House Site Patta: - House less poor persons of the village will be given Government wasteland

D- Form Patta: - Land less poor persons of the village will be given Government wasteland for

Agricultural Purpose.

The service facilitates citizens for applying Extract copies of House Site/ D- Form Patta

in lost or Mutilated case.

Apply for Extract of House site/ D- Form Patta: This section contains instructions for the operators of the Meeseva web application to apply for Extract of House site/ D- Form Patta. 1) In Meeseva home screen, under List of Services, select Revenue Department services as depicted in Figure 1.

2) Select Extract of House site/ D- Form Patta Service in Revenue Department as depicted in Figure 2.

3) Now Extract of House Site/ D- Form Patta Request Screen will be displayed as depicted in Figure 3.

4) Select ServiceType (either House site patta/ D- Form Patta) as depicted in Figure 4.

Note: - Here I am selecting House site Patta Service

5) After Selection of service House site patta request screen will be displayed as depicted in Figure 5.

6) Enter all the required information for House site patta such as Type either Survey No./T.S.No. as depicted in Figure 6.

Note: - Here, based on Type (either Survey No/ T.S.No) Some fields are changed. Following fields are populated based on Type.

1. For Survey no---- Survey Number.

2. For T.S.No --- T.S.No, Ward and Block

Note: - Here I am selecting Survey number. 7) Enter remaining Details such as Survey number, plot, District, mandal and Village for House site Patta as depicted in Figure 7.

8) Enter all applicant details such as Applicant name, Father/Husband name, House Number, Locality/ Street name, District, mandal, Village/Ward, pin Code, Ration Card No, Aadhaar card No, mobile No,, Email id and Land Line number as depicted in Figure 8.

Note: - Based on the Citizen request, the delivery option (either through Manual or Speed Post - Local or Speed Post - Non Local at the Franchisee) has to be selected. As depicted in Figure 9. The courier charge for Post (Local) is INR. 33/- and for Post to Non Local areas is INR. 46/-.

9) Collect the necessary supporting documents as indicated in the House site patta/d- form patta service page. scan the documents and upload into the system, as depicted in Figure 10.

Note: All the mandatory fields have to be filled in; otherwise request will not be accepted by the system. Note: Ensure that the Document Address of the Land is accurate as the concerned mandal officials visit Land Address during the verification of his/her Patta particulars.

10) Click Show Payment. Another window appears requesting to Confirm Payment as depicted in figure 11.

12) Based on the delivery option chosen by the citizen, the service charges will be calculated and displayed in Show Payment as depicted in figure 13.

13) Collect the amount from the Citizen and click Confirm Payment to submit the request as depicted in figure 14.

14) On confirmation, a receipt will be generated as depicted in the following figure 15. The receipt will contain the Certificate delivery date. Note: Ensure that you have sufficient balance with your service provider (SCA), else the system will not accept the request.

Note: - On submission of the request, Citizen Receives Message. Like your Request for Hosu

Site/D-Form Patta has been entered vide Application Number, Transaction Id and sent to Dept.

15) On submission, the request will be sent to the work flow of the respective Department for

further processing.

16) Once the request is approved by Department, then House Site/ D- Form Patta will be dispatched through courier to the citizen address if the Delivery Type is Speed Post Local/Non Local.

� If Delivery Type Is Manual, then the citizen need to collect the House Site/ D- Form Patta Certificate from franchisee where he/she applied for the Certificate.

Note: - D- Form Patta service also same like as above.
