COVERAGE DETAILS

Hospital Confinement Benefit: Pays a daily hospital confinement benefit if an insured person is confined

to a hospital (excluding ICU) as a result of injury or sickness. There is no elimination period on this benefit.

Hospital Admission Benefit: Pays if an insured person is admitted as an inpatient to hospital as a result of injury or sickness.

Emergency Room Treatment Benefit: Pays a benefit if an insured person receives services in the emergency room of a

hospital as a result of an injury or sickness.

Outpatient Physician’s Office Visit Benefit: Pays a benefit if an insured person visits a physician’s office, clinic, or

urgent care facility for treatment of an injury or sickness.

Wellness and Preventive Care Benefit: Pays a benefit for certain wellness screening exams or immunizations.

Rehabilitation (Continuous Care) Benefit: Pays a daily benefit if an insured person requires continuous care upon

discharge from a hospital. Continuous care requires the need for a skilled nursing facility, rehabilitation facility, rehabilitation unit, home health care, or hospice care.
