candidates
will be applicable in these seats as per Government orders issued from

(iii) DISCLAIMER
a) The admission to the CHS Post Graduate Diploma courses for 2020 admission
session will be held online.
b) All candidates are advised to read the Information Bulletin 2020 and
carefully go through the instructions on the website
www.arogya.maharashtra.gov.in before filling up the online form.
c) Information provided by candidates like name of candidate,
contact/address details, category, PWD status, SEBC status, whether
Orphan or not status, educational qualifications details, date of birth,
domicile etc. during the submission of the online application form for
CHS CPS PG Diploma 2020 will be treated as final and CHS will not
entertain, under any circumstances, any request for change in
information provided by candidates after the online submission of the
application form.
d) CHS disclaims any liability that may arise to the candidate(s) due to
incorrect information provided by the candidate during application
and admission process. Therefore, aspirants are advised to exercise
utmost caution for filling up correct information in the online
application form.

1. ABOUT COMMISSIONERATE OF HEALTH SERVICES (CHS)
1.1. Commissionerate of Health Services (CHS) comes under Public
Health Department of Government of Maharashtra (GoM). The
Government of Maharashtra established the Commissionerate of
Health Services with the objective of providing adequate and
qualitative preventive and curative health care to the people of
the state. The prime objective is to ensure greater access to
primary health care by bringing the medical institutions and
services as close to the people as possible particularly in under-
served and backward districts of the state.
For this purpose, the department operates Sub-Health Centres,
Primary Health Centres, Rural Hospitals, Urban Health Centres,
Sub-District Hospitals, District Women’s Hospitals, District
Hospitals/Civil Hospitals/General Hospitals and Regional Referral
Hospitals across Maharashtra for providing primary, secondary and
tertiary level of health care.
1.2. CHS has initiated PG Diploma courses at various district level
institutes
under its jurisdiction in affiliation with College of Physicians & Surgeons
(CPS) of Mumbai from the year 2015-16 vide GRs number number
eoSv 2015 $iz-dz-85$lsok&3]fn-17$07$2015 and number eoSv 2015$iz-dz-85$lsok&3] fn-
30$09$2015-
The course, previously called as DHS PG Diploma (DHS CPS) will now be
called as CHS PG Diploma (CHS CPS) as per Government Resolution dated
on 16-01-2020.

2. SCOPE OF COMMISSIONERATE OF HEALTH SERVICES
CHS is the body authorized to conduct the 2020 admission for PG Diploma
Courses in the institutions coming under its jurisdiction.
2.1. COMPETENT AUTHORITY:
2.1.1. The Director, Commissionerate of Health Services Mumbai has
been designated as the Competent Authority for the
admission process.
2.1.2. The Competent Authority will supervise and control the
selection process by formulating procedures and
implementing the procedures through the CPS PG
cell/Designated Authorities.
2.1.3. The Competent Authority will issue necessary executive
directives for implementing the laid down procedures and
release necessary communications, notifications, press
releases or publications regarding the same.
2.1.4. The Competent Authority has the overall jurisdiction to
conduct the admission process under the rules and regulations
governed by the PHD and GoM.

2.1. DESIGNATED AUTHORITY:
2.2. The Deputy Directors of Health Services (DDHS) of all regional
circles (Thane, Kolhapur, Pune, Nashik, Aurangabad, Latur,
Akola and Nagpur) and Assistant Director (PG Diploma
Courses)/any other officer appointed by the Competent
Authority at CHS level, will act as Designated Authority for
implementation of the procedures regarding the PG Diploma
courses 2020 admission session.
2.3. The Designated Authority will be responsible to implement the
procedures and directions given by the Competent Authority
from time to time regarding the 2020 admission session for PG
Diploma courses.
2.4. The Designated Authority may depute other officers to
represent them in the selection process.
2.5. APPELLATE COMMITTEE:
2.6. In case of allegedly arbitrary decision or violation of prescribed
rules regarding the admission session 2020, by designated
authorities or their representatives, the candidate can appeal
before the Appellate Authority.
2.6.1. The Appellate Committee will consist of following members:

3) Joint Director (Establishment),DHS Mumbai
4) Joint Director, DMER Mumbai
5) President, CPS Mumbai

3. GENERAL INSTRUCTIONS, TERMS, CONDITIONS AND REGULATIONS
3.1. NEET PG 2020 is the single window qualifying cum ranking
examination for admission to the CHS CPS Diploma courses for 2020
admission session as per the section (10) of the IMC Act 1956.
3.2. Applicants may kindly note that applying or appearing for the
2020 admission session does not confer any automatic right to
secure a CHS CPS Post Graduate Diploma seat. The selection and
admission to PG seats in any CHS institutions recognized for
running Post Graduate Diploma courses by the CHS & CPS is
subject to fulfilling the admission criteria, eligibility criteria,
medical fitness and such criteria prescribed by the MCI or GoM
from time to time.
3.3. Applications of candidates producing false or fabricated
information and/or suppression of any information in the
application forms will not be considered and would lead to
disqualification of the candidate, even at a later date. Such
candidate may be further debarred from appearing in any future
admission sessions conducted by CHS.
3.4. The candidate will be responsible for submitting true and genuine
documents. Candidates producing false/fabricated documents will
be disqualified and this will lead to cancellation of admission,
forfeiture of the fees/deposits and expulsion of the candidate from
the institute by the Competent Authority or Designated Authority. If
action against the candidate leads to lapse of seat then additional
penalty of Rs.10,00,000/- (Rupees Ten lakh only) will be imposed
on the candidate. If deemed fit legal proceedings may be initiated
against such candidates/their parents by the Competent Authority.
3.5. For Constitutional reservation category candidates:
a. Ear-Marking - Reserved category candidate entitled to admission on
the basis of merit in an open category seat will have the option of
taking admission either against his/her respective category seat or

