A 55 year old American photographer returned home

after a six-week trip to the game parks of Tanzania. Four

weeks after return, he developed headache, myalgia,

arthralgia, weight loss and intermittent fever. The patient is

noticeably confused and distracted, and physical

examination reveals hepatosplenomegaly and

lymphadenopathy in the supraclavicular and cervical regions.

The hemogram is normal, but the cerebrospinal fluid contains

20 lymphocytes per mm3. What is your

In the previous module we saw that one possible treatment option for Trypanosomiasis – African is the use of Pentamidine. In the Therapy module we can find information on Pentamidine by selecting it from the list of drug choices

To explore the Microbiology module we will refer to one of our recent example cases: “Creative DeBugging”A 47 year old man is found to have a large abscess in his left leg. Routine cultures reveal a branched, aerobic, non-sporulating, Gram-positive bacillus. What micro-organisms should be considered ? What procedure should the laboratory perform at this point ?Phenotypic tests were entered into the Microbiology module of GIDEON:

Pressing the Identify button gives us these results

The organism in question appears to be one of several species of Nocardia. In order to determine the most discriminative laboratory test to perform at this point, we will check all of the organisms, and press ‘Compare’.

As we see in this screen, GIDEON recommends that we examine Citrate, myo-Inositol and Mannitol [top three tests].

…Our organism was ‘positive’ for all three of the additional tests. After re-identifying the organism using those results we can see in this screen that the most likely identification in this case is Nocardia brasiliensis.

By click on the pathogen name we can display further information on the selection
