Dear Caregivers & Friends:

I’m happy to present the latest edition of the Family Caregiver Resource Guide. According to the Family Caregiver Alliance of the National Center on Caregiving, 43.5 million of adult family caregivers care for someone 50+ years of age. There are an additional 14.9 million caregivers who care for someone who has Alzheimer’s disease or dementia. A caregiver is an unpaid individual involved in assisting others

with activities of daily living and/or medical tasks. At one point in our lives, it is highly likely that we will hold the title of Caregiver. We will provide this assistance to our spouse, partner, family member, friend, or neighbor. Most caregivers do not get paid; formal caregivers are paid to provide care to an individual in their home or in a residential facility, adult day care facility, etc.

More than one in six Americans working full or part time report assisting with the care of an elderly or disabled family member, relative, or friend. Caregivers working at least 15 hours per week said it significantly affected their work life (Gallup Healthways Wellbeing Survey, November 2012). This statistic illustrates the reason why this resource guide is helpful. In order to assist the unpaid Caregiver, the 2014 Family Caregiver Resource Guide offers Caregivers a wealth of information to assist them in their role. Caregivers can find out how they can register their loved one with the Department of Senior Affairs Silver Alert Program, participate in the Savvy Caregiver Program offered by the Alzheimer’s Association, NM Chapter, and have a variety of important phone numbers at their fingertips.

I am thankful for all of you who have accepted the role of a Caregiver. The service that you provide makes a difference in someone’s life. As a Caregiver, you can get so busy that you neglect some of your needs, so I would encourage you to keep yourself healthy so that you can continue to provide the best care possible for your loved one. I would also encourage you to familiarize yourself with the services offered by the City of Albuquerque’s Department of Senior Affairs. The department offers a variety of services to people of all ages in our community, including Caregivers. Visit one of our senior centers or a multigenerational center and participate in an activity or class. Someone once said, “About the only thing that comes without effort is old age.” There are some people who make the aging process look effortless. There are others who need a lot of assistance as they age. Some of these individuals are fortunate to get some help. In most cases, this assistance comes in the form of another person or people. We call those who step up to help others through the aging process, Caregivers. Many seniors in our community rely on Caregivers for

so many things. Caregivers help seniors get to their medical appointments, ensure that seniors are taking their medication properly, make certain that they eat and bathe, and provide them with socialization. The role of a Caregiver is not a small task and many Caregivers do not receive a paycheck, they do it freely and with good heart. To those Caregivers reading this right now, I would like to THANK YOU for everything you do while caring for another individual. Your work requires a tremendous amount of effort.

The City of Albuquerque’s Department of Senior Affairs is pleased to continue our partnership with Prime Time Publishing Company to ensure that this publication reaches as many Caregivers in our community as possible. This booklet is a resource guide for Caregivers. Please take a moment to look at the guide and make use of the variety of services and resources that are available in our community. The challenging roles that family caregivers play in the lives of so many people are immeasurable. Their struggles, their challenges and obstacles are plentiful, yet they go about their jobs with such love and devotion that many of us do not realize what they go through. Our Family Caregiver Resource Guide is helping to provide answers and direct people to qualified caregivers throughout the greater Albuquerque area.Providing care to our most precious elderly and frail loved ones is one of the most important jobs in our society. Caregivers and those seeking care often feel as if they are out on their own, struggling with questions or lacking resources to help them.

Our 8th annual Family Caregiver Guide is made possible by our partners – the City of Albuquerque Department of Senior Affairs (DSA) and our many advertisers. We offer the Family Caregiver Resource Guide at the beginning of November to coincide with National Caregivers Month. It is distributed widely throughout Albuquerque and Rio Rancho. This year, Santa Fe’s Family Caregiver Resource Guide will be published and distributed in January.You can get a copy of the guide at the Department of Senior Affairs, the New Mexico Aging and Long Term Services Department, at our advertisers’ locations and at all major grocery stores. The Alzheimer’s Association estimates that there are currently 31,000 New Mexicans with Alzheimer’s disease, and around 80% of them are cared for in their home by friends or family. Individuals who care for a loved one with Alzheimer’s or dementia typically report higher stress levels than those who care for older adults with other conditions, and they provide care for a longer period of time. Almost 60% of Alzheimer’s caregivers report that their stress is “high” or “very high.” There is help though, and it is offered by the Alzheimer’s Association, New Mexico Chapter.

