On behalf of the Alberta College of Family Physicians and the ASA Planning Committee, it is my great pleasure to welcome you to the 60th ASA.

We are excited to celebrate the 60th Anniversaries of the Annual Scientific Assembly and the Alberta College of Family Physicians. Also very noteworthy is the Patient’s Medical Home (PMH) theme of the 60th ASA. The ACFP began work towards PMH by hosting the 2011 PMH Leadership Forum in Edmonton. The work to actualize this vision continues at the 60th ASA with several PMH keynote and workshop sessions scheduled for presentation.

We thank all the speakers who have taken time away from their busy schedules to share their ideas and best practices at our conference. We would also like to thank the exhibitors and sponsors for their support. Please support them by taking some time to visit the displays in Salon C and other designated areas of the hotel.

In addition to the stellar lineup of keynotes and workshops, we’re also proud of the record number of research Posters and Presentations at the 60th ASA. Be sure to participate in the ASA Wrap-Up Pearls plenary (Page 11) and the ACFP’s Living History project (Page 16) which are also new this year. We encourage you to relax and have some fun by attending the events and activities noted in the ASA After Hours section of this program (Page 6).

New this year is a pilot schedule for Saturday, where you will notice shorter break times throughout the day. Please be sure to let us know your preference on the ASA evaluation form included in your totebag.

On behalf of the Alberta College of Family Physicians Board and Executive, welcome to the 60th ASA!

A few months after the formation of the Alberta Division of the College of General Practice of Canada in 1954, Alberta’s first Scientific Session took place in Red Deer, with a handful of attendees. Sixty years later in 2015, the ASA has grown to accommodate 350+ delegates, and offers a variety of learning opportunities in 55 keynote, workshop, skill sessions, Mainpro-C programs and Ancillary Sessions.

Alberta’s ASA maintains its reputation of excellence in the hand-picked, unique selection of programs presented in a smaller, intimate setting. Our ASA continues to attract

attendees not only from Alberta, but also from across the nation and the United States. I commend the ASA Planning Committee for their skill in choosing sessions that provide practical information on timely, relevant topics by expert presenters, and which our attendees find so valuable and useful.

Please be sure to participate in all the experiences the 60th ASA has to offer. Thank you for attending, and we hope to see you again at future ASAs!

ANNUAL SCIENTIFIC ASSEMBLY PHILOSOPHY & OBJECTIVESThe Annual Scientific Assembly is a conference dedicated to the primary care physician. The Continuing Professional Development (CPD) program is filled with a mix of keynote and workshop sessions and provides an opportunity to earn Mainpro-M1 and Mainpro-C credits.

The objectives of the ASA are:

• To provide an update and review about the management of a wide variety of therapeutic areas seen in family medicine.

• To impart management advice about conditions frequently encountered in practice in urban settings with specialist backup, as well as in rural settings in which limited resources and services are available.

• To develop assessment skills to determine which patients can be managed by their family physicians, and which patients should be referred to another member of the health care team.

• To provide a forum for the exchange of information and an opportunity for physicians to develop informal and formal networks with their colleagues.

• To develop an understanding of how family medicine and family physicians play an integral role in the health care system.

• To equip family physicians with the knowledge they need to succeed, and to pass this knowledge on to other physicians and patients through teaching and research.

ASA AttendanceIn keeping with ASA guidelines, attendance at the ACFP’s ASA is limited to family physicians, family medicine residents and medical students only. Adopted and supported by the ACFP’s Board and Executive in 2011, this guideline fosters a trusting and collegial learning environment for family physician-specific continuing professional development.

REIMBURSEMENT& ACCREDITATIONReimbursementThis event is eligible for reimbursement under the Alberta Medical Association’s (AMA) Continuing Medical Education (CME) reimbursement program. If you are an AMA member, and provided you haven’t already used your annual CME allotment for other events, you may submit a claim to the AMA for registration fees and expenses.

AccreditationThis program has been accredited by the College of Family Physicians of Canada and the Alberta Chapter for up to 16.5 Mainpro-M1 credits. You will receive email confirmation of credits earned following the conference.

