INTRODUCTIONThis program of study is accredited by the Australian Nursing and Midwifery Accreditation Council and approved by the Nursing and Midwifery Board of Australia for registered nurses seeking to apply for endorsement as a Nurse Practitioner.

‘A nurse practitioner is a registered nurse who is educated and endorsed to function autonomously and collaboratively in an advanced and extended clinical role. The nurse practitioner role includes assessment and management using nursing knowledge and skills. The role may include, but is not limited to, the direct referral of patients to other health care professionals, prescribing medications and ordering diagnostic investigations. The role is grounded in the nursing profession’s values, knowledge, theories and practice, and provides innovative and flexible health care delivery that complements other health care providers’ (Australian Nursing and Midwifery Council, 2006).

ADMISSION REQUIREMENTSThe Master of Nurse Practitioner program requires that students are registered nurses and engaged in advanced nursing practice in a clinical setting approved by the School of Nursing and Midwifery (SoNM).

Advanced nursing practice (ANP) is a continuum along which nurses develop their professional knowledge, clinical reasoning and judgement, skills and behaviours to higher levels of capacity (that is recognisable). Nurses practising at an advanced level incorporate professional leadership, education and research into their clinically based practice. Their practice is effective and safe. They work within a generalist or specialist context and they are responsible and accountable in managing people who have complex health care requirements.

The NMBA 2014 Nurse Practitioner Standards for Practice, distinguishes between ANP and the advanced level required as a Nurse Practitioner. They term this Advanced Practice Nursing.

Advanced Practice Nursing as a nurse practitioner is a qualitatively different level to the advanced nursing practice of the registered nurse due to the additional legislative functions and regulatory requirements. The requirements include a prescribed educational level, a specified advanced nursing practice experience; and continuing professional development. (NMBA Nurse Practitioner Standards for Practice, 2014)

The Master of Nurse Practitioner program requires that students complete a minimum of 300 hours of advanced Integrated Professional Practice (IPP) with embedded 20 hours of one on one clinical supervision.

INTER-PROFESSIONAL SUPPORT NETWORKThe Inter-professional support network is a multi-disciplinary group nominated by the student in consultation with their supervisors and the Program Convenor after enrolment in the program, to support the student’s learning and development in an advanced nursing practice role. The group should be formed by the student.

The composition of the Inter-professional support network is to be determined in the context of the student’s practice setting and will include the student, the supervisors, a pharmacist or pharmacologist and other members as required including for example a clinical psychologist, a psychiatrist, an emergency physician, a consumer or carer consultant, and wherever possible a nurse practitioner.

The Inter-professional support network should arrange a meeting schedule, which can comprise formal and informal meetings of the whole group where this is possible and one to one meetings with the student when considered necessary. Meetings can be conducted using available communication technology such as tele/videoconferencing, Skype, or similar. Members of the Inter-professional support network will have ongoing access to the Course Coordinators and program convenor for the duration of the program.

Forming and working with the Inter-professional support network will be discussed during Clinical Practice A.

INTEGRATED PROFESSIONAL PRACTICEIntegrated professional practice (IPP) provides a supported learning environment for the development of clinical skills and capability in episodes of care. Throughout the program students are required to gain 300 hours of supernumerary integrated professional practice (IPP). The student requires a range of health care experiences that supports knowledge and skills development in patient centred care that is consistent with the principles of primary health care and complements the student’s specialty skills and knowledge.

Students accumulate their 300 IPP hours including 20 supervision hours in the following way. IPP is attached to the two clinical courses, NURS6621 and NURS6622. Students complete a minimum of 220 hours IPP including 15 hours clinical supervision for NURS6621, and a minimum of 80 hours IPP including 5 hours clinical supervision for NURS6622.

