which enables patient self measurement and remote
patient monitoring.
The example on this page shows the integration of an
activity tracker with an app. The data that is generated by
the patient is then synchronized with the provider portal.
This data is input for rules that will assign patient tasks
and/or trigger alerts on the provider side. This is one of
the many remote monitoring care paths that are