ASA Accredited Program – Conflict (s) of Interest DisclosureIt is the policy of the ACFP to ensure balance, independence, objectivity and scientific rigor in all its accredited educational programs. Speakers and planning committee members participating in ACFP accredited programs are required to disclose to the program audience any real or apparent conflict(s) of interest that may have a direct bearing on the subject matter of the continuing education program. This pertains to relationships within the last TWO (2) years with pharmaceutical companies, medical device companies, or communication organizations whose products or services are related to the subject matter of the presentation topic. The intent of this policy is not to prevent a speaker with a potential conflict of interest from making a presentation, but intended that any potential conflict(s) should be identified openly, so that the listeners may form their own judgments about the presentation with the full disclosure of facts. Removal of this tissue results in less bulging over the nail with weight bearing and eliminates the problem. The nail is not touched! The result is cosmetically excellent and the problem will never recur.

Mike Allan, MD CCFPDr. Mike Allan has been in practice for approximately 16 years and presently works at the Northeast Community Health Centre in Edmonton. Jerome Alonso, MD CCFP DABFM (Cert. in Sleep Medicine)Dr. Jerome Alonso is a family physician and a sleep physician who currently practices both pediatric and adult sleep medicine. Jeremy Beach, MD FRCPDr. Jeremy Beach is a Professor in Occupational Medicine at the University of Alberta. He moved to the University of Alberta from the UK in 2002 to take up the position as Residency Program Director for the Occupational Medicine Residency Training Program. He has an interest in physician health, and mortality of the medical profession, and acts as an advisor on occupational health is to the Yorkshire Postgraduate Medical Training Program. Dr. Beach is on the advisory committee of the Physician and Family Support Program.

Vishal Bhella, MD CCFPDr. Bhella completed a PGY 3 Academic Fellowship in Family Medicine at Western following his residency. He worked in Ontario in various capacities including clinic, hospitalist and ER work during early practice. Dr. Bhella relocated to Alberta to join the University of Calgary Department of Family Medicine as a Clinical Assistant Professor and Lead Preceptor at the South Health Campus Family Medicine Teaching Clinic in 2013.

Alain Brunet, PhDDr. Alain Brunet is an Associate Professor in psychiatry at McGill University and the Director of the “Society and Mental Health” Research Program of the Douglas Mental Health University Institute. He has been named one of 39 Canadians that “made the world a better place” by McLeans magazine and was a finalist in QS Discovery of the Year.

Norman Campbell, MD FRCPCDr. Norman Campbell is a General Internist, Professor of Medicine, Community Health Sciences and Physiology and Pharmacology, and a member of the Institute of Public Health and Libin Cardiovascular Institute of Alberta at the University of Calgary. Dr. Campbell currently holds a number of appointments, including: HSF CIHR Chair in Hypertension Prevention and Control; Chair of the Canadian Hypertension Advisory Committee; and President of the World Hypertension League.

He was recently named to the Order of Canada for his achievements in bettering the health of all Canadians.

Bonnie Dobbs, PhDDr. Bonnie Dobbs is a Professor in the Department of Family Medicine at the University of Alberta, Director of Research for the Division of Care of the Elderly, and Director of the Medically At-Risk Driver Centre. Dr. Dobbs, in collaboration with Dr. Diane McNeil (Director, Promotion, Prevention and Community Initiatives for Provincial Addiction and Mental Health) and Anita Saini (MC), have spent the last three years conducting systematic reviews on screening tools for detection of the top five mental health disorders in community-dwelling seniors.

Michael Donoff, MD CCFPDr. Michael Donoff has been in practicing family medicine in Edmonton for 35 years and includes teaching both students and residents. He is also a Professor in the Department of Family Medicine at the University of Alberta, where his research focus has been on competency assessment. Dr. Donoff has also been the Director of Information Systems for the Department of Family Medicine at the University of Alberta, and he actively participates in the department’s mission to study primary care health system changes.

Elliott, MD FRCPC FSAMHDr. April Elliott is a Paediatrician and Adolescent Medicine Specialist at the Alberta Children’s Hospital and an Associate Clinical Professor in the Department of Paediatrics at the University of Calgary. Over the last 12 years, Dr. Elliott has developed Adolescent Health as a sub-specialty in Calgary. Dr. Elliott received the AMA’s Medal for Distinguished Service in 2011. Ward Flemons, MD FRCPCDr. Ward Flemons is a medical advisor to the Health Quality Council of Alberta, a respirologist at the Foothills Medical Centre, and a Professor of Medicine at the University of Calgary. He has led and participated in several large reviews of both Alberta’s and other provinces’ health systems. Dr. Flemons chairs two certificate courses on Patient Safety and Quality at the University of Calgary, and lectures widely within the province, and nationally, on these issues.

