Things to BringFor your WIC appointment, bring:• Proof of income or proof of eligibility

for Medicaid, Food Assistance, or FIP• Proof of address• Proof of identity for those who need

WIC• Your children who need WIC• Your child’s shot record• Medical information from your doctor

(if you have it)• Recent height and weight• Recent hemoglobin or

hematocrit level• Special formula prescription

If you’re pregnant, breastfeeding, just had a baby, or have children under 5, you may qualify.

We can help you buy milk, fruits, vegetables, eggs, cereal, juice, peanut butter, baby formula, and other healthy foods.

If you have questions about nutrition, breastfeeding your

baby, or feeding your family, WIC dietitians can help.

The Real Deal

To file a program complaint of discrimination, complete the USDA Program Discrimination Complaint Form, (AD-3027) found online at: http://www.ascr.usda.gov/complaint_filing_cust.html, and at any USDA office, or write a letter addressed to USDA and provide in the letter all of the information requested in the form. To request a copy of the complaint form, call (866) 632-9992. Submit your completed form or letter to USDA by:
