Bronze Award Evaluation Form
Bronze Skill Award Evaluation Fly Scoring Instructions:
Please print this form, complete it as you evaluate the files. When the evaluation is completed to your satisfaction and the tier has met the requirements for the award please forward this Evaluation Form, the flies, and the completed Bronze Award Fly Transmittal Form to a member of the Evaluation Review Panel for verification and award. Once successfully reviewed by a Review Panel member, the office will be notified, payment received verified, and the tier will receive his/her award. Scoring:
Score each Aspect of each pattern and calculate totals on page 15.
Scoring is based out of 10 Points:
10 - 9 Points = Excellent 8 - 6 Points = Satisfactory 5 - 1 Point = Needs Improvement
Tier must achieve an overall score of 75% to pass on both the required evaluation flies and the tier choice flies.
Four of the Five evaluation flies must pass (75%)
Each of the Three Tier Choice flies must pass (75%)
Overall score is calculated on Evaluation Summary page 15
