Ambulance (to be used as Wellness-Van for periphery area) on hire for this hospital

payable on consolidated rate on per day and kilometer run basis.

Interested agencies may collect detail tender from this office on payment of

Rs.100/- (non-refundable) through D.D. payable to ESI fund A/c No. 1 and payable at

Ranchi between 9.00 A.M. to 4.00 P.M. on all working days upto 1.30 P.M. of 29.06.11.

Last date of receiving tender will be 29.06.11 upto 2.30 P.M. Tender will be open at 3.00

P.M. of 29.06.11., in the presence of Tenderers who want to be present, at the office of

Medical Superintendent.

Sir,

Sealed tender in two separate envelopes containing technical bid and Price bid are invited in the

name of Medical Superintendent, ESIC Model Hospital, Namkum, Ranchi in the enclosed prescribed

Performa for providing one Ambulance for use as wellness van and the same is required for use of a

team of Medico/Para-medico professionals for visit to periphery area for providing preventive and

promotive health care services to its beneficiaries. The team will visit the periphery area on a scheduled

date once in a week upto 24th

February 2012. You are requested to submit your Bid latest by 29.06.11

upto 2.30 P.M., in the form of sealed Bid under Two bid system-Technical Bid & Price Bid. Technical Bid

should consist all the technical details i.e. Registration Certificate, Insurance paper, Document in support

of Road Tax, Driving License of the driver, along with commercial term & conditions & Price Bid should

contain all the amounts chargeable i.e. daily rental of Ambulance, daily rental charges of driver,( if

chargeable additional )mileage charge i.e. km./run Per km. Diesel version/petrol version etc. mentioned

in technical bid. The Technical Bid will be opened in first instance. Technical Evaluation Committee will

make the recommendation based upon the Technical Bids received. Thereafter, the price bid will be

opened by the Committee appointed by the Competent Authority as described in subsequent paras.

Price bids of only, eligible (short listed) tenders will be opened.

The Technical Bid & Price Bid be sealed by the bidder in separate envelope duly super scribed

“Technical Bid for Wellness Van Ambulance” & Price Bid for Wellness Van Ambulance may be dropped in

the tender box kept for this purpose in this hospital upto 2.30 P.M. on 29.06.11. If the bid is sent by

post, it must reach before 2.30P.M. on 29.06.11. Bid received late will not be considered. Bid received

will be opened on the same day that is 29.06.11 at 3.00P.M. Bidders or their authorized representative

may be present if they wish to be, in case 29.06.11 is declared as holiday; bids will be opened on next

working day at the same time & venue. Bid should be accompanied by Earnest Money i.e. Rs. 3,500/- in

the form of D.D./Banker Cheque in favour of ESI Fund Account No.1 Bid without earnest money deposit

will not be accepted.

Tender form not accompanied with requisite earnest money and required documents will be rejected

out rightly without assigning any reason/entertaining any correspondence. The earnest money of

unsuccessful tenderers will be refunded without accrual of any interest in due course of time. The

competent authority of ESIC reserves the right to reject on or any tender in whole or in part, without

assigning any reason thereof.
