Core Aims of Integration



Integrated Care Teams - Vision• Co-located Integrated locality teams.

• Four teams for North Somerset

• First Team started on 28th June 2013 and will be evaluated over

the next 6 months

• Background work to develop the other sites in the meantime.

• Strong national evidence and public support for integrated services.

• Much of the Health and social care system is already working in an integrated way.



The Weston Health and Social Care Team

• Based on the 2nd Floor of the Town Hall

• Went ‘live’ on 28th June 2013

• This team is working together to:

• Develop joint care pathways• Identify and work with the most complex

individuals• Develop a joint management structure



Area The Team Covers

• The Team will cover people who are registered with the following GP surgeries

• Longton Grove Surgery• Milton Road Surgery• New Court Surgery• Graham Road Surgery• Locality Health Centre• Tudor Lodge Surgery• Clarence Park Surgery



Evaluation and Measures of Success

• External evaluation of the project is taking place and being led by:

1. R & D Integration 2. University of the West of England (UWE)

• Measures of success and Key performance indicators have been agreed

• There is a focus on findingefficiencies



Next Stage - Worle

• The next Health and Social Care Team will be based in St Georges Health Centre Worle.

• This should move forward towards the end of the year
