  • WHAT FACILITIES SHOULD DEVELOPE A WATER SAFETY PLAN AND CONSIDER POINT-OF-USE FILTERS?The facilities that should be con-cerned with Legionella are those containing domestic hot water distri-bution and those that expose the pa-tients or occupants to water points of use likely to emit aerosols that may cause the spread of legionella.

    WATER NETWORK MANAGEMENTThe internal water distribution net-work integrated into the building, whether for tertiary sector or res-idential use, is generally supplied with water by the public network. A good water management within the building must ensure its microbio-logical quality. The 1998 European drinking water Directive, transposed into French law in 2001, introduced control of the water quality at the point of use for human consumption and not just at the building entrance as before.

    Thus, the quality of distributed water is no longer the only responsibility of water suppliers but also of buildings owners and managers.

    The buildings water networks design and its monitoring in should minimize health risks. Degradation of water quality can generate a proliferation of Legionella, Pseudomonas or atypical mycobacteria, which are considered opportunistic pathogens for humans. Local authorities provide good water quality to the population.

    However, they cannot deliver a water without any microbial risk due to:

    > Complexity and disparity of thedrinking water distribution network

    > Changes of the building originalwater networks

    > New water systems

    > New bacteria outbreak

    WHAT ARE THE POINTS-OF-USE AT RISK AND WHO ARE THE PEOPLE TO PROTECT?The “High Risk” points of use (POU) expose the patient or resident to an aerosolized form of water where wa-ter particles are able to be inhaled; this aerosolization primarily occurs at shower heads and faucets. Every-one is potentially vulnerable to legio-nella bacteria during his or her life, depending on their health status.

    However most at risk are people, with weakened immune systems (also known as immunosuppressed) such as Infants, the Elderly, peo-ple suffering from chronic disease, smokers, or those undergoing cor-ticosteroid treatment.

    Hotels, guesthouses, tourism residences, retirement homes, social and medico-social establishments, structures for disabled people, communities, campsites…

  • WHAT YOU SHOULD KNOW ABOUT LEGIONNAIRES DISEASELegionnaire’s disease is a severe form of pulmonary infection caused by a bacteria: Legionella. Symp-toms are various: fever, cough and breathing difficulty. The risk of con-tracting this disease increases with age, smoking and immunodeficien-

    cy. Legionella proliferates in do-mestic hot water plumbing systems where it finds various nutrients (corrosion products, iron, and or-ganic materials).

    POU filtration is defined as the use of a device applied to a single tap for the purpose of reducing contaminants in drinking water at that one tap. POU filtration can be used at specific taps, faucets and showerheads as a temporary measure to provide a physical barrier against Legionella. Hospitals have used this technology to try to reduce disease transmission (Ortolano et al., 2005).

    POU water filtration may be an effec-tive measure for remediation situa-tions if a limited patient area can be targeted. Filters can be installed im-mediately and are a better alternative than restricting showering and pro-viding bottled water.

    Many home owners; facility owners; and operators of hospitals, nurs-ing homes and hotels utilize POU membrane filtration devices, often in a proactive manner but also in response to emergencies (USEPA, 2006d). Some hospitals use POU membrane filtration treatment in areas populated with high-risk pa-tients (e.g., in oncology wards, bone marrow and solid organ transplant units, and ICUs).

    ALERT BASED ON POSITIFS RESULTS ON LEGIONELLA

    First, analyze the residents exposure risk. If it is high (generalized contamination, concentration above 1000 CFU/L, legionnaires’ disease cases, high risk patients), installation of Point-of-Use filters are the most effective response. Chemical or thermal shocks could be effective but their implementation are difficults and with low effectiveness if the proliferation cause is not identified.

  • PREVENTING EXPOSURE TO LEGIONELLA AND WATERBORNE BACTERIA Legionnaires’s Disease (LD) is the most lethal building-related illness (BRI) caused by a microorganism. Legionella has the ability to impact both occupants of a building and sur-rounding neighborhoods making it especially dangerous. Once thought to be a rare organism, public health officials now accept that legionella is present over half nation’s municipal water supplies and has been found in to 60-70 % of hospital plumbing systems.