on an open category seat. If he/she opts for the reserved category,
he/she will be deemed to have been admitted as an open category
candidate and not as reserved category candidate. One seat in the
Institute where he/she was eligible for admission against open seat
shall be earmarked for a candidate belonging to the respective
reserved category. Such earmarked seat will be made available
immediately to the successive reserved category candidate from the
same category merit.
3.6. Cut-off date for the final assigned category:
The final assigned category of candidate(s) will be considered as per
the received documents before the last date of online application
submission. It is made abundantly clear that if a candidate is found
unable to submit documents as per the category claim as on the last
date of online application submission considered as the cut-off date.
The candidate will be considered in open category after the aforesaid
cut-off date and no changes/requisite documents will be accepted by
the Competent Authority.
3.7. Candidates shall go through the information bulletin carefully for
eligibility criteria, Online registration, Application fees and
Preparation of Provisional Merit List, Online Document Uploading
Process, Final State Merit List, Seat Distribution Chart & Online
Preference filling and selection process etc. before contacting CHS
for any queries. Queries pertaining to eligibility and other issues
will only be entertained if the information requested is not given
in the information bulletin or the website
www.arogya.maharashtra.gov.in
3.8. Information in the Information Bulletin is liable to changes based on
decisions by the Competent
Authority/CHS/CPS/MMC/MCI/GoM/GoI/Hon’ble Court from time to
time.
3.9. CHS reserves the right to withdraw permission, if any, granted

inadvertently to any candidate who is not eligible for appearing
in the 2020 admission session for Post Graduate Diploma courses
even though name/roll number is displayed in the merit list on
CHS website or on screen/display board during admission
sessions.
3.10. Candidate’s eligibility is purely provisional and is subject to fulfilment
of applicable regulations, eligibility criteria as prescribed from time to
time by Competent Authority/CHS/CPS/MMC/MCI/GoM/GoI/Hon’ble
Court.
3.11. The existing schedule, policy and guidelines are for ready
reference only but in no way, they are ought to be treated as
representative or acknowledgement of fact that CHS is bound to
follow the same in future.
3.12. In case of any ambiguity in interpretation of any of the
instructions/ terms/rules/criteria regarding determination of
eligibility/conduct of admission session/registration of
candidates/information contained herein, the interpretation of
the Commissionerate of Health Services will be final and binding.
3.13. Requests are not entertained for change in date/centre of
admission session. Candidates are advised not to canvass for or
submit such representations.
3.14. Candidates should ensure before applying for the admission to PG
Diploma courses 2020 that their MBBS degree is recognized by
MCI under IMC Act 1956. If it is found at any time that MBBS
degree is not recognized the candidature of the candidate shall
be cancelled /deemed to be cancelled.
3.15. All the correspondence regarding queries should be addressed by
email preferably. The email query will be addressed only if it is not
anonymous and contains the name, postal address, NEET roll
number and phone number of the sender. An email containing vague
or general queries that are contained in the Information Bulletin
shall not be entertained. Queries shall not be entertained from
persons claiming themselves to be representative, associates or

officiates of the applicant candidate.
3.16. The following information will not be revealed on phone or email
a) Internal documentation/status
b) Internal decision making process of the CHS/CPS
c) Any claim/counter claim arising there of
d) Dates and venue of internal meetings or names of staff officers dealing
e) Any information which in the opinion of CHS can’t be revealed.
3.17. Candidates are advised to look into
www.arogya.maharashtra.gov.in and www.cpsmumbai.org
regularly for various information and notices pertaining to CHS
PG diploma courses.
3.18. Candidates are deemed to have read, agreed and accepted the
Information Bulletin 2020 and the terms and conditions in the
bulletin for CHS PG Diploma courses on submission of
application form.
3.19. Each candidate can register and appear for CHS PG Diploma
courses 2020 admission session only once.
3.20. CPS House, Parel, Mumbai shall take the decision regarding
enrolment and registration for the PG Diploma course. Fee
structure laid by CPS House, Parel, Mumbai, apart from CHS CPS
fees, shall be applicable for all candidates.
3.21. The jurisdiction for court cases/disputes shall be within the
exclusive jurisdiction of competent courts at Mumbai only.
3.22. Reservations- constitutional, SEBC and PWD, are applicable as
per Government/MCI rules and court orders.
3.23. EWS Reservation is not applicable as speciality wise seat intake
has not increased and as per MCI Supreme Court order Write
Petition (C) NO. 55/2019. EWS reservation shall be applied only
in case if seat intake capacity has increased

4. ELIGIBILITY CRITERIA FOR CHS CPS PG DIPLOMA COURSES 2020
4.1. All candidates who have scored minimum qualifying score in NEET
PG 2020 examination as specified by GoI are eligible for admission to
the CHS Post Graduate Diploma courses for 2020 admission session.
4.2. Online application form submitted for CHS PG Diploma 2020 for both
in-service and private candidates having qualifying score in NEET PG
2020 as specified by GoI will only be considered valid.
4.3. Candidate must be an Indian National. Nationality certificate
issued by District Magistrate or Additional District Magistrate or
Chief Metropolitan Magistrate or School leaving certificate
(SSC)/Junior Institute leaving certificate (HSC) indicating the
nationality of the candidates as ‘INDIAN’ shall constitute the
proof of nationality.
4.4. Candidates in possession of MBBS degree or Provisional MBBS pass
certificate recognized as per the provisions of the Indian Medical
Council Act 1956 and possess permanent or provisional
registration certificate of MBBS qualification issued by the
Medical council of India or State Medical Council and have
completed one year of internship on or before 31st March 2020
are eligible for the CHS PG Diploma courses 2020.
4.5. Maharashtra Medical Council (MMC) registration shall be done by the
other state candidates within six months after their admission and
enrolment in CHS PG Diploma courses 2020.
4.6. The MCI/MMC registration is compulsory for all the applying
candidates. Registration certificate or provisional registration
certificate issued by MCI/MMC should be produced at the time of
online application.
4.7. In-Service candidates
4.7.1. Eligibility criteria and incentives for ‘in-service’ quota will be

