• Hypoxia• Metabolic imbalances such as hypoglycaemia• Falls and trauma to head• Unresponsiveness• Neurological disease processes Eg. stroke,

brain tumor,& epilepsy• Post-anaesthesia• New admission to form a baseline assessment.

• Severe, with GCS ≤ 8• Moderate, GCS 9 - 12• Minor, GCS ≥ 13.
