MAKING EVERY DAY COUNT

A life-limiting and debilitating condition, such as cancer, may often lead to a stage where curative treatments are no longer an option. At this time, what matters the most is spending precious time with family and friends, and being relieved from the pain and other related debilitating conditions.

Indeed, in many countries across the globe, such patients move seamlessly to palliative care, taking it for granted, so that they can be pain-free and comfortable; and that they may live life to the fullest possible till the very end.

Sadly, in India, we lack sufficient access to multidisciplinary teams of experts in palliative and end-of-life care. Even worse is when hospital care is beyond the means of a family, and patients have no option but to continue in discomfort and pain at home.

INSPIRING COMPASSIONATE CAREEstablished as a public charity in Maharashtra on 30th March 2015, The Jimmy S Bilimoria Foundation has a single-minded purpose - to deliver a much-needed palliative care facility in the city of Mumbai, primarily to cancer patients. Its PALCARE service itself was launched a few months later on 1st December 2015, when it enrolled its first patient in the heart of Girgaum, Mumbai. By 31st August 2017, PALCARE had provided its service to close to 300 patients, across the length and breadth of Greater Mumbai; and the number of new enrolments keep growing with each passing month.

With the financial backing of several foundations, corporations and individuals but most importantly the Tata Trusts, the Mahindra Foundation, the Kalpataru Trust and DASRA, our efforts are concentrated in Mumbai before expanding to other parts of the country.

Challenges We have had our share of challenges, the most exasperating being finding doctors who are capable and willing, as even very few among the medical community are aware of the benefits of palliative care. Astonishingly, there is only one institute in the whole of India that provides a MD course in palliative care - The Tata Memorial Hospital. Even more astonishing is that it is still not more than a topic on the curriculum of medical colleges, whereas it deserves a specialized degree course of its own. This lack of knowledge on what palliative care can do for patients in the medical community is not specific to PALCARE but is faced by all similar organisations in India, and every effort must be made to spread awareness, both at the government health ministry level and by individual palliative care facilities.

PALCARE has already begun the process of advocacy through press articles, interaction with students, and regular email updates; as also face-to-face meetings with select groups of oncologists, onco-surgeons, radiologists, general physicians and the Deans of leading hospitals in Mumbai. Our efforts seem to be paying off and this is evident on two fronts:

i) Many more oncologists and onco-surgeons are referring their patients to PALCARE today;ii) PALCARE has acted as a catalyst, as several new facilities have begun to spring up in Mumbai.

However, there is still a long road ahead before palliative care gets the full recognition it deserves.

When in 2013, I saw first hand, how little the medical profession, barring a handful of compassionate doctors, understood the special needs of people with advance stage debilitating conditions such as cancer, I resolved that this must change. I thereby began my journey.

Starting The JourneyMy first point of call was Mr. Ratan Tata and the Tata Trusts, who were amazing in their support. Thereafter, I approached a few good friends, who collectively helped me get off the ground and on 30th March 2015, The Jimmy S Bilimoria Foundation was registered as a charitable trust under the Maharashtra Public Trust Act 1950.

Next, along with Aneka Paul of the Tata Trusts, I went about putting the team together, beginning with our Chief Medical Officer, Dr. Poniserry Jayarajan, from Kerala which is the most advanced state in this field in all of India. At around the same time, we also recruited our Chief Psychologist, Devaunshi Mehta and our Head Nurse, Dirk D'lima. Dr. Jayarajan brought with him close to 8 years’ experience in palliative care, a rarity in those days. Moreover, he shared the same vision and goals as my trustees and I. Together, we set up a well-structured, compassionate and technically sound service, delivering homogeneous palliative care at the homes of our patients delivered by a well-trained, dedicated and caring team of doctors, nurses and psychologists.

Financial SupportLast but not the least, our challenge lies in deriving sufficient funds to provide the service to all sections of society, free of cost. Without the backing of the Tata Trusts and other benefactors such as Anand Mahindra, The Mahindra Foundation, The Kalpataru Trust, DASRA, The Reliance Foundation, The Bajaj Foundation, The Pirojsha Godrej Foundation, and several other individual donors, PALCARE would not have been able to get off the ground and continue its work, unhindered. We remain ever grateful. Many patients, both affluent or on tight budgets, have come forward with donations.

