• Current conservative treatment options of acute wounds to enhance healing by secondary intention typically involve a moist wound-healing environment.

• This is provided by hydrocolloids, hydrogels, alginates, foam and hydrofiber dressings, and paraffin gauzes.

• Recently, a new plant-derived wound therapeutic was introduced. The mixture of hypericum oil (Hypericumperforatum) and neem oil (Azadirachtaindica), is designed to create a moist wound healing environment while preventing the secondary dressing from adhering to the wound.

• We present our first experience with this product for the treatment of different acute wounds.

Case 1

The 3-year-old girl presented with crush injuries of the fingers 2 to 5 of both hands, after her hands got caught in a fitness-machine.

From the 1st day on we used 1 Primary Wound Dressing® to treat the full thickness skin lesions. The painless daily application was done by the mother.

After 3 weeks wound healing was complete.

Case 2

As a result the wounds started to reepithelialise and the patient could leave the hospital within 3 days. He used the spray autonomously at home and was seen twice weekly in our wound clinic.

A 46-year-old male patient presented with alkali burns around both ankles, caused by hot liquid cement.

Debridement of the necrotic wounds was performed in the operating room 4 days after the injury. During the first week the wound dressing with sodium hyaluronate was changed daily, then we started to use 1 Primary Wound Dressing®.

The wounds have completely healed in 7 weeks.

Case 3

Two days later we started using the wound spray. The patient applied the spray twice a day. Complete wound healing was achieved by day 16.

The 84-year-old lady presented with 2nd degree burns of the fingers 3 to 5 of the right hand. She has burned herself with a candle.

After debridement of the blisters was performed, Jelonet gauze was initially applied.

Conclusion

• Our initial clinical experience suggests that this novel plant-derived wound spray is efficacious for soft tissue defects of different aetiologies.

• The spray can easily be applied by the patient, and enables an almost pain free change of dressing.

• 1 Primary Wound Dressing® is very simple to use, safe and potentially cost-effective for the treatment of various acute wounds.
