As we look back at 2019, Sterling Health Care has a lot to celebrate. We have continued to grow and evolve as an innovative organization while staying true to our mission to improve the lives for all people in the communities we serve, regardless of their ability to pay.

In the past year we have invested heavily in services that extend beyond traditional healthcare services. We added free transportation for our most vulnerable patients. We expanded our Community Health Worker program to better advocate for patients experiencing food insecurity, homelessness and even trouble paying their utility bills. We developed a robust Chronic Care Management program to support and encourage our most chronically ill and complex patients. We completed our planning and due diligence for the launch of our telehealth services, available to patients starting in January 2020. This annual report highlights these and many other ways we have moved our mission forward.

It has never been more important for a community organization like ours to have a culture of care and compassion for the people they serve. I truly believe our people live that culture every day as they serve others. We work tirelessly to create the resources and circumstances needed to provide extraordinary care to all of our patients.

The future of healthcare in America remains uncertain. This uncertainty will inevitably require change to which we must adapt. Fortunately, Community Health Centers have been continually innovating for the last half century. Community Health Centers make tremendous efforts to adapt to the needs of their communities so barriers to healthcare can be removed. They meet the needs of the whole person. Sterling Health Care is proud to operate by this mindset. Our willingness to innovate will be how we embrace the future of health care while providing high quality care and compassion for each and every patient.

One of our top priorities is delivering high quality health care to each of our patients.

Two of our primary care facilities are recognized as Patient Centered Medical Homes by the National Committee for Quality Assurance (NCQA) because of the ongoing quality care they provide.

We now serve over 15,000 patients.

In April, we implemented our transportation service and have provided rides to over 700 patients free of charge.

This year, we launched a chronic care program focused patients who suffer from two or more chronic conditions.

This year, we provided nearly $600,000 in discounted services to our communities.

Our health center is collaborating with community partners to help address issues such as food insecurity, social isolation, substance use disorders and more.

We have achieved consistent year-over-year improvement in quality metrics and have received a quality metric award of $115,000. These funds will be used to further our innovative health care strategies.

Our Mount Sterling and Owingsville primary care facilities renewed their patient-centered medical home recognition by the National Committee for Quality Assurance.

In August, we received a quality award of $155,524 for our improvements to clinical quality measures.

We will expand our school health program to Bourbon and Powell counties.

We will begin expansion efforts in Powell County to help address the lack of access to medical care in that community.

We will open a new, state-of-the-art facility in Nicholas County that will offer medical, dental, pharmaceutical and radiological services.

We will continue to partner with local health care organizations to provide health care education to the communities we serve.

We will launch new partnerships that will enhance our women’s care services.

We expect to provide over 100,000 visits in 2020.

We expect to serve 20,000 individual patients.

For Kim Burton and her family, finding transportation to and from appointments was difficult.

Kim’s husband works out of town during the week, and he's the only driver in their household. When Kim or one of their children needed to see the doctor, her husband would take off work to drive them to their appointments. But missing work meant missing a paycheck, and his job is the only source of income for the family.

When community health workers became aware of Kim’s situation they knew our new transportation service could help. The Sterling Health Care Transportation Service identifies patients who need rides to their appointments and takes necessary measures . They arranged for Kim and her children to be driven to and from their appointments at absolutely no cost to them.

Sterling Health Care employee Greg Manning had not seen a doctor in three years. He said he felt fine.

One day when he was walking by a clinical work area, a nurse noticed Greg’s odd cough and wheezing. She asked Greg to have a seat in an exam room, while she arranged for one of the providers to have a look at him.

Upon examination, they found Greg’s heart rate was dangerously high, which prompted an EKG (Electrocardiogram) to be performed. The EKG led to a trip to the local Emergency Room, which led to Greg being transferred to Cardiology services at a Lexington hospital, where he was diagnosed with Chronic Atrial Fibrillation (A-Fib) and was scheduled for a pacemaker installation and cardiac ablation. Quick thinking on behalf of Sterling Health’s Clinical Staff gave way to Greg getting the advanced care that saved his life.

INTEGRATED MENTAL HEALTH CARECHRONIC CARE MANAGEMENT PROGRAM

We work to treat the entire person through our multifaceted mental health care services.

We help patients with two or more chronic health issues to achieve better control over their conditions.

Chronic Care Managers follow up with the identified patients with nutritional advice, medication reminders, assistance with making appointments with Sterling Health Care providers, assistance with obtaining referral appointments to Specialists, and so much more.

Patients enrolled in the Chronic Care Management Program receive one-on-one, focused care throughout every stage of their diseases, to ensure that they obtain the best possible outcomes.

Mental Health Integration bridges communication gap in care.

Our team-based care model normalizes discussing mental health in the primary care setting during every routine visit. Acknowledging the connection between mental and physical health is vital to treating the whole person, so we do just that. Every patient is screened for depression before their visit—even if it’s just for a cold. During check-ups, providers ask about depression, anxiety, substance use and more.

Each patient's primary and mental health providers meet weekly to find the best care solutions based on the full picture of their health.

From its beginning, Sterling Health Care has employed Community Health Workers to assist patients with barriers to quality healthcare.

Whether that barrier is language, transportation, finances, food insecurities, homelessness, literacy or other barriers, the Community Health Worker works closely with the patient to locate solutions available through Sterling Health Care or its community partners.

We strive to bring health care to all by eliminating the obstacles between a patient and their needs.

Through the sliding fee program, qualifying patients can pay as little as $15.00 for a medical or dental visit.

NEW FACILITY

In April 2020, Sterling Health Care will break ground on the construction of its new state-of-the-art clinic in Nicholas County. The 8,700-square-foot facility will house medical and dental providers, as well as radiology equipment and a fully-stocked pharmacy.

The new clinic will provide expanded services to the Nicholas County area. The land for the clinic was purchased in November 2019, and the project has been met with enthusiasm from Carlisle City leaders.