4.7.2. In-service candidates applying will also be
covered by relevant general eligibility criteria.

4.8. The candidates belonging to reserved category
(SC/ST/OBC/VJ(DT)A/(NT1)NTB/(NT2)NTC/(NT3)NTD/SBC/PWD/S
EBC) shall upload scanned copies of original caste certificate,
caste validity certificate and non-creamy layer certificate
(wherever applicable) at the time of registration and online
application submission for the admission for CHS Post Graduate
Diploma courses for 2020 admission session.
4.9. A candidate belonging to ‘Creamy Layer’ amongst the categories
VJ(DT)-A, (NT1)NT-B, (NT2)NTC, (NT3)NT-D, OBC and SBC must note
that the provision of reservation is NOT applicable to him/her. A
candidate claiming benefit of reservation under these categories will
be required to submit scanned copy of the 'Non-Creamy Layer
Certificate' as specified in the GR number CBC-10/2008/CR-697
VJNT- 1, dated 24thJune, 2013 during the online application. The
certificate in the prescribed proforma stating that it is valid till
31/03/2020 should be submitted.
4.10. Reservation for Socially and educationally Back ward classes (SEBC)
is applicable as per Maharashtra Act no. LXII of 2018 and its
amendment Maharashtra Act no. VII of 2019 dated 3rd July 2019.
4.11. The candidate admitted for MBBS course in reserve category on the
basis of court order for caste claim is required to submit Caste
Validity Certificate given by the Caste Scrutiny Committee. If the
matter is still pending with Hon’ble Court, candidates should submit
afresh court order for Competent Authority, CHS PG Diploma 2020
for appearing in the admission process for CHS postgraduate diploma
course.
4.12. NON-ELIGIBILITY OF CANDIDATES:-
4.12.1. Candidates who have been allotted seats through NEET
PG 2020 in all India quota/state quota/CPS and have
taken admission as per the allotment shall not be
eligible for CHS PG Diploma courses 2020.
4.12.2. Candidates who have not appeared for NEET PG 2020

conducted by NBE, New Delhi will not be eligible for
admission to CHS PG Diploma courses 2020.
4.12.3. Candidates who did not complete internship by 31st
March 2020 will not be eligible for admission to CHS PG
Diploma courses 2020.
4.12.4. The in-service candidates who were found guilty in the
departmental enquiries or against whom departmental
enquiries are pending at the CHS during the course of
admission period, will be considered not eligible for CHS PG
Diploma 2020.If the matter is still pending with Hon’ble
Court, candidates should submit afresh court order for
admission to postgraduate diploma course.
4.12.5. The candidates who had been admitted to PG
Diploma/Degree course and are pursuing/have
completed the PG degree or diploma course will not be
eligible for admission to CHS PG Diploma courses 2020.
4.12.6. Candidates who had been admitted to CHS PG Diploma
course through earlier qualifying exams and
resigned/left/not joined which resulted in lapse of seat
will not be eligible for admission to CHS PG Diploma
courses 2020.
4.12.7. Candidates who have registered for PG Diploma course
and have not passed the course will not be eligible for
admission to CHS PG Diploma courses 2020.
4.12.8. A candidate can register and apply for CHS PG Diploma
courses 2020 only once for admission to the academic
year 2020.

4.13. In-service candidate’s cut-off date for eligibility: 5th January
2020 will be considered as the cut-off date for deciding the in-
service eligibility of the candidate for all purposes including
applicability of all these rules and regulations. The age criteria
will be as per GR No. MO.NO.2019/165/SEVA-03 dated
19/3/2019 wherein candidate should not have completed 45
Years of age till the date of qualifying NEET PG 2020 exam.
4.14. It is made abundantly clear that if a candidate is found ineligible as
on 5th January 2020 in terms of these rules and regulation and if
such candidate acquires eligibility after the aforesaid cut-off date
such subsequent acquisition of eligibility will not make an ineligible
candidate eligible for any purpose what so ever, much less for the
purpose of admission process.

5. ONLINE REGISTRATION, APPLICATION FEES & PREPARATION Of PROVISIONAL MERIT LIST
5.1. The Competent Authority shall issue Notice regarding schedule of
online registration and submission of online application form for
the preparation of provisional merit list for admission to CHS CPS
postgraduate Diploma courses 2020 admissions. Candidate, who
has appeared for and qualified in NEET PG 2020 and is desirous to
take Postgraduate Diploma admissions in CHS CPS institutes,
should submit online application form which will be made
available on website www.arogya.maharashtra.gov.in. The
candidates will be required to submit single prescribed online
application form (ANNEXURE "L" Specimen Copy of online
application form). Candidates submitting this application form
will only be considered for subsequent admission process of CHS
PG Diploma Courses 2020. In-Service candidates who wish to seek
admission for the postgraduate course shall also apply online at
par with other Non-In-Service candidates and claim In-service
quota. The candidate claiming for in-service quota seat must also
claim in the online application form through the provision
provided for the same.
5.2. In-Service candidates may kindly note that online registration and
submission of application form of CHS PG Diploma Courses 2020
does not confirm any automatic right to secure a postgraduate
seat from in-service quota. The selection and admission are
subject to the fulfilment of eligibility criteria as prescribed by
concerned establishment i.e. Commissionerate of Health services
Government of Maharashtra. Competent Authority shall not be
responsible, if such in-service candidates are refused for
admission on account of non-eligibility of In-service candidature,
in In-service Quota.

5.3. Cost of online application form of CHS PG Diploma Courses 2020
is Rs.2000/-. This application fee is non-refundable. Hence
candidates are advised to go through the Information Bulletin and
ascertain the eligibility. It is sole responsibility of candidate to
ascertain online payment of fees (Transaction ID and details of
payment required for form submission.), failing which online
application will be rejected.
5.4. The Application fee (Rs.2000/-) have to be paid by the
candidate(s) at CHS Bank Account through online mode. The
details of which are given below.

The transaction ID will be required for completing the application
form. Hence candidate should note the same. Considering the
time for the reflection of paid fee in the CHS account, candidates
are advised to apply well before the last date.
5.5. Online application Form will be made available and will be
notified in due course by CHS CPS Cell on website
www.arogya.maharashtra.gov.in. Before filling Online
Application form, candidate should download information bulletin
of CHS PG Diploma Courses 2020 and read the same carefully.
The Applicants must possess the required criteria and educational
qualification as mentioned in the Information Bulletin of CHS PG
Diploma Courses 2020.

6. Online application form filling process:
The details of online application form submission process is on

Click on the above Link.
a) Application form link will be displayed. Click on Application
form link.
b) Instruction page will be displayed. Carefully read all the
Instructions for filling online form (Annexure "M"). After
reading Instructions, applicant has to click NEXT button to
go to actual Online Application form
(Annexure “L” a specimen copy of online application form)
c) Candidate should fill all the entries in application form.
d) System will validate required fields before allowing him / her
to submit.
e) The submission of application will proceed only if transaction
details are duly filled.
Before submission of Application Form, the candidate should read
carefully the conditions given under "Declaration by the
Candidate." Candidate should agree to the below given
conditions, before submitting the application form. After
submission of the form, candidate will not be able to make any
changes and no such request will be entertained.

7. "Declaration by the Candidate"
I. I hereby solemnly and sincerely affirm that each and every
statement made and the entire information given by me in the
application form is true and correct.
II. I have not concealed any material information, however if any
information submitted herein is found fraudulent, incorrect or
untrue. I understand that I am liable to criminal prosecution and I
also agree to forgo my seat in CHS Postgraduate course. I also
understand that my selection and admission to the course is also
liable to be cancelled.
III. I have carefully read the rules & regulations of CHS PG Diploma
Courses 2020 bulletin and I agree to abide by them. I hereby
accept in entire the legality, validity and correctness of these
rules. I understand by submitting this application I have accepted
the correctness, validity and/ or justifiability of all these and
that had I not accepted the correctness and validity of these
rules, I would not have submitted this application and further
that it will not be open hereinafter to challenge and/or question
validity and/ or correctness of any rule or part thereof.
IV. I undertake to submit all the required original certificates at the
time of admission to a course/Institute as per the rules, failing
which I understand that my claim for selection shall not be
granted.
V. If the candidate agrees for aforesaid conditions, then only he/she
should click the "I Agree" Button so that his application form will
be accepted.
a. During submission the system will ask “Password” to
the Candidate.
b. Eligible Candidate will be able to submit the form.
There is Provision to make changes in the Application

Form before Click on Submit Button. Candidate
should verify the correctness of entries made in
application form. There is no provision to make
changes in the application form once Submit Button
is clicked. A notification box mentioning application
number will be seen at this point. Candidates should
note the number down for further steps.
c. After submission of the form, candidate’s account
will be generated and he/she will have to log in in
their account to check the submitted form.
d. Candidate should login with Application No. (User
Name) and his/her Unique Password (created by
him/herself) to download the printout of Application
Form. Candidate has to preserve the downloaded
printout of Application Form for subsequent use.
‘Log In’ Link will be available on website -
www.arogya.maharashtra.gov.in to verify the Application
Form.
Applicants need not send hard copy of Application Form or
any Document to the CHS office.
Please note that if you commit any mistake in filling the online
Application Form, it cannot be corrected by this office. CHS will
take no responsibility of any kind for such applications.
VI. Candidate should preserve the -
i. Transaction details of fee payment.
ii. Copy of downloaded online application form
iii. Downloaded copy of Information Bulletin for subsequent
use.

Uploading of Documents: After successful form submission,
Candidate has to upload scanned copies of required original
documents online. The documents should be uploaded in the
respective categories provided.
PUBLICATION OF PROVISIONAL MERIT LIST: On the basis of online
application form submitted, after document verification of
candidates and after receipt of the additional incentive marks with
Eligibility Status of NEET PG 2020 In-Service candidates as prescribed
in clause 9 (IV) & (VII) of PG Regulation 2000 of MCI from the
Commissionerate of Health Services (CHS) Government of
Maharashtra, a provisional merit list will be prepared by the
Competent Authority. This list will be published on the website as
per constitutional and/or specified reservation including In-Service
Status claimed by the candidates. The eligibility and final assigned
category of candidate(s) will be considered as received during the
online application period. CHS will not entertain any requests for
changes or submission of the eligibility and category claim
documents received after the cut-off date.
Provisional merit list will be based on NEET PG 2020 score and
applicable incentives given to in-service candidates on the basis of
their service as finalised by establishment section at the CHS.
Objections related to provisional merit list should be submitted by
the candidates on the e-mail ID [email protected] till the
cut-off date as specified by the department.

6. ONLINE DOCUMENT UPLOADING PROCESS:
I. Online document uploading will be on the same website and on
his/her User name and password.
II. Candidate has to upload all the relevant documents on this site in
the specified groups/categories provided.

