Open enrollment for benefits will begin on 10/23/17 and will run through midnight on

11/10/17. The open enrollment process is completed through the MyJob portal. With

the exception of the Flexible Spending and MetLaw elections, all other current elections

you have will roll over into 2018. If you would like to confirm or change anything about

your benefit coverage in 2018, you will need to go to MyJob and click on the MU Benefit

Annual Enrollment portal. Marquette strongly encourages employees to review their

elections as there are changes in pharmacy coverage, health insurance premiums, and

out of pocket (OOP) maximums for 2018.

Log in to https://myjob.mu.edu and select MU Benefits Annual Enrollment.

If you do not have access to a computer, there is one available in the Human Resources

lobby, Straz Tower Room 185, during regular business hours (8 a.m.—4:30 p.m.) for

making your 2018 benefit elections through the online portal.

AGGREGATE v. EMBEDDED OUT OF POCKET MAXIMUM (OOP)

ALEX IS BACK

We are once again offering "ALEX", an interactive online benefits decision support tool.

In addition to the support the Benefits Department provides, ALEX can further assist

employees by serving as an educational and decision support resource during the 2018

open enrollment period. ALEX provides a summary of your benefits and can help you

decide which medical plan is best for you. However, you should fully review all of your

2018 benefits documents before enrolling. ALEX will be available during annual

enrollment. Check news briefs and the benefits website for the updated link.

CPHP & EHDHP (EPO & HDHP)

Embedded: An embedded out of pocket (OOP)

maximum means that a single member of a family

doesn’t have to meet the full family OOP. Under

employee + 1 or family coverage an individual family

member would be responsible for up to the single

OOP maximum only.

AHDHP (PPO)

Aggregate: With an aggregate family out of pocket

(OOP) maximum there is essentially no separate

single OOP “embedded” within the family plan.

Under employee + 1 or family coverage the family

deductible/OOP maximum applies even if only one

family member incurs claims.

2018 IMPORTANT BENEFIT CHANGES

Medical plan names are changing:

The EPO Plan is now the CPHP (Co-Pay Health Plan)

The PPO Plan is now the AHDHP (Aggregate High Deductible Health Plan)

The HDHP is now the EHDHP (Embedded High Deductible Health Plan)

Pharmacy benefits will now be administered by Navitus

The biggest change for 2018 is the transition of our pharmacy benefits management (PBM) from Express Scripts to

Navitus Health Solutions (“Navitus”). Customer service was a major factor in this decision, and we feel Navitus will bring

a local and much higher standard in this regard. While we will maintain a broad network and current benefit levels, there

will be changes as a result of the new vendor, i.e., drug tier placement, exclusions, mail order service, etc. Although we

expect this disruption to be minimal, we will do our best to communicate any changes to impacted members in a

proactive manner and to provide those members with new/alternative options. Because of this change to the Pharmacy

Insurance, new medical/RX ID cards will be provided to all 2018 medical plan participants. If you do not receive them by

the end of January or need additional ID cards please contact UMR at 800-826-9781.

Teladoc: A convenient Telemedicine Application will be available as of January 1, 2018

Teladoc provides you and your covered family members with access to U.S. board-certified providers 24/7/365 by

phone, online video or mobile app. Teladoc’s network of providers can diagnose routine, non-emergency medical

conditions, recommend treatment and prescribe short-term, non-DEAcontrolled medications when needed. For more

information regarding costs or coverage, register your account on 1/1/18 by contacting Teladoc online at Teladoc.com

or call 1-800–Teladoc (835-2362).

HEALTH RISK ASSESSMENT (HRA)

Health Risk Assessment (HRA) time is here! Health Risk Assessment (HRA) participants enrolling in the CPHP plan will

receive a Flexible Spending Account (FSA) contribution of $125 for Single coverage or $250 for non-Single (Employee+1

or Family) coverage. HRA participants enrolling in the AHDHP or EHDHP plan will receive a Health Savings Account (HSA)

contribution of $250 for Single coverage or $500 for non-Single coverage. Covered spouses must also complete the HRA

process (in addition to the employee) in order for the employee to receive the incentive. These incentives will be made

available at the start of the plan year.

Marquette University launched a NEW interactive, personalized wellness portal on October 1, 2017. Visit

marquettewellness.preventure.com to complete the health assessment and engage in wellness resources.

STEP ONE: Complete the Biometric Screening. The Biometric Screening is a simple finger stick blood test that will

analyze your cholesterol, HDL and glucose. Blood pressure, pulse, height, weight, and body mass index will also be

recorded. The results are available in about 15 minutes.

Option ONE: Biometric screening with your or your spouse's primary care provider utilizing the Medical Provider

Form below. Biometric screenings for employees and spouse seeing their primary care provider must fall between

July 1, 2017 and November 18, 2017. Subject to co-pays or deductibles. You can fax the form to Aurora Healthcare at

(414) 525-2570. This form must be completed by your medical provider but can be faxed directly to Aurora by them

or by you. Please keep a copy of the biometric results for your records as this information is required to complete the

online Health Assessment.

HRA Provider Form for 2018 Incentive

Option TWO: Complete your biometric screening for free at a Marquette sponsored Aurora offering.

Online scheduling is available for free biometric screenings. Click on the link or call 1-877-765-3213, Option 1.

Please keep a copy of the biometric results for your records as this information is required to complete the online

Health Assessment.