Louis Hugo Francescutti, MD PhD MPH FRCPC FACPM CCFP FRCP(Ire) FRCP(Edin) FRCP(Lon) ICD-D CCPEDr. Louis Hugo Francescutti received his combined Doctor of Philosophy (Immunology) in 1985 and his Doctor of Medicine in 1987 from the University of Alberta. While training as a general surgery resident, he became fascinated with the subject of trauma prevention. In 1994, he completed his further studies in injury control while working toward a Masters of Public Health at Johns Hopkins University in Baltimore, Maryland.

Over the past two and a half decades, Dr. Francescutti has spearheaded various public safety awareness initiatives and campaigns and was selected at one of Alberta’s Top 100 Physicians of the Century by the Alberta Medical Association/College of Physicians and Surgeons of Alberta, and was also awarded the Alberta Centennial Medal from the Government of Alberta in 2005. In 2010, he became president of the Royal College of Physicians and Surgeons of Canada.

Juan Antonio Garcia-Rodriguez, MD MSc(MedEdu) CCFP Dips Sports MedDr. Juan Antonio Garcia-Rodriguez is an Assistant Professor in the Department of Family Medicine at the University of Calgary. He practices family medicine and sports medicine in Calgary. Dr. Garcia obtained his medical degree in a six-year program in Colombia, completed his family medicine residency in Calgary, obtained a Masters degree in medical education, holds a diploma in sports medicine, and has two years of formal training in orthopaedic surgery.

Tobias Gelber, MD CCFP(An)Dr. Tobias Gelber works as a primary care physician in Pincher Creek, AB where he provides full scope primary care and GP anesthesia. Dr. Gelber is the President of the Section of Rural Medicine, is the past-chair for the Chinook PCN, past-chair of the AMA Primary Care Alliance Board, and was the Chair of the AMA Committee of Formal Patient Attachment which produced a position paper on the subject of Formal Patient Attachment. He was awarded the AMA’s Early Careerist Award in 2008.

Lee Green, MD MPHDr. Lee Green is a family physician, health services researcher, and Professor and Chair of the Department of Family Medicine at the University of Alberta. He holds the inaugural Alberta Innovates – Health Solutions Translational Health Chair, a major research endowment from AIHS to support primary care transformation. Dr. Green practices at the Kaye Edmonton Clinic and is a member of the Edmonton West PCN. He is a member of the Primary Health Care Steering Committee and chairs the Measurement and Evaluation Working Group.

Owen Heisler, MD MBA FRCSCDr. Owen Heisler is Assistant Registrar for the College of Physicians and Surgeons of Alberta with a primary responsibility as Complaints Director. Prior to this position, he had senior medical administrative positions in Edmonton, Victoria, and Red Deer which, in turn, were proceeded by a 20-year career as a general surgeon in Red Deer, AB. Dr. Heisler has an interest in knowledge translation, ethics, and evidence-informed care pathways.

Eric Jablonski, MD CCFPDr. Eric Jablonski is a family physician based in Calgary, AB. He completed his residency training at the University of Calgary in 2012 and, since that time, has been locuming around southern Alberta in various urban and rural primary care clinics as well as doing rural emergency work. Khurram Jahangir, BSc(Hons) MBChB CCFP(EM) MRCGP MFSEM DRCOG DFFPDr. Khurram Jahangir works as an Emergency Physician based in Edmonton and also has a strong background in Rural Family Medicine. He was recently appointed as the CPD Regional Educator for Alberta by the CFPC. He has a special interest in academic medicine and medical education, and is also Assistant Dean and Associate Clinical Professor in the Faculty of Medicine and Dentistry at the University of Alberta.

Monique Jericho, MD FRCPCDr. Monique Jericho is the Medical Director for the Calgary Eating Disorder Program and is a Clinical Lecturer at the University of Calgary. Dr. Jericho pursued her residency training in psychiatry in Toronto and Calgary, focusing her studies on the child and adolescent population and on the development of her skills as a psychotherapist. She began her medical practice in 2007 at the Alberta Children’s Hospital, working with populations that don’t “fit” well within the existing frameworks for health care.

She spent most of her career as a hospital pharmacist and educator in Red Deer, AB and has received several national awards through the Canadian Society of Hospital Pharmacists. Currently, Dr. Lindblad is the Knowledge Translation and Evidence Coordinator for the ACFP and an Associate Clinical Professor with the Department of Family Medicine at the University of Alberta.