YEAR 1, TRIMESTER 1 – CLINICAL PRACTICE A (NURS6621)The first course of the M NP is NURS6621 Clinical Practice A. NURS6621 requires students to complete the 220 hours of IPP over a period of 12 months. This is a hurdle requirement. Students are required to achieve a satisfactory clinical practice rating in the ANSAT NP in order to move forward to their final trimester and enroll in NURS6622 Clinical Practice B. Appendix 6. ANSAT Nurse Practitioner tool

YEAR 2, TRIMESTER 3 - CLINICAL PRACTICE B (NURS6622)For completion of this final course of the program, The ANSAT-NP is used as a comprehensive, summative assessment item to determine the required achievement of the NMBA Nurse Practitioner Standards for Practice 2014. This is a hurdle requirement; students complete 80 hours (2 weeks) of IPP, over the duration of NURS6622. On assessment of the 80 hours of IPP, the ANSAT-NP tool is reviewed by the course coordinator to confirm satisfactory clinical practice and achievement of the NMBA Nurse Practitioner Standards for Practice 2014 in order for the student to move forward in their final semester and present to the final assessment, the long case Viva Vice and then onto graduation. Appendix 6. ANSAT Nurse Practitioner tool

As described above, the enrolling student is required to provide a letter of approval from their employer and/or Head of Department and from any external organisations, dependent on where the student seeks to undertake their IPP, which includes documented confirmation of support of the student for IPP. A letter and form to complete by the employing organisation (or another identified organisation) to support IPP on enrolment is available in Appendix 3a and Appendix 3b. IPP can only be undertaken in the student’s chosen venues when a site suitability checklist has been completed by the student and the site is approved by The University of Newcastle (Appendix 4) and an agreement is in place which clearly outlines responsibilities of students, the University and the organisation prior to engaging in IPP within any facility, including the management of indemnity and insurance. Agreements are organised through the Program Convenor.

Please contact the Program Convenor for any questions about sites for IPP, site suitability and agreements.

The University of Newcastle Department of Rural Health, can provide information and contacts to students wishing to gain suitable rural and Aboriginal and Torres Strait Islander IPP experiences. Our program has contacts for sites that students can approach for IPP and Nurse Practitioners and Medical Practitioners who may be approached to be supervisors. The department of Rural Health provides student orientation, cultural awareness training and other supports, including linking students to suitable accommodation. We have facilities located in NSW at Tamworth, Taree, Armidale, Moree, Port Macquarie and Coffs Harbour. These areas can provide a range of suitable IPP experiences e.g. Aboriginal Medical Services, primary care and mental health services - giving students the opportunity to enjoy a unique lifestyle while completing IPP in minimum blocks of 2 weeks. The University of Newcastle Department of Rural Health will discuss the supports they can offer enrolled students to gain a range of IPP experiences during the first course. NURS6621 Clinical Practice A .

Please contact the Program Convenor if you wish more information or you can identify that you require to follow up on these opportunities in your enrollment application.

STUDENT VERIFICATIONPrior to engaging in IPP, all students enrolled at the University of Newcastle, regardless of location of IPP, require evidence for verification to practice e.g. police check, immunisation status etc., as advised on enrolment by student support and enrolment services. For details of verification go to:

newcastle.edu.au/about-uon/governance-and-leadership/faculties-and-schools/faculty-of-health-and-medicine/resources/for-students/student-placement-information/nsw-health-verification-requirements

Note: Current NSWHealth employee students can provide their staff link ID, to link their student profile with their healthlink profile to evidence their verification to practice. This will also show evidence of completion of the 5 mandatory training modules required before IPP can be conducted within a NSWHealth facility. Non NSWHealth students will also need to complete these online modules if they are intending to conduct IPP with a NSWHealth location. A link to these modules is automatically issued two weeks prior to such IPP placement once you have submitted all required verification documentation.

DIRECT AND INDIRECT CLINICAL SUPERVISION MODEL FOR IPPThe direct and indirect clinical supervision model for the M NP program is underpinned by an experiential learning framework. Essential to this framework is the Driscoll Reflective Model for clinical supervision which supports intentional reflection on practice for practice improvement. The aim of reflective practice is that the student will learn from these reflections and review their perspectives to act differently in the future.

Supervision of the Nurse Practitioner student can be direct or indirect. At all times, the M NP student remains accountable for their own actions and is responsible for all delegated functions.

Direct Supervision is when the supervisor is present to observe, work with and direct the student being supervised.

Indirect Supervision is when the supervisor does not observe the student’s activities. In these instances, the student is supervised by a senior clinician and the primary supervisor must be available for reasonable access. What is reasonable will depend on the context.

Example 1. The student may spend IPP time under direct supervision (supervisor present), in an acute admission center in the morning and then attends a group health promotion activity or home assessments with another staff member for the afternoon (e.g. an NP or medical officer). Supervision has been delegated. The student is now under indirect supervision. Arrangements can be made as the primary supervisor to be reasonably available e.g. via phone, skype or at a 1:1 supervision session later.

