What is it?

A Lockout/Tagout (LOTO) program ensures that energized equipment or machines are not started or used while they are being serviced or cleaned.

It is also referred to as an energy control program or procedure.

Definitions

Lockout: A physical lock that does not allow a machine to be energized or started.

Tagout: A tag used on machines that cannot be locked out for various reasons. It is a warning, but not a lock, that the machine should not be started or energized.

How is it accomplished?

By de-energizing machines and equipment.

By affixing the appropriate lockout or tagout devices to energy-isolating devices.

(In other words: turn the machine off, turn the breaker off, and literally lock the machine from being able to be started)

What is the purpose?

According to the Occupational Safety and Health Administration (OSHA), employees servicing or maintaining machines or equipment may be exposed to serious physical harm or death if hazardous energy is not properly controlled.

Compliance with the lockout/ tagout standard prevents an estimated 120 fatalities and 50,000 injuries each year.

What types of energy are included?

Hazardous energies can include:• Electrical• Mechanical• Hydraulic• Pneumatic• Chemical• Thermal

Who does the program affect?

There are three types of employees described in this program:

• Authorized employees• Affected employees• Other

Authorized employee

Authorized employees are trained in implementing a LOTO procedure. They have the means necessary to control hazardous energy.

This is the employee who would perform maintenance or service for the machine.

Affected employee (and other)

These employees are required to recognize when a LOTO procedure is being used and why it is being used.

This employee will not initiate a LOTO procedure but will recognize when one is in progress and refrain from using the machine.

What type of employee am I?

No employee at Evolution Industries can be categorized as an Authorized employee.

All employees are Affected employees, which means that you may need to recognize a LOTO procedure being used, but will not have to implement one yourself.

What types of devices are included?

Most machines at Evolution are able to be unplugged. These machines do not require a LOTO procedure.

If you service one of these machines, you must unplug it first. The plug must remain under your control at all times until servicing is complete.

The only equipment at Evolution that must use a LOTO procedure during servicing are the ovens and the air conditioning unit.

At this time, the only personnel authorized to service these machines are outside vendors.

They will be required to use a LOTO procedure while at Evolution Industries.

What do locks and tags look like?

Training

Training will be provided on a yearly basis.

Additional training may be required if:• There is a change in your job assignment• There is a change in machines, equipment

or processes that present a new hazard• Whenever a periodic inspection reveals

noncompliance • An employee demonstrates negligence or

unfamiliarity with the established program

affected employees.You must learn to recognize when a LOTO

procedure is in place and REFRAIN from starting or energizing any included equipment.

When in doubt about a piece of equipment, always ask your manager.

If you notice a machine or equipment that needs servicing, notify your manager immediately.