Savvy Caregiver is a free, seven-week, evidence-based course that teaches practical strategies for caregiving while increasing caregiver skill and decreasing caregiver stress. The program is a unique approach to family caregiver education. It has been developed by clinicians and educators who understand that caregiving is in many ways a form of clinical work, and have tailored the training accordingly. The central concept is the notion of

strategy; throughout the program caregivers are urged to learn, develop and modify strategies that will be used to accomplish the goal for their particular caregiving situation. Another main tenet of the program is for the caregiver to integrate a contented involvement with the person they are caring for into their daily life. The program presents ideas from many disciplines and sources. Throughout the program caregivers are urged to learn, develop and modify strategies that will be used to accomplish the goal for their particular caregiving situation.

Previous participants report that the class has helped them see their caregiving responsibilities in a new light, and that they continue to use the strategies they learned as they travel through their caregiving journey.

The goal of the City of Albuquerque, Department of Senior Affairs’ new Manage Your Chronic Disease (MyCD) Program is to improve the physical and emotional health of adults with chronic diseases, and their caregivers, by helping them gain self-confidence in their ability to manage their symptoms and how their health problems affect their lives.

The MyCD Program follows the Chronic Disease Self-Management Program developed by Stanford University’s School of Medicine Division of Family and Community Medicine. The MyCD Program is a guided workshop delivered two and one-half hours, once a week, for six weeks, in community settings such as senior centers, churches, health care facilities, etc. People with different chronic health problems (regardless of age), and their caregivers, come together to attend these workshops that are facilitated by two trained leaders, one or both of whom are non-health professionals with chronic diseases themselves.

Subjects covered include: • techniques for dealing with

problems such as frustration, fatigue, pain and isolation;

• appropriate exercise for maintaining and improving strength, flexibility, and endurance;

• appropriate use of medications;

• communicating effectively with family, friends, and health professionals;

• nutrition; and how to evaluate new treatments.

Classes are highly participative, where mutual support and success build the participants’ confidence in their ability to manage their health and maintain active and fulfilling lives. Participants make weekly action plans, share experiences, and help each other solve problems they encounter in creating and carrying out their individualized self-management program. The COA MyCD Program does not conflict with an individuals’ existing health improvement program or treatment plan, but rather is designed to enhance regular treatment and disease-specific education.

Expected outcomes of the COA MyCD Program include improvement in program participant self-reported levels of: health, energy, stretching and strengthening exercise, and endurance exercise while, concurrently, documenting participant decreases in health distress and hospitalizations.

If you are reading this resource guide looking for information on how to better care for a loved one, you have probably crossed one of the largest hurdles in taking care of yourself as you take care of a loved one – you have identified yourself as a caregiver. As with other situations or problems, you must first acknowledge the condition exists before you can deal with it.

Recognize that caregiving is a job. And it is often NOT a one person job. Even if the caregiving is currently a 3-hour per week running-to-buy-groceries-for-Mom job, it can very quickly and unexpectedly escalate into a 168-hour per week – in other words, 24/7 obligation.

That’s more than four 40-hour work weeks – a four person job. A good place to begin is to create a “job description” for your caregiving by defining:

What is your goal in providing care?

What tasks need to be performed to achieve this goal?

How much of this can you realistically do?

What do you need help with? What would you like help with?What do you need to learn to

achieve the goal? Think about who you know, or what

agency described in this guide, can help you achieve the goal.

Who likes to do some of the household chores you hate to do?

If you need to learn to change Mom’s surgical dressings, where could you get training in this?

Give yourself permission to accept help. Use the resources listed in this resource guide, whether they are local or national. Give yourself permission to utilize respite care – think of it as vacation time from your caregiving job. You take vacations from your paying job after all.

Caregiving can be socially isolating and extremely stressful. Be aware you are not alone. In the U.S. alone, approximately 50 million people – nearly one-quarter of the population – are family caregivers. There are support groups both locally and online that can help you get through the rough spots. (As with all online support groups/chat rooms, take care in using them and don’t give anyone any personal information such as your Social Security number, street address, etc.)

Take care of yourself first. If you’ve ever flown on a commercial jet, you’ve heard the flight attendants give the presentation about what to do if the cabin pressure drops: you are instructed to put your own oxygen mask on before trying to help anyone else with their masks. The same is true with caregiving – you have to take care of yourself in order to take care of anyone else. But caregivers can easily get so wrapped up in caring for their loved one, they put off their own healthcare needs. Because of the isolation and stress, you need to be watchful for signs of depression and physical ill-health. Caregivers are especially prone to back injuries and stress-related health problems. The best gift you can give your care recipient is your own good health.

Communicating With DoctorsCaregivers often find

themselves in an awkward position with doctors and other health care providers. You might feel like you are not getting the information you need or they aren’t aware of your role in the elder’s life. Here are a few tips that may help improve communications:Identify Yourself as a Caregiver

Don’t just say, “I’m her daughter.” Be clear about your role so the health care provider knows why you are there. Say, “I’m her daughter and I’m the one who takes care of her at home.”