Once document are submitted online, it will be verified. If all the
relevant documents are correct and complete, it will be informed to
each candidate via email and SMS on the registered email id and mobile.
For candidates, whose documents are deficient as per his/her claim,
grace period is provided till the last date of online application
submission for resubmission of the documents. These documents will be

notified via email and SMS on their registered email id and registered
mobile numbers.
7. FINAL STATE MERIT LIST
Objections received from candidates on the provisional merit list
published by the department will be scrutinized and verified and
thereafter final merit list will be published by the Competent
Authority as per the schedule.
8. ONLINE PREFERENCE FILLING PROCESS AND SELECTION PROCESS BY COMPUTERISED ROUND:
The Institute-wise Availability of Seats for 2020-21 Sessions for online
preference form filling process are as per (Annexure-I)
Admission to CHS postgraduate Diploma courses for academic year
2020-21, will be done online by way of filling of the online “Preference
Form” through the Log-in ID created by the candidate (specimen copy
of preference form- Annexure- M). The selection of the candidates will
be made on the basis of preferences given by the candidate.
Preferences (choices) will be allowed to fill online to those candidates
who are found to be eligible. List of eligible candidates will be
published as final state merit list. Seat allocation shall be made
centrally at a later date on the basis of final state merit list published
by the state CPS cell and preferences exercised by the candidate.
The candidate can fill online preference form multiple times before
clicking submit button. Once submit button is clicked, then there is no
provision to change the preference(s). These preference(s) will be
used for first admission round and subsequent admission round(s) of
selection/up gradation (if any). Up gradation in subsequent rounds is
compulsory.
There shall be two or more rounds of selection of candidates
depending on the availability of vacant seats. The selection will be on

the basis of merit and the preferences submitted by the candidate. For
second and subsequent rounds (if any) no separate online preferences
will be filled by the candidate. The choices of courses and Institutes
once entered online shall be final and irrevocable. This final data shall
be used in entire selection process.
The Subject & Institute combination code is a five-digit number
which is specific for each Institute & Subject combination and a suffix
indicating the seat as Private(P) or In-service(I). Candidate should fill
minimum one preference and maximum as desired. It is not compulsory
to fill all Preferences. The first two digit of the code indicates specific
Institute / institution and last two digits indicates particular subject.
The suffix indicates the admission through Private or In-service quota as

The Private candidate(s) should verify the fee structure given in the
bulletin before filling online preferences. The private candidate(s)
should also take in consideration while filling the preference form that
if In-service seats remain vacant after the regular rounds the
Competent Authority may decide to make these seats available to
eligible private candidates as per their merit. So the Private candidates

can also opt for in-service seats by filling up the preference codes as
specified above.
The allotment of seats is based on the Preferences submitted by the
candidate for the same Institute and same subject
Candidate should fill his most preferred subject and Institute code as
preference no. 1 followed by his next choice as preference no. 2 and so
on. While filling the subject and Institute codes, five -digit number
should be entered in five boxes of CHS PG Diploma 2020 preference
form. Candidate must be very careful while filling the preference form.
Candidate can fill minimum one preference or any desired number of
preferences for CHS PG Diploma Courses 2020 admission. If a candidate
does not give preference for a particular code then it means that he /
she is not interested / desiring to take that PG seat. Candidate should
note that there will be no change / addition / deletion in his / her list
of preferences at later date / stage.
A) Status Retention:
In case if a candidate to whom a seat has been allotted in the
first round and is satisfied with the allotment, such a candidate must
submit an online Status Retention Form in the given format and
Payment of the Course fee on or before prescribed date. Candidate
who has submitted status retention form will not be eligible for
remaining subsequent round(s). It is hereby made clear that the
period provided for submission of Status Retention Form is crucial
and vital. This period shall not be extended for any reason
whatsoever. Submission of the Status Retention Form is an
irrevocable and irreversible act for the candidate submitting such
Status Retention Form. It is made clear that under no circumstances
whatsoever, a candidate will be allowed to submit a Status Retention
Form after the aforesaid last date. Under any circumstances
whatsoever, a candidate who has submitted a Status Retention Form

shall not be allowed to withdraw the same. All the candidates
therefore, should take the note of this before submission of such a
Status Retention Form. Names of all the candidates who have
submitted the Status Retention Form as above within the prescribed
time and the seats occupied by them will be removed out of

B) JOINING THE INSTITUTE: (submitting online willingness to join the
Institute and Payment of the Processing fee at CHS)
All the candidates to whom a seat has been allotted in the 1st
round on the basis of Merit-cum-Preference will have to compulsorily
submit online willingness to join the institute and course so allotted
on the website and pay the processing fee for the same by ‘LOGGING
IN’ in the candidate(s) account created on the website.
If a candidate gets selected for a particular Diploma course,
then the candidate is required to join that course as per the
procedure (submit online willingness). In case he /she does not join
the course, he / she will not be eligible for subsequent admission
process of CHS PG Diploma 2020. It is made clear that even if a
candidate to whom a seat has been allotted in the 1st round wants a
betterment of his/her choice, such a candidate must initially accept
the allotment and join the institute by submitting the willingness
online and pay the processing fee at CHS (details of which are given
below), failing which such a candidate will not be Eligible for all

further admission round(s) including MOP-UP round(s) for academic
year 2020-2021.
After submission of the willingness the candidate have to pay the
non-refundable processing fee of Rs. 1000/-

If any candidate to whom a seat has been allotted in the 1st round does not
join the institute and course seat so allotted (willingness to join &
processing fee submission), it will be presumed that such a candidate is not
interested in the admission process and for all further admission round(s)
including MOP-UP round(s) of admission, such candidate shall not be
considered for allotment of any seat.
The seat so allotted to such a candidate who does not join, will be
presumed to be a vacant seat as on and from the next date after the date
set out as the last date for submission of willingness to join the institute in
terms of such allotment. Such a vacant seat will be allotted in the next

remaining eligible candidate(s). All the candidates who have filled the

Inter-se Round: Category wise seats remaining vacant after the initial
rounds will then be filled up through inter-se rounds.

