Rehabilitation Services
• provides comprehensive medical and vocational rehabilitation for HBT member employees on Long-Term Disability (LTD);
AND…
• supports the employer in the delivery of the collectively bargained Early Intervention and Enhanced Disability Management Programs (EIP & EDMP)

What is new about referral process?
• employer is responsible for initiating process of obtaining employee’s authorization
• employer remains responsible if the employee does not provide authorization

Process change facilitates earlier …
• employer access to medical and vocational rehabilitation support for their employee
• employer management of their employee’s absence if their employee decides not to engage with HBT Rehabilitation Services
• gathering and sharing of information by HBT

Emphasis on “Early Referral”
• rehabilitation can start any time after the employee and their health care provider agree it is medically appropriate
• the earlier rehabilitation starts…the better
conversion from short-term absence to long-term disability rises sharply after 45 days

Referral criteria
• Rehabilitation Services are available to employees:
– enrolled in the Healthcare Benefit Trust's LTD Plan
– with a medical issue that could lead to an LTD claim

Referral criteria
• Referrals are appropriate if medical or vocational rehabilitation can assist to:
– shorten or avoid sick leave that has the potential for an LTD claim
– prevent an LTD claim
– reduce the duration of an LTD claim

Referral form
• employee can use this form for self-referral and submit by email, fax or mail• HBT will contact
employer when form received
• employer can use this form or continue with spreadsheet currently in use• Submit by email or
fax

Authorization
• employee must complete this form and submit by email, fax or mail
• required prior to HBT engagement with the employee
Referral by Email Pop-up Message
• Desktop Email Application(Outlook/Outlook Express etc.) • an email addressed to rehabilitation
services will automatically open
• your completed form or authorization will be automatically attached
• Internet Email (Hotmail/Gmail etc.)• save the completed form or
authorization onto your computer
• attach this form to an email and send to [email protected]
