Notes:

In this online presentation we will look at identifying evidence based guidelines and the roles

they play in our clinical practice; describe common limitations encountered by providers

when using evidence based guidelines in clinical practice; and how to apply the most recent

evidence-based health promotion and clinical prevention guidelines for selected scenarios

commonly encountered in clinical practice.

What is Evidence-Based Practice

Notes:

So what is Evidence-Based Practice or EBP? In 2015, Melnyk & Fineout-Overholt stated that

EBP is a Life-long problem solving approach to clinical practice, leads to evidence-based

clinical decision making, which is influenced by external and internal evidence from research,

clinical expertise, and patient preferences.

External evidence from research includes evidence-based theories, opinion leaders and

expert panels. Clinical Expertise incorporates internal evidence generated from outcomes

management or quality improvement projects, a thorough patient assessment and

evaluation, and use of available resources. Patient preferences and values must be

incorporated when providing evident based care.

What are Evidence-Based Guidelines or Recommendations

Notes:

In 1990, the Institute of Medicine (IOM) defined evidence-based guidelines or clinical practice

guidelines as. Systematically developed statements to assist practitioners and patient

decisions about appropriate health care for specific clinical circumstances.

Why is it Important?

Notes:

So why are evidence-based guidelines and recommendations so important for advanced

practice nurses to know and utilize. For one it helps to facilitate decision making about

appropriate health care by providing us with a summary of the relevant healthcare literature.

It also offers us assistance in decision making aspects of our clinical practice. As you

continue through your education, you will learn these guidelines impact you or your

institution’s ability to receive the highest reimbursement allowed.

Recommendations for Clinical Practice Development

Notes:

The process used to develop evidence-based guidelines and recommendations involves both

content and methodology experts. This process is clearly defined, reproducible and results

in a systematic development. First the clinical question must be defined and addressed by

the guideline. Then the criteria that will be used to select the studies and support the

guideline or recommendation are defined. Then a systematic literature review and

evaluation of the quality of the data obtained in the systematic literature review is evaluated.

Limitations of Evidence-based Guidelines

Notes:

Sometimes there are limitations of the evidence-based guidelines or recommendations for

clinical practice, let’s review these. Sometimes the evidence supporting a particular

recommendation may be lacking due to guidance is limited by the number of high quality

studies reviewed in the development process. Another limitation is the supporting studies

leading to the recommendation may be misleading due to the limitations of the study design

resulting in biased results, which can lead to incorrect recommendations. This is why critical

appraisal is extremely important to assess inconsistencies. The evidence may be

misinterpreted by the authors of the guidelines due to differences in opinions and values or

conflicts of interest. Sometimes the guidelines do not consider or address patients’ needs,

resources available, costs, efficacy and/or safety considerations. Another important

limitation is the new developments identified in the research may not be captured by

outdated guidelines.