Mop-Up round: (The decision regarding whether to conduct Mop-up
round through physical counselling will be decided later by the
Competent Authority considering the situation in the State.)
- In-Service vacant seats: If these seats remain vacant, will be filled
by MOP Up round. For filling these seats, no fresh preference(s)
will be called from the candidates. These seats will be added to
the seat matrix and allotted as per the preferences (choices)
already exercised by the eligible candidate(s) during online
preference filing system. Private candidate(s) who have opted for
in-service seats during preference form filling will also be
considered for these vacant seats based on their merit.
- PWD vacant seats: If the seats reserved for the person with
disabilities in a particular category remain unfilled an account of
unavailability of candidates the seats shall be included in the
annual sanctioned seats for the respective category and allotted
as per the preferences (choices) already exercised by the eligible
candidate(s) during online preference filing system

Mop-Up Round (s): After conducting two or more admission rounds of
seat allotments if seat(s) remain vacant due to non-allotment, non-
reporting for admission and resigning after joining or any other reason,
such seats will be filled in Mop-Up round(s) which will be conducted by
State if required based on necessity. The details of the Mop Up round(s)
will be declared by way of notice on the website in due course.
A) Eligibility for MOP-UP Round(s):
1. All candidates admitted in regular online rounds and secured
the admission (joined the course online and paid processing
fee(s)) but not filled status retention form.
2. All the candidates who are wait listed and to whom choice
was not available during previous rounds.

B) Candidates NOT ELIGIBLE for Mop-Up Round(s):
1. Candidate to whom a seat has been allotted in computerized
admission rounds and not joined.
2. Candidate who has filled status retention form.
3. Those candidates who had joined and resigned after allotment of
seats in previous rounds.
4. Those candidates who are joined but not working in the
institution.

9. VALIDITY OF CHS PG DIPLOMA 2020
The validity of the admissions of CHS PG Diploma 2020 shall be only for the
current academic session i.e. 2020 academic year for PG Diploma courses
and cannot be carried forward for the next session of admissions for PG
Diploma courses.
10. LIST OF MEDICAL PG DIPLOMA COURSES
The list of available medical post graduate diploma courses is given in
Annexure-B
11. DISTRIBUTION OF MEDICAL PG DIPLOMA SEATS
SEAT DISTRIBUTION CHART:
The seat distribution of institutions for CHS PG Diploma 2020 will be made
available on website i.e. www.arogya.maharashtra.gov.in, which contains
distribution of available seats.
The seats reserved for person with disability (PWD) candidates and In-
service quota seats will be shown in the chart. The institute-wise, subject-wise
and category-wise distribution will also be available on website. It should be
noted that category candidates are eligible and can avail open merit seats by
virtue of merit. Category seats, if at all remain unclaimed by a particular
category candidate are made available to other categories & to open category
candidates as per inter-se category round procedure
11.1. The seats available for admission to medical postgraduate diploma
courses through CHS PG Diploma 2020 will be from institutions
located in Maharashtra i.e. institutes coming under PHD like
Regional Referral Hospitals, District Hospitals, District Women
Hospitals, Civil Hospitals and Sub District Hospitals.
11.2. The seats available for admission to medical postgraduate diploma
courses will be distributed as 50% for in-service quota and
remaining 50% for private quota.
11.3. Available CHS PG Diploma seats will be distributed in accordance

to constitutional reservation policy of the state as mentioned in
(Annexure-C & D) The specified reservation under Persons with
Disability (PWD) category shall be filled as mentioned in
(Annexure-E)
11.4. The seats distribution for in-service CHS and private candidates
will be as per government policy which is subject to change from
time to time.
11.5. Postgraduate Diploma seats left unfilled in academic year shall
not be carried forward to the next or subsequent academic year.
The date of starting the academic year will be declared in due
course.
12. CAUTION NOTICE
a) Candidates are advised to refer to website
www.arogya.maharashtra.gov.in and Information Bulletin 2020 for
authentic information, periodic updates about CHS PG Diploma 2020.
b) Candidates are advised not to be allured by various claims of any
party or person or institute for securing seat in CHS PG Diploma 2020
as per regulations.
c) Candidates are advised to bring any such information to the notice of DHS
by email:[email protected] or by writing to Director,
Commissionerate of Health Services, Arogya Bhavan, 1st Floor, St George
Hospital Campus, P. D'Mello Road, Mumbai – 400001 MAHARSHTRA.
13. UNFAIR MEANS
If during the course of admission/training, a candidate is found
indulging in any of the following, he/she shall be deemed to have used
unfair means.
11.1 If the candidate is found to have made a wrong statement in
his/her application form for admission or has attempted to secure

or has secured admission to the medical postgraduate diploma
course by making a false statement or by production of false
document.
11.2 If the candidate hides information about having taken admission in
any PG course in different institute or having completed PG
course in any other specialty.
11.3 If at any stage a candidate has tampered with any entry in the
certificate or statement of marks or any certificate issued by any
governmental or non- governmental body or any other document
that has been issued to him/her by the board.
11.4 Impersonation.
11.5 Threatening any of the officials connected with the conduct of the
admissions or threatening any other candidate.
11.6 Candidates found to have attempted or trying to attempt
personally or through another person to influence or pressurize a
member, or any officer or official connected with the admissions
of CHS either at the office or their respective residences, in any
matter concerned with admission sessions.
11.7 Pressurizing a member, or any officer or official for change in
place of posting or deputation after the admission session.
11.8 Any candidate found guilty of having adopted any one or more of
the above unfair means/conduct is liable to be penalized with a
penalty by the CHS which may vary from cancellation of the
admission/expulsion for the next 7 years and/or cancellation of
candidature as may be decided by the Competent Authority/CHS.
11.9 Disclaimer:-The decision of CHS shall be final and binding for
declaration of any person / candidate guilty of foregoing or such
offences as shall be classified as Unfair Means cas

