Who should not take TRIKAFTA® (elexacaftor/tezacaftor/ivacaftor and ivacaftor)?Do not take TRIKAFTA if you take certain medicines such as:• antibiotics such as rifampin (RIFAMATE®, RIFATER®) or rifabutin

(MYCOBUTIN®)

• seizure medicines such as phenobarbital, carbamazepine (TEGRETOL®, CARBATROL®, EQUETRO®), or phenytoin (DILANTIN®, PHENYTEK®)

• St. John’s wort

Talk to your doctor before taking TRIKAFTA if you take any of the medicines or herbal supplements listed above.

What should I tell my doctor about my medical conditions before starting TRIKAFTA? Before taking TRIKAFTA, tell your doctor about all of your medical conditions, including if you:• have kidney problems

• have or have had liver problems

• are pregnant or plan to become pregnant. It is not known if TRIKAFTA will harm your unborn baby. You and your doctor should decide if you will take TRIKAFTA while you are pregnant

• are breastfeeding or planning to breastfeed. It is not known if TRIKAFTA passes into your breast milk. You and your doctor should decide if you will take TRIKAFTA while you are breastfeeding

TRIKAFTA may affect the way other medicines work, and other medicines may affect how TRIKAFTA works.Tell your doctor about all the medicines you take, including prescription and over-the-counter medicines, vitamins, and herbal supplements. The dose of TRIKAFTA may need to be adjusted when taken with certain medicines.Ask your doctor or pharmacist for a list of these medicines if you are not sure. Make sure the list includes medicines from all your pharmacies, if you have more than one.Especially tell your doctor if you take:• antifungal medicines including ketoconazole (such as NIZORAL®),

itraconazole (such as SPORANOX®), posaconazole (such as NOXAFIL®), voriconazole (such as VFEND®), or fluconazole (such as DIFLUCAN®)

What are the possible side effects of TRIKAFTA® (elexacaftor/tezacaftor/ivacaftor and ivacaftor)?TRIKAFTA can cause serious side effects, including:• High liver enzymes in the blood is a common side effect in people

treated with TRIKAFTA. These can be serious and may be a sign of liver injury. Your doctor will do blood tests to check your liver:

— before you start TRIKAFTA

— every 3 months during your first year of taking TRIKAFTA

— every year while you are taking TRIKAFTA

Your doctor may do blood tests to check the liver more often if you have had high liver enzymes in your blood in the past.

Call your doctor right away if you have any of the following symptoms of liver problems:

— pain or discomfort in the upper right stomach (abdominal) area

— yellowing of your skin or the white part of your eyes

— loss of appetite

— nausea or vomiting

— dark, amber-colored urine

• Abnormality of the eye lens (cataract) has happened in some children and adolescents treated with TRIKAFTA. If you are a child or adolescent, your doctor should perform eye examinations before and during treatment with TRIKAFTA to look for cataracts

What were the most common side effects seen with TRIKAFTA® (elexacaftor/tezacaftor/ivacaftor and ivacaftor)?The most common side effects of TRIKAFTA include:• headache

• upper respiratory tract infection (common cold) including stuffy and runny nose

• stomach (abdominal) pain

• diarrhea

• rash

• increase in liver enzymes

• increase in a certain blood enzyme called creatine phosphokinase

• flu (influenza)

• inflamed sinuses

• increase in blood bilirubin

These are not all the possible side effects of TRIKAFTA. Call your doctor for medical advice about side effects. You are encouraged to report side effects to FDA at 1-800-FDA-1088.

Trina was already thinking about finishing her latest robot kit. Trey grabbed his sketchbook, pencils, and his favorite toy robo-dog, Cybark, and headed down to the kitchen with Trina.

“Good morning!” said their mom and dad cheerily.

“Morning!” the siblings replied.

