In our drive to provide the best chance for each individual/couple to conceive, we are continually upgrading our treatment protocols and our IVF laboratory equipment. MEET DR. KUMAR

ASHIM KUMAR, M.D.

Ashim Kumar, M.D. is well versed in all aspects of reproductive medicine including ovulation induction, intrauterine insemination, in vitro fertilization (IVF), intracytoplasmic sperm injection (ICSI), and preimplantation genetic diagnosis (PGD). He has a special interest in hormonal and functional causes of infertility such as polycystic ovary syndrome (PCOS), tubal disorders, and male fac-tor. Dr. Kumar is Board Certified in Reproductive Endocrinology and Infertility, and Obstetrics and Gynecology.

For his surgical acumen, Dr. Kumar was recognized by the American Association of Gynecologic Laparoscopists with a reward for Special Excellence in Endoscopic Procedures. He has co-au-thored the Resolve brochure on pelvic adhesions and the Society of Reproductive Surgeons’ guidelines on the treatment for fibroids for infertility.

Dr. Kumar completed his fellowship in Reproductive Endocri-nology and Infertility from the combined program at University of California Los Angeles and Cedars-Sinai Medical Center working with world-renowned leaders in the field. He completed his medi-cal education and residency in Obstetrics and Gynecology at Emory University in Atlanta, Georgia.

In addition to having published numerous articles in peer-re-viewed journals on endocrine and surgical aspects of infertility, he has presented his work at the annual meeting of the American So-ciety for Reproductive Medicine. He serves as an ad hoc reviewer for the journal Fertility and Sterility. Dr. Kumar has written several textbook chapters on management of tubal factor infertility and ovarian function including premature ovarian failure.

Our most recent IVF pregnancy rates, representing 1,501 cycles from January, 2007 to May, 2010. We are proud to announce our results are, once again, among the best in the nation. Our pregnancy rates are particularly favorable as FSAC treats all patients regardless of age, FSH levels, or prior outcome. The following IVF pregnancy rates are inclusive of ALL PATIENTS, including those with poor ovarian reserve/egg quality (elevated FSH levels) and other conditions that negatively impact pregnancy rates. Pregnancy rates for couples without elevated FSH levels or diminished ovarian reserve are significantly higher than those listed below. Please note that these are actual live birth rates, not clinical or ongoing pregnancies. We are also pleased that our delivered high order multiple pregnancy rate (triplets or more) was only 1.3%.

FSAC comprehensively reviews each IVF cycle with the entire FSAC team (physicians, embryologists, and nurses) to ensure the optimal tailoring of the medication regimen, laboratory techniques, and embryo transfer for each pa-tient. With continual refinements in IVF Laboratory technology, ovulation in-duction techniques, and physician-only monitoring of follicle measurements, our success rates for in-vitro fertilization continue to rise, and rank among the very best in the country.

FSAC also maintains a very busy controlled ovarian stimulation/insemi-nation program, performing over 1400 inseminations annually. Success rates for this treatment are approximately 20-25% per cycle. Many of our patients conceive with this therapy, with significantly lower cost and stress, without being streamlined directly to IVF.

We are pleased to report that our IVF Program continues to develop and grow. As one of the busiest programs in the Western U.S., we are again on track to perform well over 500 IVF cases (including those with ICSI and PGD) this year. In addition to FSAC’s success with IVF, we are committed to treating our patients with less aggressive treatments when indicated.

We are fortunate to have experienced tremendous growth since we opened in 1999, yet we care for our patients today the same way we treated the first patient who walked through our door – with respect and compassion.

Our pregnancy rates reflect the care and dedication of the entire FSAC team. Dr. Buyalos, Dr. Hubert, and Dr. Kumar use the most successful ovarian stimulation protocols. They personally monitor follicular growth for each pa-tient and use ultrasound guided-embryo transfers. The expertise of Man Li, Ph.D., HCLD, ELD, (IVF Laboratory Director), combined with our state-of-the-art laboratory facilitate the highest embryo quality available today.

PREGNANCY RATES WITH OTHER TREATMENTS:Our success in treating infertility without IVF is due to metic-

ulous physician monitoring of follicle/endometrial growth and di-rect physician performance of all ancillary procedures, including intrauterine inseminations. All procedures, including ultrasounds, are performed by board-certified reproductive endocrinologists, not nurse practitioners or technicians. This personalized approach en-sures that our patients receive the highest medical and psycholog-ical support possible.

SART & CDC STATISTICS:We report our statistics to the Society for Assisted Reproductive

Technology (SART) division of the American Society for Reproductive Medicine (ASRM), the central reporting agency that ensures the ac-curacy of pregnancy rate data. Not all programs are registered with SART. We encourage you to examine pregnancy rate data from all programs by contacting ASRM or by going to the Center for Disease Control web site. The ASRM asks us to remind you that as entry criteria are highly variable for each program, a center-by-center comparison of results is not valid.