14. CONDITIONS TO BE SATISFIED BY CANDIDATE AFTER SELECTION
A) All selected candidates will have to complete all four terms totalling to
two years under CHS CPS programme only. Failure to do so will result in
penalty of ‘seat lapse’ condition.
B) The selected candidate will have to submit all the relevant original
certificates/documents.
C) The selected candidate must have paid the fees prescribed by CHS
Mumbai and CPS office, Parel, Mumbai.
1. Fees Structure:
A) The fees for the course for the private candidates as decided by the
Competent Authority meeting under the chairmanship of the
Principal Secretary (Health), dated 9th September 2015 shall be
Rs.2,00,000/- (Rupees Two Lakh only) per year.
Stipend will be paid to the candidate(s) at the rate of Rs. 25,000/-
per month as approved in NHM PIP 2020-21 (stipend may change
subject to government decision in future.) according to the
attendance.
B) There are no fees for the course for the eligible in-service candidate
taking admission on in-service quota seats apart from the processing
fees of joining and application form fees.
C) In-service candidate taking admission against private quota seat,
will have to pay tuition fees of Rs.1,10,000/- per year.
2. Bond:
A) As per government resolution [GR number eoSv 2015$iz-dz-
85$lsok&3] fn-07$11$2015, GR number eoSv 2015$iz-dz-85$lsok&3] fn-
25$07$2016 and any GR issued in this regard from time to

time]candidates joining the CHS PG Diploma 2020 will be
required to sign a bond to serve the Public Health Department
under GoM for a period of two years, failing which she/he will
be required to pay Rs. 5,00,000/- (Rupees Five lakh only) for
the default. (Annexure-G)
B) The bond is to be written on Rs. 100/- (Rupees 100 only) stamp-
paper, signed by the candidate and should be notarized.
C) The Proforma for bond is given in Annexure-H.
3. Penalty: A) The candidate who has joined for PG diploma course should
complete the tenure, failing which she/he will be required to pay
Rs.10,00,000/- (Rupees Ten lakh only) for non-completion of
tenure.
B) Any candidate responsible for the lapse of CHS PG Diploma Seat under
any circumstances will have to pay penalty of Rs.10,00,000/- (Rupees
Ten lakh only).
C) Candidates who have submitted status retention form and subsequently
do not join the institute will be liable for seat lapse penalty
Rs.10,00,000/- (Rupees Ten lakh only).
D) Any candidate responsible for causing seat lapse will not be allowed to
take admission under CHS CPS for next two years.
4. Cancellation and Refund:
A) Candidates who have joined the institute and due to any reasons
want to cancel the admission then CHS has no liability for refunding
the fees.
B) If a candidate expires or becomes unfit for undergoing medical post
graduate diploma course as per MCI norm decided by notified
Medical Board within 3 months from the date of admission, full
refund shall be made.

15. DURING THE COURSE
A) Candidates are supposed to be on call for 24*7 a week. They can
avail up to 8 days of leave per year. Leaves taken/absentee beyond
this limit will cause the term to be delayed for completion by those
many days.
B) Candidates can avail library facility for study established in the
institute as per rules of the institute.
C) Candidates will have to undergo compulsory trainings,
supplementary for their subject, time to time.
D) Any behaviour causing harm to the institute/patients or their
relatives/co-workers, disturbance in service provision or indecent
behaviour towards patients or their relatives/co-workers will lead
to immediate cancellation of admission.
E) Candidates cannot work as an individual medical practitioner
during course period. Any such act will be liable for legal action
under ‘professional misconduct’.

CONSTITUTIONAL AND OTHER RESERVATION
1. The candidates recognized by the State of Maharashtra as belonging to
the following backward classes, shall alone be eligible to claim
admissions against the seats reserved for such backward class
categories. The constitutional reservation at Directorate of Health
Services is as mentioned below:

2. Candidates belonging to aforesaid backward class categories will be
required to submit the Caste Certificate and Caste/Tribe Validity
Certificate.
3. Caste Certificate issuing authority: Caste Certificate stating that the
caste is recognized under a particular backward class category in
Maharashtra State is issued by Sub Divisional Magistrate/Executive
Magistrate/Metropolitan Magistrate in Maharashtra State.
4. Reservation for Socially and educationally Back word classes (SEBC) is
applicable as per Maharashtra Act no. LXII of 2018 and its amendment
Maharashtra Act no. VII of 2019 dated 3rd July 2019(Annexure –D)
5. The Caste/Tribe validity Certificate issuing authority is as follows: a) For candidates belonging to Scheduled Caste, Scheduled Castes