But our needs are ever-growing, and are on an annual basis. We urge that anyone who can offer even a small amount please do so, as to help us with our work for many years to come, or till we can become self-sufficient.

Our journey has only just begun. We hope PALCARE will be able to expand its reach and spread its benefits for not only this generation but future generations as well.

Another one of our challenges has been that many patients and their families were unable to come to terms with palliation, seeing it as a sign of ‘giving up hope’ or ‘letting down’ the patient family member by not fighting to the very end. This is particularly so amongst the upper echelons of society where finances are not a primary concern and, therefore, the ability to run from one line of treatment to another is not a constraint.

But let’s make it clear: PALCARE does not advocate that patients and their families must give up hope; or that new lines of treatment need to be completely ignored, across the board. What PALCARE maintains is that patients must not unnecessarily be made to suffer and that every effort must be made to alleviate the pain and other debilitating symptoms, bringing the patient and their family to a state of emotional stability, whether there is hope or not.

Some of our patients are in severe pain, discomfort and depression when PALCARE first visits them. Their families too are traumatized by the situation and fear of the unknown. But within a couple of visits, thesedebilitating factors are brought under control, and the patient and family get a new lease on life. In so doing, PALCARE works towards providing a better life and relief from pain, to patients of all ages, as well as all stages.

An experienced and compassionate medical team is paramount to delivering a service that aims to make each and every patient pain free, and bring them to a state of serenity, while also taking care of their emotional needs. The Medical team is, therefore, the very core of the foundation and no efforts are spared to bring it up to the best international standards.

Dr. P Jayarajan leads and supervises the medical teams. His primary responsibility is to ensure a superlative facility, where systems and structures are religiously maintained, best practices are followed and where the team places utmost emphasis on the patient and their family's needs. He also ensures that the organization stays abreast of new developments in the field and brings the best for the benefit of the patient.

Additionally, the Chief Medical Officer is constantly honing the skill sets and knowledge of the doctors and nurses with the help of the Medical Officers.

PALCARE places utmost importance on the role psychologists plays in a palliative care set up. Moreover, end-of-life care not only strives to alleviate a patient’s physical suffering, but also helps both patient and family

confront and deal with fears of dying and loss. PALCARE focuses on bringing both patient and family to a state of emotional stability and satisfaction, as this aspect is crucial to the overall well-being of the patient.

Both our counsellors, unlike in some other palliative care organisations, are qualified and trained psycholo-gists. As our service expands, we will increase the number of psychologists

Our team of 7 nurses, almost all of whom have been with us from inception and are the core of the service, lovingly and regularly visit patients, keeping in touch by by phone even when they don't visit and make sure that every need of the patient and their families are addressed as best as possible.

Guided and supervised by the CMO and his assistant doctors, nurses visit patients every week, interact with them and their caregivers, plan and monitor the medical protocol and provide feedback to the lead doctor in charge of the patient. They are the face of PALCARE.

TrainingAt PALCARE, the best practices and treatments are presented and to achieve that, our staff goes through rigorous training and constant monitoring.

• Training Programmes include an intensive 6-week certificate course at the pre-eminent Tata Memorial Centre and occasionally at CanSuport, New Delhi.• Training Programmes include in-house sessions, study tours, annual refresher courses and attendance at conferences and seminars.

Mrs. Rozzie D’Souza*The Joy Of Being At My Daughter’s Wedding Rozzie D’Souza, a woman of immense religious faith, had been the primary care giver to her husband through his illness. She now herself required end-of-life care, as she struggled with the symptoms of the ovarian cancer that refused to leave her. To her daughters, even though both were medical professionals, palliative care was a new experience; but they willingly embraced it as a curative treatment option for their mother.

At the time of PALCARE’s first visit, Rozzie made it clear that her main concern was to be able to attend the upcoming wedding of her younger daughter. Symptoms of pain, persistent vomiting, constipation, nausea, fatigue, and a lack of appetite made her uncertain about being able to be present, to give her daughter away. PALCARE assured her that the team would do everything in its power to manage her symptoms and keep her comfortable; and hopefully, help her be present at the wedding.

As the wedding date approached, Rozzie’s symptoms began to play up but PALCARE spared no efforts to make her stable again. The medical team’s huge efforts were rewarded when a family friend of the bride sent a WhatsApp photograph from the wedding reception, showing a smiling Rozzie by the side of the newly-weds.