I am a mother of three beautiful children; Lay-lah (10), Amayah (9) and Asyaiah (7). My husband, Lorenzo, and I have been married for 11 years. He has served in the Military for a decade. Although being a military family comes with challenges, it has also made us stronger. For us, becoming par-ents was our dream come true and we completed our family at a young age. I truly love being preg-nant and wanted to help other couples, who were faced with obstacles, start a family.

My first journey began five years ago. I met the intended parents; we instantly connected and were excited to get started. Soon after, I found out I was pregnant with a baby girl. In December of 2008 their beautiful healthy baby was born. When you see how much love parents have for their newborn it makes the experience so satisfying. I am grateful to be part of that miracle.

My second surrogacy journey was an experi-

ence full of love but also loss. For this journey I decided to help a gay couple start a family. After meeting the dad’s we were all excited to move forward. We implanted one embryo and five days later got the positive results. I was pregnant! Both dads flew to my hometown for the big gender re-veal. I knew the gender, because of a previous ul-trasound so I made them a cake and inside it was the color blue for a baby boy. We cut the cake and the dad’s found the exciting “color” announcing their son! The due date approached fast and that summer their healthy baby was born.

I went into my second surrogacy thinking we may do a sibling project, meaning I would help them have another baby. But a year ago we heard some devastating news. The biological fa-ther found out he had brain cancer. He was strong and fought hard, but lost his battle in March of 2013. The last year was really hard for us all.

Even though I had only known him for a little more than two years it was like losing a family member. It breaks my heart to say that cancer took his life but it did not take away all of the love we have for him. Before he got sick one of his goals was to start a surrogacy agency and help others find the joy that he and his partner found through the process. He knew I had always wanted to work for an agency as well. I wanted to not only be a sur-rogate but I be able to connect others and help them on their path of becoming parents. I recently was hired to to work for Expect Miracles. This is another dream come true because I am not only doing this for me but for the amazing man we lost. I hope he is proud of me for achieving this goal. I love getting the chance to show that surrogacy is a very positive experience. It is my passion. I am here to help others and be the best mom, wife, friend and surrogate I can be.

I was a teenager when I first found out about my condition. I was born without a uter-us. The condition is called Mayer-Rokitan-sky-Kuster-Hauser Syndrome (MRKH) and it af-fects about 1 in 4500 women. The severities of this syndrome vary, but all women who suffer from MRKH face one consistent truth: you will never be able to get pregnant. Accepting this diagnosis was an emotional battle, but overtime and as I grew older, I was able to have a more optimistic look on my future. However, the condi-tion continued to loom over me when it came to matters of the heart and falling in love.

When I met my husband, Zack, I was so afraid that I would ruin his chance of being a father. I held back and tried not to get involved for his sake. Well, true love is very persistent and he stole my heart anyway. He has been by my side and sup-portive ever since. I have known for a long time that I’ve wanted to have a family and that it would

be a difficult process. I have healthy ovaries, so I can still have children but they would need to be carried by a surrogate. A friend and fellow MRKH sister connected me with an organization that offered to help with the surrogacy process. Now, thanks to Surrogacy Together and Expect Miracles my prayers have been answered; I’m getting the chance to become a mother!

Being a family with my husband means the world to us both. To be able to have so much love and not be able to share it with a little miracle is heart wrenching. We can’t wait to be the best parents we can be and for our love to grow. I am extremely excited to start our surrogacy journey and also be living proof that infertility, whatever the diagnosis may be, is not the end of the road to parenthood. Today, I am more determined then ever to beat MRKH and prove that my desire to become a mother can overcome the challenges and together we will reach our destination.

COMPANY OVERVIEW Expect Miracles Surrogacy was founded in Northern California in late

2006. We originally contracted with IVF Michigan that has 5 clinics in Michi-gan and Ohio. The first two years we limited our clients to patients of IVF MI. In 2008 with the economy struggling and the laws in many of the Mid-West-ern States becoming less and less favorable toward Surrogacy we moved the office to Southern California and started working with several San Diego and Los Angeles area IVF clinics in addition to our clients from IVF MI. For the last four years our growth and doubled each year.

Based on the % of growth we continue to see we expect our Agency to double again this year, 2014. We have added 4 staff members since Jan. 1st and continue to locate more qualified case workers in order to assure the women that act as surrogates have the resources they need to do a good job for the parents that they carry for. 80% of our clients are international. We have procedures to help with every step to ensure that even with the distance their surrogate is healthy and their child is getting the best possible care.

This year we are excited to add the only Guarantee Program by a suc-cessful Agency in the U.S. that offers a full refund to couples who are not successful in two years. We have had about 10% of our Intended Parents not achieve the goal of having a child due to the high costs to continue IVF treatments if the first cycle does not work. This option solves that issue and we look forward to assuring the EVERYONE goes home with a baby in the end!

