decedent’s survivor who is handling the funeral arrangements is entitled to receive

a copy of any preneed agreement in the

possession of the funeral establishment that has been signed and paid for, in full or

in part, by or on behalf of the decedent.

IT IS CUSTOMARY TO PAY FOR PROFESSIONAL SERVICES AND MERCHANDISE

AT THE TIME OF ARRANGEMENTS.

BRUNE MORTUARY IS NO LONGER IN A POSITION TO OFFER FINANCING,

THEREFORE, PAYMENT OR THE SPECIFIC METHOD FOR PAYMENT IS

REQUIRED PRIOR TO CREMATION, THE FUNERAL OR MEMORIAL SERVICE.

FOLLOWING TYPES OF PAYMENT WILL BE ACCEPTED:

1. CASH OR PERSONAL CHECK

2. LIFE INSURANCE ASSIGNMENT (Policy must be verified and be willing to work with the Funeral Home.)

3. ESTATE OR PROBATE CASES (We do accept Estate or Probate accounts. Due to the undetermined amount of time of settlement, we make a $700.00 charge of this service which includes filing the Creditor’s Claim), plus 10%

interest per month until paid.

4. VISA or MASTERCARD/(at time of need)

5. We do not accept VA payment In all cases, financial arrangements must be completed prior to any services rendered

THE GOODS AND SERVICES SHOWN BELOW ARE THOSE WE CAN PROVIDE TO OUR CUSTOMERS YOU MAY

CHOOSE ONLY THOSE ITEMS YOU DESIRE. HOWEVER, ANY FUNERAL ARRANGEMENT YOU SELECT WILL INCLUDE

A CHARGE FOR OUR BASIC SERVICES OF FUNERAL DIRECTOR, STAFF AND OVERHEAD. IF LEGAL OR OTHER

REQUIREMENTS MEAN THAT YOU MUST BUY ANY ITEM WHICH YOU DID NOT SPECIFICALLY ASK FOR, WE WILL

EXPLAIN THE REASON IN WRITING ON THE STATEMENT WE PROVIDE DESCRIBING THE FUNERAL GOODS AND

SERVICES YOU SELECTED.

SERVICE CHARGE

A. MINIMUM PROFESSIONAL/ADMINISTRATIVE, FACILITIES & EQUIPMENT

We charge every family for our basic service which includes: staff assistance in response to your

initial request for service consultation, service plans with cemetery, crematory, and clergy. Also included is

a fee to cover a proportionate share of our overhead costs such as taxes, licenses, insurance, maintenance of

facilities and equipment, utilities, and a host of other costs related to being in business to serve the public

twenty four hours a day, seven days a week. This fee for our basic services and overhead will be added to

the total cost of the funeral arrangements you select. This fee is already included in our charges for immediate

burial, immediate cremation, forwarding or receiving remains.

2. IMMEDIATE CREMATION $ 635.00

3. SERVICE CHARGE FOR SHIP OUT/SHIP IN/DIRECT BURIAL $ 995.00

4. MINIMAL SERVICE INVOLVEMENT $ 395.005. SATURDAY SERVICE FEE $ 350.00
