Remote Independent Verification Module

What is Remote Verification?

Remote Verification (or independent check) is a critical LOP to ensure that critical steps (single-point-vulnerabilities) in action-by-action procedures are performed correctly.

The sum of the error rate of each critical step determines the overall failure rate of the action-by-action procedure.

Further, independent verification can be used to ensure that occupational safety related LOPs are also performed correctly.

Examples:• voltage check of de-energized equipment• pressure check of the system before disassembly

of a valve• equipment ID check of de-energized equipment

Independent verification can occupy as much as:• 20%-30% of the work force • about 20% of the procedural execution time due

to waiting on the independent verifier.

This can be a very burdensome process.

Independent verification can fail because of two reasons:1. Blind trust of the verifier in the requestor, thus

performing the verification without independency.

2. Distraction of the verifier, often related to non-job related conversations with the requestor or attempting to multitask during verification.

• The remote verification module eliminates the need and time wasted spent on coordination between the verifier and the requestor, as well as physical travel to a site to verify the job.

• Instead, a photo of the performed action (such as the voltage reading of an de-energized system) and the doer are sent via KALETM to the verifier for review and approval.

• Regardless of location or schedule, this verification process can be carried out instantly. The verifier, whom can be located anywhere, reviews the job performed and either approves or rejects the performance. With this instant feedback and approval, the doer can proceed to the next action without delay.

• The entire process of verification from sending the request, the approval/rejection, and feedback and receipt is documented and can be viewed in the notification center and report center to track and record the progress of a job.

reducing the error rate of independent verification).

The doer takes a picture (or has a co-worker take a picture) of the performed action to be verified.

In this example, it is a voltage reading. After the doer also takes a picture of himself, he can also fill out a description as well as any other important information for the verifier to review. Both pictures are stamped with the date and time. Once complete, this is sent immediately to the assigned verifier for review and approval via KALETM and email notification.



The notification center provides the infrastructure for instant notice of request, acknowledgement, and receipt of request.

The verifier receives a verification request that with a quick snapshot of the action to be performed, a short note, the requester’s name, and date and time of completion.

Verifier then clicks ‘open report’.



After the verifier receives the request in the notification center, the verifier is able to see the full Remote Verification Report after clicking ‘open report’.

The verifier is then taken to the Report Center, where all remote verification reports are saved. Here the verifier is able to see the full documentation of this request, and can easily print, approve, or not approve with a click of a button.



After reviewing the verification request documentation and feeling comfortable the requester can carry forward with the action/job/task being performed, the verifier approves this request by clicking ‘accept’.

The verifier also has an option to provide any additional comments or feedback to the requester regarding the action/job/task being performed and/or next steps.



Once approved (verified), the requester receives receipt of this approval via the notification center and can carry on with completion of the action/job/task at hand.

Clicking ‘Acknowledged’ marks this notification as “read” for easy organization and call-for-action in the notification center.

Clicking ‘open report’ will take the requester to view the final report.



The final Remote Verification Report can be viewed by the requester and verifier in the Report Center. The verification request and status of approval is permanently documented for future records.

Here, the report can also be printed, deleted, shared to other KALETM users in the company, and can also be marked as “critical” to help rank/organize requests.



All Remote Verification reports saved can be viewed in the Report Center.

Here, reports are titled by date/time created and requester name.

Each report can also be tagged with the ‘critical’ icon if marked critical.