“Remember when we visited Dr. Lori Laboratory at the CF Center?” Dad said. “We discussed starting TRIKAFTA® and asked her a bunch of questions. After reading a few pages, Trina and Trey glanced up at each other in surprise— suddenly, they were wearing new gadgets and accessories. Then they looked up and realized they weren’t in their kitchen anymore. A dirt road lay beneath their feet, forking off into several different paths. Trina hurriedly pulled out a map of Trilandia to plot their course to Science City.

It wasn’t long before they arrived in Science

City, with its circuit trees and tall, gleaming buildings. Trey, who was

always very curious, had a whole beehive of questions buzzing around in his brain. He turned to his sister and asked, “What is TRIKAFTA®?

“I know a little about it,” Trina said. “Basically, our genes have changes, or mutations, in them. And that’s what causes CF. Right, Cybark?”

CF is caused by mutations in the cystic fibrosis transmembrane conductance regulator, or CFTR, gene. Everyone has 2 copies of this gene. If both copies have a mutation, the CFTR proteins produced from those genes don’t work like they should.

THE UNDERLYING CAUSE

DEFECT 1: Fewer CFTR

proteins get to the cell surface, where they are

normally located.

DEFECT 2: CFTR proteins don’t open correctly if they do reach the cell surface.

These defects can cause thick, sticky mucus to build up in organs, like the lungs.

CELL: A building block of life— your entire body is made up of cells! Different cells create different proteins, including CFTR proteins.

CFTR PROTEIN: A channel, or tunnel, that lets water and chloride ions into and out of the cell through a gate.

CHLORIDE ION: A particle that is part of salt. When it moves into and out of cells, water goes with it, which helps keep mucus thin and free-flowing.

MUCUS: A slippery substance that lines many different organs.

“Whoaaa,” said Trey. “I didn’t know all that was happening inside my body!”

“Now I’m even more curious to see how TRIKAFTA® works,” said Trina.

With the help of Cybark’s supersonic sniffer, Trina and Trey found Dr. Lori’s lab at the tippy-top of a very tall and very shiny building. The lab doors opened with a whooosh. The siblings looked around in awe at all the high-tech equipment.

“Welcome!” boomed Dr. Lori, with a coffee cup in her hand and a smile as bright as the sun.

“You have a spaceship that shrinks?!?” Trina said with delight.

“Of course!” said Dr. Lori. “We’ll use it to see how TRIKAFTA works in a cell with CF.”

“All 3 parts come together to help CFTR proteins work better. Elexacaftor and tezacaftor help more proteins reach the cell surface. Then, ivacaftor helps the proteins stay open longer.”

“Why do we need more CFTR proteins at the surface? And why do they need to stay open longer?” Trey asked.

“If I remember correctly,” replied Trina, “the more CFTR proteins there are at the surface—and the longer they stay open—the more chloride ions can move into and out of the cells. That helps the mucus stay thin and free-flowing.”

With Science City far behind them, Trina and Trey wanted to rest up before entering the Forest of Flavorful Foods. They found a little creek and decided to sit beside it.

While Trina rummaged around in her backpack for a snack, Trey found that the round stones on the bank were perfect for skipping across the crystal-clear water. He easily made some lighter stones skip at least three times before they sank. Then he wanted a new challenge. He tossed a bigger, heavier stone with all his might across the creek, crossing his fingers for at least one skip. And…it sank with a plunk, making a new question buzz around in his brain.

“Hey, Cybark, do all kids take the same amount of TRIKAFTA®?” he asked. “Because we’re all different sizes, like these stones, and they don’t all skip the same way.”

IMPORTANT SAFETY INFORMATION

GUIDANCE FOR TAKING EACH DOSE

Your child should always take TRIKAFTA with a meal or snack that contains fat to help the body absorb the medicine.

Talk to your healthcare provider about all the medicines your child takes, as the dose of TRIKAFTA may need to be adjusted. Make sure your child takes every dose of TRIKAFTA exactly as prescribed by their doctor. Because the 3 parts of TRIKAFTA work together, each and every dose matters.

Avoid foods and drinks that contain grapefruit while taking TRIKAFTA because they may affect the amount of TRIKAFTA in the body.

