Ms. Stelzner is a board certified Adult NursePractitioner. She earned herBachelor of Science degreefrom the University of Tampaand her Master of Sciencedegree from the University ofSouth Florida. Ms. Stelzner hasover 10 years of cardiac experience and has a specialinterest in acute cardiac careand lipid management. She ismarried and has two children.

Dr. Fontanet was born andraised in Puerto Rico. He completed his cardiology fellowship at WashingtonUniversity School of Medicinein St. Louis, Missouri. He isboard certified in InternalMedicine and CardiovascularDiseases and Echocardiographyand has special competencyCertification by the Council of Nuclear Cardiology andNASPE. Dr. Fontanet is marriedwith three teenage children.

Dr. Farooqi was born inPakistan. He has lived and practiced Cardiology in theUnited States since 1976. Hecompleted his Cardiology fellowship at UniversityHospital of West Virginia.Married with three teenagechildren, Dr. Farooqi brings an abundance of experienceand knowledge to our FloridaMedical Clinic CardiologyDepartment. He is board certified in Internal Medicineand Cardiovascular Diseases.

Nuclear Stress Test

Nuclear Imaging is used in the evaluation of individualswith chest pain and other symptoms caused by physicalexertion and in the follow-up of patients treated for hardening of the arteries with such procedures as balloonangioplasty or bypass surgery. During nuclear imaging smallamounts of radioactive materials are used along with a specialized camera and computer to provide precise picturesof the heart and arteries that can detect blockages of arteries.

A New Image ofCardiology

Echocardiogram and Doppler

Echocardiography is a procedure that uses ultrasoundwaves to create images of the heart. Sonar waves are directed through the heart via a probe placed on the chest.Reflected sound waves are harnessed to produce heartimages. The images are analyzed to assess how the heartfunctions as a pump, the integrity of the heart valves, andassess the overall anatomy and function of the heart.

24 Hour Holter and Event Recorders

Event monitoring utilizes a small, portable, battery-powered machineused by a patient to record an electrocardiogramover a long period of time.Patients may keep the recorder forseveral weeks. Each time symptomsare experienced, the patient presses abutton on the recorder to record the EKGsample. As soon as possible, this sample is transmittedto the doctor’s office by telephone hookup for evaluation.

Tilt Table Test

The tilt table test is designed to evaluate the function ofthe patient’s blood pressure sensors and ability to maintain a normal blood pressure. During this simple examination thepatient is standing up, strapped to a table at 80 degrees.Vital signs and electrocardiogram are constantly monitored.This study is used in the evaluation of dizziness and faintingspells.

Anticoagulation Clinic

Offers safe and effective treatment of patients on anticoagulation. In certain patients, anticoagulation withCoumadin is necessary. The dose of this medication, unlikethat of most, needs adjustment since each patient respondsdifferently. The Anticoagulation Clinic is designed to systematically assess the level of anticoagulation andpromptly adjust the dose of blood thinners.

Adult Congenital Heart Disease

Because of the relative rarity and complexity of surgicallycorrected congenital heart disease, a new subspecialty ofcardiology has developed to provide adult patients withcongenital heart disease the appropriate expertise in theircare. Dr. Fontanet is a board certified cardiologist specializingin non-invasive cardiac diagnosis and congenital heart disease.

Arrhythmias and Syncope

Arrhythmias are disorders of the regular beating of theheart. Arrhythmias can occur in the healthy heart and be of no clinical significance. They can also indicate seriousproblems that can lead to stroke or sudden cardiac death.Holter Monitor, Transtelephonic Event Monitor and Tilt TableTesting are among the services provided in the ArrhythmiaClinic.

Atherosclerosis and CoronaryDisease Prevention Clinic

This comprehensive programallows for the diagnosis and treatment of any form of atherosclerosis in the body. Anumber of diagnostic studies are available to establish early diagnosis including nuclear cardiology, vascular ultrasoundand MRI angiography.

Congestive Heart Failure

One of Dr. Farooqi’s areas of expertise is heart failure.The dedicated clinical staff of the Heart Failure Clinic providecomprehensive work-ups and follow-up with attention todetail. This is a necessary element in successful treatmentprograms. The clinic offers outpatient inotropic infusions,implant and follow up of Bi-ventricular pacemakers.

Lipid Clinic

The clinic emphasizes treatment of common and unusualforms of lipid disorders. Several studies have shown thatLipid Clinics such as the one at Florida Medical Clinic will bethe most effective in preventing progression of coronaryartery and peripheral vascular disease.

Pacemakers and ICDs

Our Pacemaker Clinic was established to provide appropriate follow up after the completion of electrophysiologic procedures, pacemaker and ICDs implantation. Dr. Fontanet has approved special Certificationand Competence in Pacemakers-Defibrillators (NASPE).

Pulmonary Hypertension

Pulmonary hypertension is a rare and life threatening illness. The clinic offers expert evaluation as to the cause of the disease and the opportunity for advanced treatmentoptions and referral to other specialists involved in the careof patients with pulmonary hypertension at Florida MedicalClinic.

Laboratory Results

Our physicians will thoroughly review the results providedby the diagnostic center before the information is releasedto you. In most cases, we will have the results to you within 3-5 business days of the testing. A nurse or medicalassistant will contact you with your results. If you have notheard from us within 5 days, we ask that you call the officeand ask to speak with a nurse about your test results.

It is important to take your prescription medications regularly. Therefore, we request that you contact our officeat least three days before you run out of medication. FAX ortelephone the office between 8am and 4pm for prescriptionrefills. Please provide the following information:
