CubHub Family Portal Overview


By logging in to CubHub you will have full access to your loved ones records including:

• All Patient Details• Complete Medical Chart• Calendar

Your provider will give you your user name and temporary password

*Please note the user name must always include the .company code (in this example the user name is lharris.mo because services are being provided by company Mount Olympus)

After you’ve set your permanent password if you forget it always use the Forgot Password option to avoid being locked out

Logging in to CubHub


Upon first log in you will be prompted to create your permanent password & set security questions.

CubHub is HIPPA compliant and passwords must meet the following requirements: minimum 8 and maximum 20 characters in length; one uppercase letter; a number & a symbol

After you’ve set your permanent password if you forget it use ‘Forgot Password’ Three failed log in attempts will lock your account and your provider must be contacted to unlock it

Password & Security Question Updates


At any point after you’ve set your permanent password & set your security questions you may click on the CubHub Bear in the top left of the site and click preferences

Here you can update you password and/or update your security questions

Accessing Patient Info


Once you are logged in you will be see your family members name If you have opted-in with your provider you may also see their pictureClick on the circle with their name to view all information

Sections of Patient Information


Profile: Includes all demographic information including physicians and services being providedContacts: Patient contacts including physicians & family membersChart: The complete medical recordCalendar: Current schedule

*Please note all information is view-only and cannot be edited; we recommend reviewing all info to ensure there are no discrepancies; if any are found reach out to your provider & they will update

Calendar


• Blue: Locked for payroll and billing• Dark Grey: Caregiver has submitted their visit• Green: Scheduled with a caregiver• Light Gray: Caregiver has clocked in to the visit• Red: No caregiver assigned

• You may toggle from month to month or look at a specific week or day

• If you have any questions always contact your provider

Patient Chart: Plan of Care


The plan of care section displays:

• 60 Day Summary recorded on last assessment• Patient Allergies• All Archived plan of cares from most recent (current)

To download an archived plan of care simply click on the date

Patient Chart: Assessments


• Displays all assessments • Any assessment not in a status of ‘working’ may be viewed• Click on the Visit Name to download the PDF of the assessment

Patient Chart: Visits


• Displays the daily visits caregivers complete in the CubHub Mobile App• Scheduled status: have not been started• Working status: have been clock in to but have not been submitted• Pending QA status: have been submitted and are waiting for review• Pending Corrections: Have been sent back to the clinician to make a correction• Completed: Have passed QA • You may view the PDF of a visit by clicking the blue visit name; in this example: PDN Shift (PDF)

Patient Chart: Orders & Other Documents


Orders• Displays all orders including 485 (plan of care)• May view by clicking on PDF icon to download original or on 0 to view any attachments

Other Documents• Includes any external attachments (i.e. admit paperwork; hospital discharge orders)• Includes system generated documents (i.e. MAR; Med Reconciliation)

Patient Chart: Live Patient Record


The Live Patient Record is just that – a live record of clinical information You can reference the LPR at any time to review specific information that was recorded by your caregivers in the mobile appThroughout you will see the ‘H’ which shows history for that specific clinical item including Medications

Signing The Visit in the Mobile App


When your caregiver has finished their shift they will request your signature before they can submit

The first page (pictured left) is your acknowledgement of your provider policiesYou may click on the link to review itThen check the box that you accept and click Parent PCG Signature

The screen (pictured right) will load and you will confirm their visit times:

Clock In: when the caregiver started care Clock Out: when the caregiver reported off to you or the next caregiver

Sign the screen and click Submit