converted to Buddhism, Vimukta Jati, Nomadic Tribes 1,2&3 and
Other Backward Class, Caste Validity Certificate is issued by
Divisional Caste Certificate Scrutiny Committee of respective
Divisional Social Welfare Office (Konkan, Pune, Nashik,
Aurangabad, Amravati and Nagpur)
b) For candidates belonging to Scheduled Tribes the Tribe Validity
Certificate is issued by Director/Deputy Director, Tribe
Certificate Scrutiny Committee of respective regions (Konkan,
Pune, Nashik, Aurangabad, Amravati and Nagpur)
6. The candidate should have claimed the constitutional reservation in the
online application form. The candidate claiming constitutional
reservation must submit original caste validity certificate on or before
the last date of online application form submission failing which the
category claim will not be granted.
7. NON CREAMY LAYER-
A candidate belonging to ‘Creamy Layer’ amongst the categories (C) to
(G) must note that the provision of reservation is NOT applicable to
him/her. A candidate claiming benefit of reservation under the
categories (C) to (G) above will be required to produce ‘Non-Creamy
Layer Certificate’ as specified in the Government Resolution.No.CBC-
10/2008/CR- 697/VJNT1, dated 24th June 2013. The certificate should
be valid up to 31/03/2020. The Non-Creamy Layer Certificate is issued
by Sub Divisional Magistrate/Deputy Collector/Collector of the district.
However such a Non-Creamy Layer Certificate shall be produced in any
case on or before the last date of online application form submission
failing which the category claimed will not be granted.
APPLICANTS BELONGING TO SPECIAL BACKWARDCLASS
N.B.: Hon’ble High Court of Mumbai had stayed admission to SBC
category as a separate quota and therefore, vide Government Circular
from Social Welfare, Cultural Affairs and Sports Department No. CBC-
1095/WS/264/BCD-5 dated 24th October 1995, the persons belonging to
Special Backward Class(SBC) are to be considered as OBC candidates.
8. Inter-se amongst the reserved categories will be operated at the end of
each admission process against the unfilled reserved categories seats as
per merit and choice of reserved categories candidate.

The seats remaining vacant from various categories will be filled
in during inter-se admission process as follows:
a) From among the candidates of their respective group from the In-
terse Merit List of that particular group where the vacancy exists.
The group are as follows:
GROUP – I
(i) Scheduled Castes and Scheduled Caste converted to Buddhism(SC)
(ii) Scheduled Tribes including those living out-side specified area(ST)
GROUP – II
(i) Vimukta Jati (DT)(A)
(ii) Nomadic Tribes(NT1(B))

b) If the seats still remain vacant, then the seats will be filled, from
among the candidates of all categories mentioned above from the
all categories combined merit list.
c) If the seats still remain vacant, then the seats will be filled, from
among the candidates of the common merit list.
d) Seat remaining vacant under SEBC category will be available to
open category during the inter-se round.

9. Ear Marking: Reserved category candidate entitled to admission on the
basis of merit in an open category seat will have the option of taking
admission either against her/his respective category seat or on an
open category seat. If she/he opts for reserved category she/he will be
deemed as an open category candidate and not as an reserve category
candidate. One seat in the institute where she/he was eligible for
admission against open seat shall be earmarked for a candidate
belonging to the respective reserved category. Such reserved seat will
be made available immediately to the successive reserved category
candidate from the same category merit.
10. Admission under constitutional reservations will be governed by the
Maharashtra Schedule Caste, Scheduled Tribes, Denotified Tribes
(Vimukta Jatis), Nomadic Tribes, Other Backward Classes and Special
Backward Category {Regulation of Issuance and Verification of Caste

submitted original documents back unless I pay the penalty fees.
2. I will not behave in a manner that may cause harm to the patients or
their relatives/co-workers, cause any disturbance to service provision
of the institute, indecent behaviour towards patients or their
relatives/co-workers or indulge in professional misconduct. If I fail to
do so, I will be liable for immediate cancellation of admission and pay
penalties of Rs. 10,00,000/- (Rupees Ten Lakhs Only) for causing seat

3. During the course period, I will not work as individual medical
practitioner anywhere else. If I fail to do so, it will be considered as
professional misconduct and I will be liable for immediate cancellation

Only) for causing seat lapse. I accept the term that in this situation, I
will not get my submitted original documents back unless I pay the
penalty fees.
4. On completion of my PG Course, I will be bound to serve Public Health
Department of Maharashtra as per government order for a minimum
period of two years. If for any reason, I fail to serve or choose to leave
the bond period in that event DHS will be entitled to claim and recover
from me a sum of Rs. 5,00,000/- (Rupees Five Lakhs Only) as
penalty amount as per the GR Dated: 25 July,2016.
5. In the event I fail to comply with conditions of this Undertaking, the said
amount of Rs.10,00,000/- (Rupees Ten Lakhs Only) or Rs. 5,00,000/-
(Rupees Five Lakhs Only) shall become forthwith payable by my parents /
guardians / sureties jointly and severally at the discretion of the
Commissionerate of Health Services and the Commissionerate of Health
Services will be entitled to seek legal remedies against my parents /
guardians / sureties. If I am an in-service candidate, this penalty sum can
be recovered from my salary at the discretion of the Commissionerate of
Health Services.

General Instructions for filling online application Form
1. Before filling Online Application form Download INFORMATION BULLETIN and read the same carefully. Candidates ascertain the eligibility before filling application form and paying fees.
2. Candidate should note the password given by Him/Her and keep in safe custody. It is necessary for further login. Safe custody of the password is the responsibility of candidate.
3. Please note that if a candidate commits any mistake in filling the online Application Form, it cannot be corrected by this office. The Office will take no responsibility of any kind for such forms.
4. The Applicant must possess the required criteria and education qualification as mentioned in the Information Bulletin.
5. Applicant email id and mobile number will be used for further correspondence.
6. After reading above instructions to fill Online Application Form, the Applicant has to click Go to New Registration button to display Online Application Form.
7. Candidate should Login with Application No. (User Name) and his/her Unique Password and verify the correctness of entries made in application form. There is provision to make changes in the Application Form before SUBMIT. There is no provision to make changes in the application form after SUBMIT button is clicked. Candidate will have to pay registration fee at CHS Bank account, the details of which are given in the Information Bulletin. Mandatory details about transcation are required before submitting the form.
8. After successful Payment, candidate should upload required Documents before the scheduled date. The candidate should pre-scan the documents, name it and keep ready on the desk. Instructions For the Image Upload is As follows
o Size of each document must be less than 500 kb. o Width (Horizontal) of document must be Greater than 400px o Height (Vertical) of document must be Greater than 600px. o Document must be in .pdf Format only.
9. Office will do Online Scrutiny of uploaded Documents. Candidate who has not