Example 2. The student may be undertaking IPP for a week in a clinic setting. The supervisor is only working for a short session (half a day) in this setting. Arrangements can be made as the primary supervisor to be reasonably available e.g. via phone, skype or at a 1:1 supervision session later. In this case arrangements are made that a senior clinician (e.g. an NP or medical officer) in the workplace is accessible to the student for temporary oversight, when the primary supervisor is not present This model is particularly applicable to student IPP, located in rural or remote settings, or where IPP across multiple sites or geographical distances.

To assist with the indirect and direct model of supervision. Students should select one or two cases under indirect IPP, to present at 1:1 supervision.

Appendix 5 provides a template used by the student, to prepare a case for clinical supervision.

It is the responsibility of the student to alert the Program Convener of the need to nominate alternate supervisor as required for periods of times of absence of the primary supervisor (e.g. supervisor on leave or IPP conducted in a different organisation).

The relationship between the student and the clinical supervisor is based on mutual trust and respect and whilst the discussion which takes place during the meetings is confidential, the supervisor is required to report unusual, illegal and/or unethical practice by the student disclosed during supervision. Avenues for reporting such practice will be discussed with the Course Coordinator in accordance with the National Law (NSW) as indicated above.

SONIA is the student placement management system. It is available as a web-based platform to students and their clinical supervisors. Students complete electronic time sheets in SONIA to record their IPP time and a clinical supervisor signs them off. SONIA keeps track of the IPP time recorded. The Course Coordinator and Program Convenor can check the hours completed.

Student and supervisor are also expected to keep a record of reflective Clinical Supervision, including the time and place and length of sessions. The student will log this in their excel Portfolio Logbook-Clinical Supervision and upload a time sheet of completed 1:1 clinical supervision signed by the supervisor to SONIA.

Appendix 5 will support this process.

More information on the model of supervision and IPP requirements is available on the “Nurse Practitioner Programs” Blackboard site

Page 9: ENROLMENT PACK School of Nursing and …...nursing discipline has been ranked in the top 100 in the QS World University Rankings by Subject in 2017 and 2018. The first Nurse Practitioner

REFLECTIVE CLINICAL SUPERVISION: MEETINGS AND RESPONSIBILITIESStudents require 20 hours of one on one reflective, clinical supervision as a program requirement. This supervision can be face-to- face, by phone or via Skype. The students 20 hours of supervision are attached to the two clinical courses. It is recommended that the students meet with at least one clinical supervisor each fortnight (face to face) for the duration of each of these courses. Clinical supervisor and student need to establish the framework for this supervisory relationship at the first meeting. Clinical supervision is encouraged over the entire length of the program of study e.g. monthly, when the student is not engaged in IPP.

Students are expected to prepare for each meeting. The clinical supervisor facilitates discussion and feedback. The student keeps notes for further reflection on key issues. Meetings should last for a minimum of one hour. Appendix 5 provides a template used by the student, to support and log evidence of reflective practice in clinical supervision.

ASSESSMENT OF SATISFACTORY CLINICAL PRACTICEANSAT NP

NURS6621 Clinical Practice A. The ANSAT NP tool is completed by the clinical supervisor in SONIA as a formative assessment of satisfactory practice, at 2 week (80 hour) intervals of completion of attached IPP. At the completion of the 220 hours of attached IPP the ANSAT NP is completed for the third time by the clinical supervisor in SONIA as a summative assessment. The completed ANSAT-NP tools are scanned into SONIA or completed as an electronic form directly into SONIA.

NURS6622 Clinical Practice B. The ANSAT NP is completed by the clinical supervisor in SONIA as summative evidence of satisfactory clinical practice and achievement against the NP standards and scanned into SONIA or completed as an electronic form directly into SONIA. The clinical supervisor should meet with the student in week 11 or week 12 of this last course to complete the ANSAT-NP. These arrangements are discussed during NURS6621 Clinical Practice A.

Appendix 6. The ANSAT NP tool is attached with descriptors. The ANSAT resource manual and tools are available on the Nurse Practitioner Program Blackboard site.

STUDENT PORTFOLIOThe excel student portfolio is an assessable item for the two clinical courses NURS6621 and NURS6622. The learning plan is completed at the beginning of each course. The student identifies their clinical goal/s and learning needs against the NP standards 2014 and may consult with their supervisors and course coordinators in developing their Learning Plan.

