• The Brockton Neighborhood Health Center is co-located with Vincente’s Tropical Supermarket

• Expansion of Health Center allowed for an additional 6,700 patients to be treated annually

• Sites collaborate on a nutrition program, complete with demonstration kitchen in supermarket to showcase healthy eating and food preparation,

• Partnership demonstrates food as medicine, nutrition benefits and prescriptions

• Serves patients of Health Center and greater Brockton community

• The network has led to an increased understanding of community needs and barriers