Option THREE: Complete a comparable biometric screening at a spouse's employer-sponsored HRA screening and

fax directly to Aurora Healthcare at (414) 525-2570. Biometric screenings must fall between July 1, 2017 and

November 18, 2017. In order to connect all documents, please make sure Marquette University as well as the

Marquette employee’s name is prominent on any alternative form you submit. Please keep a copy of the biometric

results for your records as this information is required to complete the online Health Assessment

STEP TWO: Complete an online health assessment with the results from your biometric screening. The new online

health assessment must be completed after the biometric screening is performed; upon completion of the assessment,

you are strongly encouraged to print your personalized report that helps to identify potential health concerns and

lifestyle behaviors that may need to be changed; your specific results are 100% confidential. The health assessment is

accessible on our NEW wellness portal marquettewellness.preventure.com, which launched on October 1, 2017. Please

see the Preventure link below for instructions on how to log in.

Aurora: To check if Aurora received your biometric screening results, you can call Aurora at 877-765-3213, Option #1.

Preventure: If you need assistance, click the “Need help logging in?” link, or contact Preventure Customer Support at

888-321-4326 or [email protected]. For future reference, be sure to bookmark or “favorite” the site. Each

time you visit, just log in with your username and password, and click “Sign in”.

HEALTH SAVINGS ACCOUNT (HSA)

Flexible Spending Accounts (FSA) make it easy for you to set aside a portion of your salary on a pretax basis and use it to

pay eligible health and/or day care expenses.

A Limited Purpose Health Care FSA option has been added for AHDHP and EHDHP plan participants who wish to set

aside funds to reimburse eligible dental and vision only expenses (and prefer not to use their HSA funds for these

expenses). The Heath Care FSA, which reimburses eligible medical, pharmacy, dental and vision expenses is still available

to CPHP plan participants and employees not enrolling in Marquette’s health plan. The Dependent Care FSA remains

available to all benefit eligible employees. All three FSA components are “use it or lose it” programs.

The three account options are:

Health Care FSA — Can be used for eligible out-of-pocket medical, prescription drug, vision and dental expenses

Maximum annual limit: $2,600. You must be enrolled in the CPHP plan or another eligible medical plan not offered by

Marquette. If enrolled in the AHDHP OR EHDHP, you are not eligible for the Health Care FSA.

Dependent Child Care FSA — Can be used for certified day care expenses incurred for the care of your child(ren).

Maximum annual limit: $5,000. No matter what medical plan you are or are not enrolled in, you can elect this benefit.

Limited Purpose Health Care FSA — Can be used for eligible out-of-pocket dental and vision expenses only.

Maximum annual limit: $2,600. You must be enrolled in the AHDHP or EHDHP plan. If enrolled in the CPHP plan, you are

not eligible for the Limited Purpose Health Care FSA.

A Health Savings Account (HSA) is a tax advantaged personal savings account that can be used to pay for medical,

pharmacy, dental, vision and other qualified expenses now or later in life. Unlike FSA funds which are “use-it-or-lose-it”,

your HSA balance rolls over from year to year, with the potential of earning interest along the way. The account is

portable, meaning if you ever change health plans or leave Marquette, you can still use and/or take the HSA with you

because it’s your money and your individual account.

There are two ways Marquette will contribute funds to your HSA. First, EHDHP participants will get an automatic

contribution of $250 for single coverage and $500 for non-single coverage. Second, if you enroll in the AHDHP or

EHDHP and complete (along with your covered spouse, if applicable) the Health Risk Assessment (HRA) process,

Marquette will provide a $250 contribution for single coverage or a $500 contribution for non-single coverage. Any and

all Marquette contributions will be made on or around the first of the year (for current employees), as long as the

participant meets all of the requirements.

In addition, employees can contribute their own funds to an HSA. Beginning in January you can elect pre-tax payroll

deductions. You can also make after-tax contributions from your personal checking account, which can then be deduct-

ed on your personal income tax return. You will need to initiate this after-tax process on your own—Marquette will not

facilitate.

2018 annual contributions from all sources (you, Marquette and any other individual) may not exceed $3,450 if you have

single coverage or $6,900 if you have non-single coverage. Individuals age 55 and over may make an additional $1,000

catch-up contribution. Married individuals may each make a catch-up contribution, although the maximum amount that

can be contributed to one HSA for 2018 is $7,900 (the $6,900 statutory maximum plus a $1,000 catch-up contribution).

There is a 6% excise tax on any individual contribution that exceeds the allowed annual maximum.

How do you get started with the NEW wellness portal?

Step 1: Set up your new login. Beginning October 1, 2017 you can access the wellness portal at

marquettewellness.preventure.com. You will need to follow the log in instructions and register as a first time user. Please

follow the registration formula as outlined within the log in instructions guide and on the log in page.

Step 2: After logging in, take your Health Assessment. Complete your assessment by November 22, 2017, and be one

step closer to earning your wellness reward.

Step 3: Check your reward status in the EARN page of the wellness portal. Here you can check your progress and

ensure you are on target to receiving the wellness reward.

The wellness reward is given to all Marquette Employees and Spouses who are enrolled in the Medical Plan.

Medical covered spouses must also complete the Health Assessment and Biometric Screenings, in addition to the

employee, to earn the wellness reward.

If you are not enrolled in the medical plan for the 2018 calendar year we

encourage your participation and hope you enjoy all the interactive

wellness resources! There are many ways for you, no matter your medical

plan status, to get involved with Marquette Wellness. The Marquette
