You must complete this step before you can move on. Employees and covered spouses: Obtain your biometric measurements from your annual preventive exam. Employees may participate in one of the limited biometric screening events across the city. Covered spouses must go to the doctor. Your biometric measurements must be from February 1, 2017, or later. You need up-to-date measurements for your:

• Blood pressure• Total and HDL cholesterol• Height• Weight • Waist circumference

STEP 2 - Online health assessment

You must complete this step before you can move on.Employees and covered spouses: Use your up-to-date measurements to complete the online health assessment at mycigna.com by November 30, 2017. Note: Spouses must complete the health assessment on their own mycigna.com account; if they do not have a mycigna.com account, they will need to create one. TIP: Be sure to use biometrics from February 1, 2017, or later. Don’t choose answers like “I don’t know” for your biometrics; answer all the questions.

All medical services must be with a Cigna healthcare professional and will be automatically reported through a claims submission, except where noted as self-reported.

Coaches can counsel you in weight loss, smoking cessation, medication adherence, disease management and many other health concerns. Option C: Onsite Departmental Programs (Available for employees only)Average required completion time: 6 weeks Available: August 1, 2017 – March 9, 2018 Actions: Attend a weekly Onsite Lifestyle Management Program for six weeks. Topics include diabetes prevention/management, cardiovascular disease prevention/management and general prevention. Option D: Fitness Tracker (Cigna Apps and Activities)Average required completion time: 3-4 months Available: August 1, 2017 – March 9, 2018 Actions: Track physical activity, diet and health goals using a fitness tracker device such as a Fitbit or Up by Jawbone, or app such as My Fitness Pal, Map My Fitness, RunKeeper and more. Apps and devices from an approved list can be integrated with Cigna’s Apps & Activities portal to complete various goals and challenges such as tracking your sleep, taking 10,000 steps or testing your blood glucose. Each goal and challenge earns varying amounts of points depending on the level of difficulty. You must earn a total of 1,000 points.

Troubleshooting

Who is eligible to earn the discount? Active employees and their covered spouses are eligible for the incentives. Retirees and dependents may participate in the program but are not eligible to earn the medical rate discounts.

How do I earn the discount? Earning the discount for active employees is easy. Just correctly complete your online health assessment by November 30, 2017. Then complete one of four wellness engagement options by the corresponding due date, and you can earn the $600 medical rate discount for active employees.

How does my spouse earn a discount? To earn the $300 discount, your covered spouse must complete the online health assessment on their own mycigna.com account (not the employee’s mycigna.com account) by November 30, 2017, as well as complete one of the three available wellness options by March 9, 2018. To earn the $300 discount, your covered spouse must complete the online health assessment on their own mycigna.com account (not the employee’s mycigna.com account) by November 30, 2017, as well as complete one of the three available wellness options by March 9, 2018.

What is the maximum discount I can earn? Employees in the employee only or employee + children tier can earn a $600 annual medical rate discount. Employees earn an additional $300 discount if their covered spouse completes the health assessment and one wellness option. Employees in the employee + spouse or employee + family tier can earn up to a $900 annual medical rate discount.

If I don’t take the online health assessment, can I still participate in the Wellness program and earn my discount? No. The online health assessment is a prerequisite to Wellness participation. If you do not complete the health assessment by November 30, 2017, you are not eligible to earn a medical rate discount.

Why does Option A have a different ending date? Preventive care activities take longer to report, so those activities need to be completed by February 2, 2018, to be included.

I just started working at the City of Houston. Am I required to take the health assessment? Employees with benefits effective on or after September 1, 2017, are exempt from taking the online health assessment, and will be eligible for the annual discounted rates.

I’m due for another annual physical after the health assessment period ends November 30, 2017. How should I get my biometric measurements? As in previous years, free, confidential biometric screenings will be available at work sites throughout the city, but appointments will be limited this year. Be sure to make an appointment as soon as they become available. You can also make an appointment with your doctor and pay a copay or coinsurance to get your measurements.

How do I know when I have completed the requirements to earn a discount? When you log in to MyCigna.com, you and your covered spouse will see a percentage completed toward your goal. Earn 100 percent by completing the health assessment (10 percent) and wellness engagement options (90 percent) by the deadlines to earn up to a $900 discount on your 2018 medical plan rate. Employees with a covered spouse on their plan will need to earn 200 percent (100 percent for you, the active employee, and 100 percent for your spouse). Allow at least 60-90 days for preventive care activities to be auto-reported. You must self-report dental or vision exams, Healthy Pregnancy Program (Powered by ComPsych), Vaccinations (Shingles, Tetanus, Varicella, Hepatitis A and B, and/or Pneumonia), Introductory Onsite Nutrition Coaching visit and EAP Coaching visit.

Who is exempt from participation in wellness program?

• Employees on FMLA (August 1 - November 30, 2017)

• Employees out on Workers Compensation (August 1 - November 30, 2017)

• Employees out on Military Leave (August 1 – November 30, 2017)
