Explosives incidents and complaints January 2018 report This document presents statistics for all explosives safety and security incidents and complaints reported during January 2018. It also includes a cumulative summary of all safety and security incidents and complaints for the last twelve month period. The number of injuries and fatalities occurring in the community and industry are presented for the month of January 2018, as well as the cumulative total for the financial year 2017-18.

Explosives incidents and complaints January 2018 report This document presents statistics for all explosives safety and security incidents and complaints reported during January 2018. It also includes a cumulative summary of all safety and security incidents and complaints for the last twelve month period. The number of injuries and fatalities occurring in the community and industry are presented for the month of January 2018, as well as the cumulative total for the financial year 2017-18.

Vehicle incident (1 incident) A vehicle has caught fire inside a storage shed whilst loading explosives product.
Ammonium Nitrate Prill
Fire extinguished by emergency services. All exposed or contaminated product removed and disposed of. Disabled vehicle removed from storage shed. No injuries.

Upon investigation, following the second failure, it was discovered the auger bearing had failed and the auger screw was twisted. Site has rectified situation and maintenance program
While completing an underground inspection on the charging of a development heading, the shift supervisor noticed that three charged holes had started to smoke.
Inhibited emulsion
Reactive ground found to be cause. Advice sent to mine manager regarding considerations for future charging in the area.
2 x charged blast holes in pre-charged ring slumped and boosters and detonators had fallen into stope dirt. Explosives not noticed by operator and were tipped into truck.
Boosters and Detonators
Stope dirt segregated. Sifting through segregated material to reclaim non detonated explosives. All boosters and detonators have been located.
Pump incident (1 incident)
As a road tanker was being filled with emulsion the driver closed the belly valve on the tanker before stopping the pump. The pump pressure increased causing an immediate failure of a burst disk and a small spill occurred as a result.
Ammonium nitrate emulsion
The small spill was cleaned up and placed into the rework system. The reason the disc ruptured was known, a replacement has been installed and pump was restarted with no further incident.
Storage (1 incident)
A locking mechanism has failed, preventing access to a magazine.
Detonators A locksmith has removed lock, magazine was cleared, and welding repairs conducted in accordance with Australian Standards.

A leak of explosives at a storage site. Security sensitive ammonium nitrate
Stock take initiated and no discrepancies found. Leaked product recovered. Matter referred to Queensland Police Service.
Vehicle incident (2 incidents)
A vehicle near miss incident has occurred when an explosives transport vehicle and a dump truck came within 30 metres of each other.
Detonators No collision or injuries.
The rear trailer from a triple combination loaded with explosives has detached and rolled, causing spillage of product.
Ammonium nitrate emulsion
Driver error and road conditions were the contributing factors in this incident.

Contact occurred at low speed. Light vehicle sustained minor dent to cabinet door. A vehicle collision has occurred when a loaded MPU has reversed into the open explosives carry box door on a shot ute.
