Interested eligible candidates are invited to appear in the 'Walk-in-Interview' on 2nd February,2018 at 9.30 am to 10.00 am at the office of the Chief Medical Officer of Health (old building),Balurghat, Dakshin Dianjpur, Pin- 733101 for the contractual post of 01 (one) 'Para MedicalWorker. (PMW)' under NLEP, Dakshin Dinajpur on purely contractual basis at a consolidatedmonthly remuneration of Rs. 16,0001- per month. All interested candidates are requested to attend thesame with their Original testimonials along with a prescribed application form.

Selection criteria :a) Essential:-

i) Passed Higher secondary or equivalent with having PMW training certificate fromGovt. recognized institute.

ORM.S.W.IB.Sc. with 3 years experience in the field of health.Working knowledge of computer.Age should not exceed 65 years as on 01.01.2018.The applicant should be permanent resident of West Bengal. Self attested photocopy of document certifying permanent residential status of this state (Passport orEPIC or AADHAR card or PAN Card or Driving License or Ration card).Self attested photocopy of the all mark sheets and certificate.Self attested photocopy of Birth Certificate, Admit Card I Certificate of MPExamination or equivalent Examination affiliated by any Board recognized by Govt.of India or any State Govt.Self attested P.P size photograph (2 copies).All document for which photocopies will be attached with the application, must beproduced in original in the verification board before entering into the interview hall.An application fee in form of Demand Draft of Rs. 100.00 for General applicant andRs. 50.00 for reserved categories to be submitted with the application. The DemandDraft should be in favor of District Health & Family Welfare Samiti, DakshinDinajpur payable at Balurghat.No TAIDA will be paid to the candidates for the selection test I interview.Authority reserves the right to cancel all or any application without assigning anyreason.