Get written permission for private information to be shared with you.

Many healthcare professionals will not be comfortable letting you stay in the room during an exam or talking to you about your elder’s health because they want to protect your elder’s privacy. It’s best to deal with this issue by having the elder sign HIPAA form(s) giving you the right to this information. The form stays in the elder’s chart so any of the doctor’s staff can see they may share information with you without getting in trouble.

If you have one, provide a copy of the Power of Attorney

for Health Care to the doctor. Ask for a longer visit when you call to make appointments.

Most clinic and doctor’s office visits are very short. Telling them you will need more time to discuss your concerns is very important. Or ask for a consultation appointment.

Before the appointment write down your concerns and questions and any information you want the doctor to have. That way you’ll be able to stay organized and make the best use of your limited time.

Always take a complete current list of the medications the elder is taking with you to any health care appointment.Useful Hints• Write down questions to ask

the doctor in advance so you don’t forget.

• Ask about prescription drugs to be clear what they are for and how to take them.

• If needed, be sure the physician and hospital have a copy of Health Care Power of Attorney or DNR paperwork.

• What else can one do to make sure the medication works the way it’s supposed to?

• Take your medication list to the pharmacy and ask for answers to any questions that weren’t answered by the doctor.

• Is this in any way related to other problems the elder has?

• What treatments are available?

• Ask about alternative

treatments like massage or acupuncture.

• Is this a problem that gets better or does it stay with you? If it may improve, how long might that take?

Other Questions to Ask a Doctor or Clinic Staff• Does the primary care

physician have a case manager assigned to the practice group who might be able to assist the patient and his/her family caregiver system?

• When a patient is discharged from the hospital is there a discharge planner available to assist with follow up plans at home? (Medicare requires formal discharge planning.)

• If indicated, a request can be made to the primary care physician for a home health and home safety evaluation consultation. This consult would be aimed at assessing:

• Home safety and equipment needs

• Education/training needs • Skilled care needsAn assessment can introduce

a home health social worker into the picture to help broker and make linkages for the family caregiver to other resources.

MedicationsFamily caregivers should be encouraged to ask for assistance with the costs of prescription medications when indicated. Assistance can include: • The medical practitioner

prescribing the most affordable alternatives (FYI: a generic drug must have the same active ingredient(s) as its brand name equivalent)

• Referral to the NM ALTS MEDBANK for assistance in obtaining free or low-cost medications and counseling for Medicare programs such as the discount drug card and Part D prescription drug coverage

• Family caregivers might also inquire about the availability of caregiver support group services offered through many of the health care systems.

Hints for Managing Medications

Managing your care recipient’s medications can be a challenge. Here are some hints from a University of New Mexico senior care case manager that might help you:

First: Talk with the pharmacist or physician about all of the medications the care recipient is taking:

• Is there any duplication?• Are all of the medications

necessary?• Are there indications that some

of the medications should not be taken together?

Second: Talk with the physician and ask him/her to rewrite the prescriptions so some or all of them can be refilled at the same time thus saving you many different trips to the pharmacy each month.

Third: If you live separately from the person you care for and are concerned with the care recipient not taking or not remembering to take their medications, try the following:

• Purchase a pill box with large compartments and lettering and arrange medications a week at a time.

• Call the care recipient when the medications are to be taken and talk with her/him until they are done taking their medications. If this is successful, you may not need to make the daily trips to ensure medications are being taken.

More than 38,000 individuals in New Mexico suffer from Alzheimer’s disease or other dementia. Statistics show that about 60 % of these individuals will wander at some time or another. The Albuquerque Police Department and the Department of Senior Affairs are working together through the Silver Alert program to ensure missing persons with Alzheimer’s disease or other dementia are quickly returned to their family or caregiver. Silver Alert is a public notification system to broadcast information about missing persons who suffer from Alzheimer’s disease of other dementias. The system notifies the public of a missing senior using television, radio, electronic billboards, reverse 911 and traffic signs. The public is asked to call a phone number if they see the missing person. Individuals who are 60 or older and suffer from Alzheimer’s disease or other dementia are eligible to register for Silver Alert. The Department of Senior Affairs will place vital information and a photo of the individual on a USB drive and give the drive to the family member/caregiver who accompanied the person to the Silver Alert registration.