With their backpacks full of foods from the forest, Trina and Trey were ready to get back on the road.

“Guys, wait up!” shouted Trey, with his and Trina’s backpacks slung over his shoulders.

“Oh no, did I forget it again?” Trina said apologetically. She tended to be a little forgetful. “Wait, what happens if I forget a dose of my TRIKAFTA?”

“Why don’t we ask Dr. Lori?” said Cybark, letting out a bleep-blip-bloop as he contacted the doctor.

“Hello!” said Dr. Lori as Cybark projected her image. “I’m thrilled to see you both again. What can I do for you?”

WHAT TO DO IF A MORNING DOSE WAS MISSED

Do not take more than the usual dose of TRIKAFTA® (elexacaftor/tezacaftor/ivacaftor and ivacaftor) to make up for a missed dose.

If it’s been 6 hours or LESS since the morning dose is usually taken:• Take the missed dose with fat-containing food as soon as possible• Then take the next dose at the usual time with fat-containing food

If it’s been MORE than 6 hours since the morning dose is usually taken:• Take the missed dose with fat-containing food as soon as possible• Then do not take the evening dose. Resume regular dosing

As they entered Keep-in-Mind Carnival, the siblings took in sights, sounds, and smells from across the land. Each food cart had a different snack from the Forest of Flavorful Foods—from ice cream to string cheese to so much more! And everywhere they turned, they saw rides and games that reminded them of all the things they had learned in Science City. Trina’s eyes were drawn to a rocket-themed roller coaster rumbling above her.

“Ooh, I could spend all day here,” said Trina. “Look!” she shouted, pointing to a mysterious man wearing a long cape and a top hat, standing next to a carnival booth. “I think that’s him.”

“Um, excuse me, Mr. Memory Magician?” Trey asked politely as he approached the man.

“Yes!” answered the man, with a raised eyebrow. “It is I, the Memory Magician! At your service!” he said with a deep bow and a dramatic swish of his cape.

“Dr. Lori told us to come find you,” Trey replied. “She said you’d give us some ideas for remembering to take our TRIKAFTA®.”

“But of course,” said the Memory Magician as he twisted his long, thin mustache. “Watch closely.”

“I’m going to need some assistance for these tips,” said the magician as he opened his large cape, “from two people who can help you make TRIKAFTA a part of your routine.” He dropped the cape, revealing:

“Mom! Dad!” Trina exclaimed. “How did you get here?!”

“SHH, it’s magic!” whispered Mom, with a twinkle in her eye.

“Now, one way to remember your morning and evening doses is to take them with a meal, like breakfast,” the magician said, signaling Mom to reach into the hat. Trina and Trey gasped in amazement as she pulled out a steaming plate of eggs and sausage. “Or dinner,” he said. When Dad pulled out a plate full of macaroni and cheese, the aroma made the siblings’ stomachs grumble.

“You can also ask your parents to set reminders on a phone or calendar,” said the magician with a wave of his wand. Cybark let out a yip! before disappearing in a puff of smoke. “Or, in your case, your robo-dog can remind you,” he said as Cybark reappeared in his hands.

“Another idea may be to give you a sticker for every day you take your TRIKAFTA,” said the magician as shiny stickers appeared all over Cybark. “Try putting them on a calendar to help you track your treatment.”

“Whoa!” Trey exclaimed as a very sparkly Cybark scurried back to them.

“Thanks for the tips, Mr. Magician!” said Dad as he gave the man a pat on the back. “You’ve taught all of us something new today.”

“Great work, kids!” said Dad proudly. “Now it’s time to head back home.”

Trina pulled the book out of her backpack. She flipped to the last page and began to read. “‘You and your child have learned so much about TRIKAFTA® in each of your adventures,’” she read. “‘Now your child is ready to start their new treatment! Just remember, talk to your child’s doctor if you have any questions about TRIKAFTA.’”

When Trina looked up from the book, she realized they were back at home. The sun had set, and Trey was fast asleep. Cybark was tucked safe and sound against his chest. Trina let out a long yawn.
