Foundations in Palliative Care:A programme of facilitated learning for care home staff

What is the tool?• Updated educational resource building on the previous toolkit
reflecting the current changes and direction within palliative
care.
• Purpose is to offer the opportunity to care home staff to develop
and enhance their skills in caring for their residents many of
whom will have multiple long term conditions.
• The pack contains 4 modules:
• 1. First principles
• 2. Communication
• 3. Pain and symptom management
• 4. Bereavement Care
How and Who should use this?• Toolkit is designed to be facilitated by an experienced
facilitator with relevant knowledge base, delivered within the care home
• Several models can be used eg. direct face to face by an external provider on a rolling programme, or a ‘train the trainers’ approach
• Modules designed to run over a month with workshops weekly ( ideally)
• Aimed at staff of all levels (dependant upon level of skill and experience in generalist palliative care)
• Toolkit contains facilitators manual and participant workbooks as well as information for the care home manager
How can you get a copy of the toolkit?
• Can be supplied free to Macmillan professionals ( i.e. Macmillan nurses- many of these professionals are the palliative care education champions within each NHS health board area)
• Is available to individual care homes for a small donation (suggested £5-£10 each)
• Can be supplied to larger numbers of care homes for the cost of the print run- cost depends upon the number required.
• The key to success is FACILITATION so ensure you have appropriate available facilitator before you order one!!