At the end of each IPP session (day, hours or minutes) the supervisor signs the time sheets in SONIA. At the end of each course the supervisor/s sign the outcomes of the students learning plan and the Supervision activities in the students excel Portfolio. Your excel Portfolio is available to you on the Nurse Practitioners Programs Blackboard site and instruction provided during NURS6621 Clinical Practice A.

The following items of the Portfolio are submitted by the student to Blackboard on completion of each of the courses (NURS6621 & NURS6622). In this way the portfolio enables the development of comprehensive evidence of practice that demonstrates the NMBA Nurse Practitioner Standards 2014.

1. Course Portfolio Learning Plan

2. Course Portfolio Logbook-Clinical Supervision

3. Course Portfolio – Outcomes of learning plan

The supervisors are the only people who have the authority to sign off on the ANSAT NP and the student portfolios. The clinical supervisor should meet with the student in week 11 or week 12 of the 12-week courses to review and sign the portfolio. It is the student’s responsibility to submit the portfolio of evidence to Blackboard. The student’s grade for the course cannot be finalised until the portfolio, signed by the supervisor, has been received by the course coordinator in Blackboard and meets the specified criteria.

Page 10: ENROLMENT PACK School of Nursing and …...nursing discipline has been ranked in the top 100 in the QS World University Rankings by Subject in 2017 and 2018. The first Nurse Practitioner

UNSATISFACTORY CLINICAL PERFORMANCEWhere unsatisfactory clinical practice is identified on the ANSAT-NP, the clinical supervisor will notify the student and the Course Coordinator and or Program Convenor. The ANSAT-NP tool will form the basis of the practice development plan that is developed by the clinical supervisor/s in consultation with the student and the course coordinator, to address any unsatisfactory practice issues.

See Appendix 7 Practice Development Plan.

In consultation with the student and the Course Coordinator, the clinical supervisor should develop a practice development plan following the guidelines given below:

• Detail the major area(s) of unsatisfactory clinical performance (identified in the ANSAT-NP tool).

• Specify the aims to demonstrate satisfactory performance in the area(s) identified.

• Develop a range of methods/activities/resources which student can use to achieve the aims.

• If the student and/or clinical supervisor have reason to believe they cannot work together in the preparation and execution of the plan, the clinical supervisor or student will contact the Course Coordinator and or Program Convenor to discuss alternative arrangements.

• Review date/s for the plan will be set and feedback on progress given to the student and course coordinator by the supervisor/s.

• Where the student has not been able to meet the aims identified in the practice development plan and clinical practice remains unsatisfactory, the clinical supervisor consults with the Course Coordinator before any unsatisfactory IPP assessment is determined.

• The University of Newcastle Student support services will be offered to the student by the Course Coordinator.

• Practice improvement plans will be incorporated into future IPP as required.

SUMMATIVE ASSESSMENT - FINAL VIVA VOCEAs a requirement of the nurse practitioner program, the ‘long case’ Viva voce model is used as a summative assessment. It seeks to examine the student’s ability to apply knowledge in an actual clinical situation (ANMAC2015). NURS6622 Clinical Practice B a capstone and final course for the program with students being required to complete a final summative viva demonstrating achievement of the NP standards for practice 2014 by undertaking a long case viva voce examination in defense of their submitted case study. The examination requires the student to use professional communications skills to collect, analyse, synthesise and evaluate clinical information, to use differential diagnostic procedures and determine a management plan. It assesses learning outcomes related to graduate attributes of deep learning, application and synthesis of knowledge and high-level clinical reasoning, knowledge, rationales, attitudes and values underpinning the case study. The expert panels engaged for viva voce examinations consist of representatives from the following three categories:

• Chair: An appropriately qualified and experienced academic nurse from the School of Nursing and Midwifery. This panel member is also the Academic Examiner;

• A pharmacy or pharmacology examiner; this panel member is the Pharmacology Examiner;

• A nurse practitioner or medical practitioner with appropriate specialty expertise and qualifications. This panel member is the Clinical Expert Examiner.

SUPPORT OF SUPERVISORSClinical supervisors are provided with a Supervisors Handbook detailing the IPP and clinical supervision requirements of the M NP program. Clinical supervisors are encouraged to contact the Course Coordinator or Program Convener with questions or concerns regarding the clinical supervision process and requirements or further resources to support the role.

