June 19th, 2020 WELLNESS BENEFIT REFUND PROGRAM These last few months have been tricky and definitely impacted both our work and home life. We have had to cancel several events, including the Employee Safety and Wellness Fair. We are currently working on a program through PowerDMS where employees who want to earn the wellness point for a Safety and Wellness Fair can log in and read some materials and/or watch videos. Several of the vendors that planned to participate at this event sent in information we could share. Below are all the activities you can earn points for and some other options: 1. Bio-screening on campus (held in October) 2. Flu shot (if off campus must provide proof) 3. Non-tobacco user or complete a cessation program (must provide proof) Form on-line under the employee portal 4. Preventative Health Screening (form to be completed by Healthcare provider and submitted to DirectPath) Form on-line under portal 5. 110 visits to our wellness center 6. Health Coaching with DirectPath 7. 2 community/charity fitness events (walking, running, bicycling) 8. Safety/Wellness fair attendance 9. Participate in Community/Church league sports. Must be able to show proof of enrollment in the league (ex. Parks & Recreation, Boys & Girls Club, Corvallis Sports Park, Organized Church Leagues). 10. Meet with a financial advisor 11. Meet with a nutritionist or Registered Dietician 12. Walk or Bike to work 108 times in the benefit plan year (Oct-Sept) must complete punch card and submit to HR I know some community and charity events have been cancelled. Once you have signed up you just bring us a copy of your registration to HR for credit. If you have completed #10 or #11 just have the person you met with sign something that stated the date they spoke to you and turn in to HR. If you are walking on breaks/lunches you can pick up forms to track your participation or sign in with the front desk in your work area. 5 points = $360 refund and 3 points = $180. Participation is not mandatory just a way to earn back some of the plan cost. Special points of interest: Oregon Governor issues face covering mandate Open Enrollment will be coming soon.