She had almost not been able to make it to the wedding ceremony as she was in terrible pain that very morning. But PALCARE’s CMO had prescribed suitable medicines which alleviated the pain, allowing her to travel to the venue for the nuptials and amazingly, she was able to stay on for the reception.

"I couldn't eat a lot, but I made sure to taste everything", Rozzie later recounted to the PALCARE team with a glint in her eye. A few weeks later, she passed away peacefully at home in the presence of her daughters.

HOW PALCARE HELPEDMiss Radhika Patel*Upholding Our MissionHaving come to terms with the fact that Radhika was beyond cure and her time with her family was limited, they decided that the best place for her was being comfortable in their own home. In addition to suffering from ovarian cancer, Radhika had embattled a long-standing psychiatric condition; and despite being on medication, her life had been badly affected as a result.

That same day, the PALCARE team paid Radhika a visit at her home. Afraid, uncomfortable and unable to express the symptoms that ailed her, Radhika typified a patient who is left directionless in the absence of a possible cure. Her mother informed PALCARE that until a month prior, Radhika had been one of the most exuberant young girls she knew; she had braved radiation and chemotherapy always with a smile on her face. But now, with cancer taking over her body, she had difficulty breathing, was severely constipated and clung to her mother constantly.

Within the first week of receiving care under PALCARE, Radhika’s breathlessness was controlled and constipation relieved. Our psychologist met up with Radhika and her family on a regular basis, to address any fears and anxieties that they might have.

Unfortunately, Radhika was brought to PALCARE at a very late stage, and it was not long before her physical health deteriorated rapidly. In May 2017, Radhika passed away in the comfort of her home, surrounded by her family, just the way the family had wanted and in keeping with PALCARE’s own mission.

Ms. Anita Mhatre *Bidding Fond Farewells Anita Mhatre was an unmarried 60-year-old woman, who came to PALCARE with stage 4 cancer of the spine. With no siblings, and a mother who passed away several years ago, Anita lived with her elderly father. They shared their home with other close relatives; but unfortunately, they were not on talking terms.

At the time of her enrollment with PALCARE, Anita was bed bound, unaware that her illness was beyond cure. She was partially paralysed, and suffered from severe pain and bed sores. Despite this condition, she remained optimistic that she would recover one day.

After a few weeks with PALCARE, Anita’s pain was brought under control. Her bed sores were being managed well and she was made physically comfortable. However, her frustration of not being able to get better continued to afflict her mental wellbeing. The team psychologist provided her constant counselling and one of Anita’s friends, her primary caregiver, was constantly in touch with the PALCARE team, seeking advice.

Subsequently, the full extent of Anita’s condition was revealed to her by the treating physicians. Even though shocked, Anita seemed relieved. On discussion with the PALCARE team, she decided to mend ties with her relatives, knowing she would have to leave her father in their care.

Anita’s close friends brought her foods she most enjoyed, watched her favourite movies with her and talked of wonderful memories of the days they had had together. A few days before she died, she thanked the PALCARE team saying she wouldn’t have had such a loving sendoff had it not been for PALCARE’s support.

Anita Mhatre passed away in the comfort of her home, surrounded by friends and family, and without suffering. Just what PALCARE had hoped to achieve.

Mrs. Sareena Khan*Her Own Home At Last In October 2016, Sareena Khan, a middle-aged woman in her early 50s, walked into PALCARE’s office with end-stage cancer, seeking palliative care. Her husband had died 5 years prior in a tragic accident leaving her to look after her two unmarried sons, now in their 20s. More than anything else, she said, at this last stage of her life, she wanted a new home for her sons which would be their very own.

On the first home care visit, the PALCARE team spent time with the family, carefully explaining the prognosis and what end-of-life care is. They assessed Sareena’s symptoms and started her on morphine, immediately bringing her pain and breathlessness under control. She did well for some time.

Alas six months later, Sareena suffered from a tragic fall and slipped into a coma. Her two young sons were devastated and needed emotional support, which the PALCARE psychologist ably provided. To their

immense delight, exactly four days before she passed away, Sareena’s wish was fulfilled when her sons were able to close the deal for their new home.

Our service is 100% free, and is therefore dependent on donations. The show of support has been enormous and we are indeed grateful. We still have a long way to go, if we are to ensure unhindered continuity of our service. But we have been very fortunate so far indeed.

Besides all this, is the cost of Training and Continuous Education Programmes.