A supervisor orientation meeting will be held at the beginning of the first year of study for the student (TBA). Supervisors can attend face to face or be linked to via Skype technology.

Conjoint positions and University Affiliates are provided a University of Newcastle email address, access to staff resources and the Supervisors for M NP programs Blackboard site. This site will include resources for supervisors and enable supervisors to join quarterly supervisor collaborate meetings with the program convenor and course coordinators (Skype type technology hosted within Blackboard). Supervisors without conjoint positions will have opportunity to join quarterly Zoom/Skype supervisor meetings with the program convenor and course coordinators and receive additional resources via email.

CONTACTING THE PROGRAM CONVENORPlease do not hesitate to clarify enrolment requirements with the program convenor.

Students in remote locations are advised to contact the program convenor prior to completing their application to discuss the completion of the site suitability checklist, including access to supervision and suitable IPP experiences, access to academics and the logistics of attending the residential workshop in person or online.

On successful enrolment to the program, please contact the program convenor via email and arrange a time to introduce yourself and discuss your study, supervision and IPP plans.

APPENDIX 3A ORGANISATIONAL APPROVAL LETTERSCHOOL OF NURSING AND MIDWIFERY FACULTY OF HEALTH AND MEDICINE, UNIVERSITY OF NEWCASTLE

University Drive, CALLAGHAN, NSW 2308

Dear Colleague,

To enrol in the Master of Nurse Practitioner, the University of Newcastle requires the applicant to provide a letter of support (attached to this letter) for integrated professional practice (IPP) from the Director of Nursing (DON) or equivalent head of department / senior clinical manager of the organisation.

Under the ANMAC Nurse Practitioner Accreditation Standards 2015, students are required to undertake a minimum of 300 hours supernumerary integrated professional practice (IPP) as part of the program requirements, spread across the time of study. IPP can be undertaken during the program in 3 ways.

1. Undertaking scheduled and supervised integrated professional practice in the clinical setting where they are employed, but not rostered at that time. The service provided in the healthcare setting could continue to be delivered without the nurse practitioner student’s presence.

2. Undertaking unscheduled, opportunistic and supervised integrated professional practice in the clinical setting where they are employed and counted in the roster of the clinical setting. In this circumstance the normal provision of rostered duty care cannot be compromised.

3. Undertaking supervised practice in a clinical setting where they are not employed.

Successful entry into the program requires students to provide documented evidence that the employing organisation and or other organisations will support the student to complete the professional experience requirements of the program. The student requires a range of health care experiences that supports knowledge and skills development in patient centred care that is consistent with the principles of primary health care and complements the student’s speciality skills and knowledge. Apart from the student’s regular place of work, employers may provide the student with opportunities to gain IPP across a range of clinical experiences.

Two clinical supervisors are to be nominated at the time of application and acknowledged by the DON/manager. The clinical supervisors must be:

1. An endorsed nurse practitioner (NP). If an endorsed NP is unavailable then a senior clinical nurse working in the same field as the student, with advanced practice knowledge and skills and a recognised qualification at a minimum level of a master’s degree or fellowship of a professional college or equivalent combination of formal education and clinical experience.

2. The second clinical supervisor should be a medical practitioner. Where a medical practitioner is not available then a health practitioner working in the same field as the student, with advanced practice knowledge and skills and a recognised qualification at a minimum level of a master’s degree or

FELLOWSHIP OF A PROFESSIONAL COLLEGE OR EQUIVALENT COMBINATION OF FORMAL EDUCATION AND CLINICAL EXPERIENCE.One on one supervision and assessment of satisfactory clinical practice are completed regularly throughout the program by the clinical supervisors with the student. Students and supervisors are supported by The University of Newcastle, via the program convenor and course coordinators and University resources. Successful completion of the program requires the student provide evidence of competency achievement against the NMBA Nurse Practitioner Standards for Practice (NMBA 2014).

Please provide the letter of support on the template that follows and complete the forms on the following pages. Please nominate a contact person at your organisation for the program convenor to discuss a student placement agreement or similar if one does not currently exist and IPP requirements, before any IPP can be conducted by the student in your workplace.

Thank you for agreeing to support this student as they embark on this program of study. I look forward to a collegial working relationship between the university and your healthcare facility.

If there are any questions, please do not hesitate to contact me via email or phone (email or mobile number below is best).Yours sincerely,

Dr Melissa Robinson-Reilly