When the family member/caregiver determines that their loved one is missing, they are asked to call 242-COPS. An officer from the Albuquerque Police Department will come to the residence, or the place from which the person is missing, and initiate an investigation. If the person is not found immediately, the Silver Alert will be issued, notifying all media sources of the importance of finding the individual as quickly as possible. The alert will contain a photograph of the person and state their name, description, age, relevant medical conditions, vehicle type if applicable and their last know whereabouts. If you had previously registered your loved one with Silver Alert, this important information will be on the USB drive in your possession that you already completed with the assistance of the Department of Senior Affairs. At that time, you will turn over the USB drive that contains this vital information to the Albuquerque Police Office working with you. Registration for Silver Alert is on-going. Those familiar with resources, programs or organizations use short cuts or acronyms to talk about resources. Here is a glossary for some of the terms.

AAA: Area Agency on Aging (The Triple A) Six Area Agencies on Aging serve the State of New Mexico; the Albuquerque/Bernalillo County area has one.

ADLs: Activities of Daily Living. Routine personal activities such as bathing, dressing, toileting, eating and walking (ambulation).

Advance Directives: Legal forms such as a “Durable Power of Attorney for Health Care and Instructions for Health Care and End-of-Life Decisions”. Also referred to as a “living will.”

APS: Adult Protective Services

Assisted Living: Assisted living is an intermediate level – between independent living and nursing homes – of long-term care appropriate for many seniors.

Benefits Counseling: A program of the NM Aging and Long-Term Services Department which provides information that might reduce your Medicare payments or help you with the costs of food, utilities and medications.

Caregiver: anyone providing physical, financial or emotional support for a relative or loved one whose daily activities are limited by diminished mental or physical function.

Centennial Care is the new name of the New Mexico Medicaid program. Centennial Care will begin January 1, 2014 and services will be provided by

four managed care organizations (MCOs). These services include physical health, behavioral health, long-term care and community benefits.

Most people who are currently enrolled in a New Mexico Medicaid program are eligible for Centennial Care. Eligibility for all Medicaid programs requires that individuals meet certain federal guidelines. These include citizenship, residency and income requirements.

To apply for Centennial Care coverage, you can complete an application on-line beginning October 1, 2013 at www.yes.state.nm.us. You may also apply in person at your local ISD office, by fax, or by mail. To download an application, go to ww.hsd.state.nm.us/isd/apply.html. If you have application questions, you can call HSD’s new Medicaid Expansion Hotline toll-free at 1- 855-637- 6574 or for general questions, please call 1-888-997-2583.

Continuity of Care: Range of resources and services which help caregivers in their efforts to maintain the care recipient’s quality of life.

Cremation Authorization form: An advance directive which allows you to authorize your body be cremated and to designate how you want your ashes disposed of.

Discharge Planning: Medicare defines discharge planning as: “A process used to decide what a patient needs for a smooth move from one level of care to another.”

Under this 1996 Federal law, health care providers cannot release your medical information without your express consent.

HUD: U.S. Department of Housing and Urban Development.

IADL: Instrumental Activities of Daily Living.

Institutional Medicaid: A Medicaid program that pays for nursing home care for individuals who are in a qualifying medical institution for at least 30 days when nursing home care is determined to be medically necessary.

ISD: Income Support Division of the New Mexico Human Services Department

Level of Care (LOC): “Level of care” can mean a number of things depending on the context.

LIHEAP: Low-Income Home Energy Assistance Program

Long-Term Care (LTC): a variety of services including medical and non-medical care for those who have a chronic illness or disability.

Medicaid is a state and federal government health insurance program that is administered by the state where eligibility is determined by the states established criteria. It is for people with low income and limited assets. Medicaid covers medical and personal care options, along with benzodiazepines, behavioral health drugs, not covered by Medicare. With prior approval/authorization, it may also cover eyeglasses, hearing aids and dental work.

VA benefits – Medications may be provided at low or no cost depending upon your VA status.

PACE: A Program of All-inclusive Care for the Elderly (PACE) Generic term for day health centers that include a medical clinic, adult day care, social services, rehabilitation, transportation, in-home services and inpatient care (when needed). In Bernalillo County, this service is available through Total Community Care, LLC. To qualify for PACE, the senior must be aged 55+ and live within the county or within 30 minutes travel from center. Free for recipients of both Medicare and Medicaid. Private pay also accepted.

POA: Power of Attorney – a legal form designating someone else to act on your behalf if you become unable to chose medical care for yourself.

At New Mexico Cancer Center, we aggressively manage cancer and the side effects of treatment and the disease to help our patients feel better so they can spend more time at home. This method of treatment is called a "medical home." As a result of what has been created here, Dr. Barbara McAneny received a grant to duplicate this process at six clinics across the country.

As part of the grant, we offer treatment until 8 p.m. during the week and on the weekends so our patients don't seek care in the ER. By extending our hours, we keep patients out of the hospital and at home where they're happy and comfortable.
