You will be an “applicant” for the Faculty Travel Registration “program”

1 & 2 - Enter the dates you will be in each location (can be approximate at this time)

3 - After you click “Add to Itinerary,” the location and dates will appear here

Complete the 4 questionnaires and 1 signature document

Note that you will only complete one of these depending on which type of program you are leading