Donna Manca, MD FCFP MCIScDr. Donna Manca has been a family doctor for over 25 years and practices at the Grey Nuns Family Medicine Centre in Edmonton, Alberta. She obtained her Master’s degree from the University of Western Ontario in 1989. In 1993, she joined the steering committee of the Alberta Family Practice Research Network (AFPRN) and became AFPRN’s Clinical Director in 2001. She joined the Department of Family Medicine at the University of Alberta in 2003 and is the Director of Research in the Department. She is one of the clinical directors involved in the Canadian Primary Care Sentinel Surveillance Network (CPCSSN) and the chair of the CPCSSN Privacy and Ethics Standing Committee. Her research expertise includes mixed-methods approaches including qualitative research methods.

Kerry McBrien, MD MPH CCFPDr. Kerry McBrien is an Assistant Professor in the Departments of Family Medicine and Community Health Sciences at the University of Calgary. Her research interests include health economics and health services research, with a focus on chronic disease management in primary care. Dr. McBrien’s unique research background and her experience as a primary care physician have been critical to development of the chronic kidney disease clinical pathway.

Diane McNeil, PhDDr. Diane McNeil is the Director of Promotion, Prevention and Community Initiatives, Provincial Addition and Mental Health. Dr. McNeil received her PhD in Psychotherapy from the Department of Psychiatry at the University of Alberta. Mehrnoush Mirhosseini, MD CCFPDr. Mehrnoush Mirhosseini is an Associate Clinical Professor in the Division of Palliative Care Medicine, Department of Oncology at the University of Alberta. She is a palliative care consultant and a member of Community Consult Team in Edmonton Zone Palliative Care Program. She provides consultation services in diverse settings, including homes, facilities, hospitals, and hospices. Her main area of interest is prognostication and she has collaborated locally and nationally with other researchers to streamline the issue of survival prediction for palliative care population.

Kathryn Schneider, BHScPT BKin(Hons) DSc DipManipPT PhDDr. Kathryn Schneider is an Assistant Professor and Clinician Scientist (Physiotherapist), University of Calgary. She is a Clinical Specialist in Musculoskeletal Physiotherapy and a Fellow of the Canadian Academy of Manipulative Physiotherapy. Dr. Schneider has completed certification programs in Vestibular Rehabilitation (Emory University, 2003) and Advances in Vestibular Rehabilitation (Duke University, 2009), and a Doctor of Science degree (DSc; Andrew’s University, 2008).

Britt Simmons, MD CCFPDr. Britt Simmons graduated from the University of Alberta in 1981. Lori Teeple, MD CCFP(EM) FCFPDr. Lori Teeple is a family physician with specific expertise in wound care and palliative care. She is also highly involved in “lifestyle as preventive medicine”. She was the former lead physician for complex continuing care and rehabilitation at St. Thomas Elgin General Hospital and is currently a regional palliative care consultant physician providing. In both these settings expertise in wound care is a high priority. Dr. Teeple is an award winning teacher at Western University in London ON where she is an associate professor in medicine and family medicine. Primary Care Networks Program Management Office

The Primary Care Networks Program Management Office (PCN PMO) provides operational and business administrative support services to Primary Care Networks (PCNs), as well as tools and resources to support PCNs in communications, evaluation and quality, and business planning. In

addition, the Primary Care Network Evolution initiative is managed through the PCN PMO. We provide opportunities to collaborate with all other support programs such as PMP, TOP and AIM in developing and delivering tools, resources, training and support for this initiative.

Alberta Referral Pathways | eReferral, AHS (Bronze) 7th Level Stairs

Alberta Referral Pathways is a provincial collaborative program bringing together care providers involved in a patient’s journey through the health system to determine what is required on a referral, what services

are available and what providers can do to improve access to care. The program facilitates the provincial discussions and framework necessary to develop a simple, clear and timely referral experience for patients and providers. The developed pathways are reviewed to reflect up-to-date best practices and then adopted across the province.

eReferral is Alberta’s first paperless referral solution and allows Alberta Netcare users the ability to securely submit, track and manage referrals in real time. The limited production rollout launched in July 2014 for referrals to hip and knee arthroplasty, breast cancer and lung cancer.

Together, Alberta Referral Pathways and eReferral are working to improve patient access to care and to transform Alberta’s referral experience.
