ICH CAHPS Attestation ReminderFacilities with fewer than 30 eligible in-center hemodialysis patients during calendar year 2018:• Were exempt from administering the ICH CAHPS survey
during calendar year 2019.• Should attest to that exemption in the ESRD Quality Reporting
System (EQRS) by 11:59 PM PT on March 2, 2020.

EQRS System Maintenance and HARP Go Live
EQRS will be unavailable due to system maintenance from 8:00 P.M. EST on Friday, February 21, 2020 until
8:00 A.M EST on Monday, February 24, 2020.
• Effective February 24, 2020, HARP credentials will be required to access the EQRS application. Access the EQRS application at https://eqrs.cms.gov/globalapp.
• HARP is a new account registration application that is replacing the Enterprise Identity Data Management (EIDM) system.

• All EQRS users must complete a HARP account setup.– CMS automatically migrated existing EIDM accounts and user
roles to HARP. To complete the migration, users must:• Log in to HARP via https://harp.qualitynet.org/login/.• Register a two-factor authentication device.• Select a Challenge Question.
– Non-EIDM users can register for a new HARP account. • HARP resources including training materials, Quick Start
Guides, Frequently Asked Questions, and instructional videos can be found on MyCROWNWeb.org at https://mycrownweb.org/harp-training/.

The Annual ESRD Facility Survey is:
• A CMS Office of Management and Budget reporting requirement for all ESRD facilities and Transplant Centers to provide information to CMS
• Designed to collect ESRD dialysis facility (2744-A) and Transplant Center (2744-B) information regarding:– Patient Census– Treatment Records– Staffing Records
• The information is used to assess and evaluate the local, regional and national levels of medical and social impact of ESRD care and is used extensively by researchers and suppliers of services for trend analysis.*

Getting Started
• Facilities initiate the CMS-2744 completion process in CROWNWeb, by generating a form and submitting it for acceptance by the Network.
• The CMS-2744 form goes through various stages before it is complete. Facilities must monitor their forms’ status.
• The form is complete once it is in a “FINALIZED” status.

CMS-2744 Flow• ESRD Networks have local submission deadlines to help ensure
that CMS-2744 forms are “FINALIZED.”• The facility generates and submits the CMS-2744 form to the
Network for review. The Network rejects the CMS-2744 if thereare errors or accepts the form if there are none, and submits it toCMS.