CHRISTY ANDERSONCEO FOUNDER OF EMS Christy founded the Agency in 2006 when she started researching Surrogate Agencies for her own needs of becoming a mother and became very disappointed by the options and services offered by the Agencies that were available with California Surrogates. They were typically operated by Lawyers who just provided profiles of sur-rogates but failed to see how the surrogates needed to be screened and the concern of Intended Parents. The other option was Agencies operated by former Surrogates. The Surrogate run Agencies only saw surrogacy as a way to get the most for other Surrogates and also did not supply high quality Surrogates that Christy would be willing to trust in carrying her child. She saw what was missing was an Agency operated in the effort to advocate for the Intended Par-ents in finding them the best Surrogates, providing the highest level of background screening and psychological evaluations. The unique thing Christy continues to bring to the Agency is her interview with each approved Surrogate Candidate.

JON ANDERSONCO-CEO DIRECTOR OF OPERATIONSJon joined the company when operations were moved to Southern California in 2008. Jon is a father through Surrogacy to a daughter born in 2010. He is active in the day to day operations of Expect Miracles Surrogacy. He coordinates outside contracts with doctors, negotiates with insurance and web design contracts. Daily he does the initial new Intended Parent intake and assists them in applying to the Agency. He coordinates appointments for Intended Parents and oversees the staff that monitors the Surrogates. Jon is dedicat-ed to making Surrogacy a positive experience for everyone involved with the final goal a healthy infant to complete a family.

Screening in-cludes in depth interviews, background checks, medical approval by her maternal doctor, medical records and physical exam performed by the qualified IVF physicians.

THEIR BACKGROUND:Our Surrogates come from stable homes with supportive families and friends. They have found pregnancy to be pleasurable. They feel skilled and confident about participating in a surrogacy arrangement.

THEIR CONFIDENCE:Our surrogates are highly confident from the start about the medicalprocess, their ability to look after the well being of the baby and the birth. BUSINESS INTRODUCTIONWhether you are the Surrogate Agency, In-

tended Parent or Attorney you want the assur-ance that no funds will change hands until ALL of the instructions in the transaction have been followed. Allison-McCloskey Escrow Company has the obligation to safeguard the funs and/or doc-uments while they are in the possession of the escrow holder, and to disburse funds as required.

Be sure to choose Allison-McCloskey Escrow Company an independent licensed escrow provid-er who is 100% neutral, and whose sole business is providing escrow services. We are licensed by the Department of Business Oversight (DBO), who requires adherence to the most stringent stan-dards in the industry.

THE ESCROW PROCESSThe escrow process begins with the signing

of escrow instructions by the clients (Intended Parents) and the Surrogate Agency, if any. The ini-tial deposit is deposited into the escrow account and surrogate fees and bills are paid from this account. Any request for funds from this account will require the signatures form the Agency and /or Intended Parent and will be paid directly by the Allison-McCloskey Escrow Company to each request. After all bills and fees related to the Sur-rogacy agreement have been paid, any refund due is issued to the client (Intended Parents) and the escrow account is closed.

Allison-McCloskey Escrow Company has been working with Surrogate Agencies for 12 years and many Attorneys have recommended us to their Surrogate Agencies.

We also handle Egg Donation Agreements, but special language is required to assure the anonymity of the donor, so please notify us when opening escrow.

If we had been able to get married in our state sooner, we would have done so long ago. We met back in 2005 while studying abroad in Madrid, Spain. Now, nine years later we will be among the first gay couples married in Illinois after the same-sex marriage law takes effect in June of 2014. This opens the door for the next step, starting a family, because it allows legal parental protection. We don’t want to have a child to fulfill our own needs, but rather to sacrifice part of our life to give to a baby. We are at the point in our lives where we are ready for that opportunity.

After many hours of research we found Expect Miracles Surrogacy and noticed that they had a financial assistance program, we applied and started the journey together. We are so grateful for the medical advances and to organizations like this one that have opened the possibility for people like us

to start a family. We want to make parenthood as equal as possible so we’ve decided both of us will be donors. It will make it that much more special to have two babies with each other’s DNA. This will be as close as we can get to having children together and completing our family.

With a very strong support system and our families behind us 100%, we feel equipped to face the difficulties and challenges of raising children as a same gender couple. Our families will be by our side every step of the way and are especially excited for grandchildren! We’ve waited a long time to make this official and in the process show others you don’t have to fit into a “traditional mold” to start a family. Attorney and Intended Parent Lesa Slaughter knows only too well the emotional anxiety along with the

After two surrogates, almost two years and several failed IVF transfers, Lesa and her husband began their family in early 2010 by welcoming twins Brock and Eva, born through gestational surrogacy.
