Dear FPU Community,

At Fresno Pacific University, we are committed to a comprehensive employee benefit program that helps employees stay healthy, feel secure, and maintain a healthy work/life balance. Our goal is to provide you with coverage that is rich in benefits, at affordable prices.

Open enrollment is the only time you can change the following benefits (unless you have a “qualifying event” like getting married or having a baby):

Medical, vision and dental plans Flexible spending accounts Supplemental Term Life Voluntary Insurance

This year’s Open Enrollment period will run from May 11th, 2020 through June 12th, 2020 at 5:00 pm. It is mandatory to review your benefits each year. If you are happy with your benefits you still need to approve them for the new benefit year and, remember that your flexible spending account does not automatically renew – you must re-enroll every year. All changes are made via MyWorkplace. If you wish to decline benefits, you must log in to MyWorkplace. This is necessary due to the Affordable Care Act (ACA) requirements and will affect your 1095-C tax forms prepared by Fresno Pacific University.

Be sure when you log into MyWorkplace to check your personal information. Make sure your dependents are listed correctly and that your levels of benefit coverage align properly to the new benefit year. It is important to verify your home address, preferred email address, and other personal information as well. You may return and make changes as many times as you want in MyWorkplace until open enrollment closes on Friday, June 12th, 2020 at 5:00 pm. *All enrollments must be completed no later than June 12th, 2020 by 5:00 pm. There will be no exceptions – extensions will NOT be granted this year.

Log In Instructions:

Log into www.MyWorkplace.net Select New User Registration or Returning Users button Enter your Email and Password to sign in. Select Open Enrollment Use the menu options to navigate through the Open Enrollment Benefit Portal. You will be

making elections for each of the benefit categories available in your benefits package. Once you have signed in, you can: Enroll or remove a family member from coverage You should add new family members before making changes to your plans. Select the ‘Manage My Beneficiaries’ and fill in the dependent information (legal name, SSN, Date of Birth). Then click on the Add button. To enroll or remove family members, you can choose the specific plan menu portion. Select the appropriate box. A check mark indicates the change is complete. Enroll or make a plan change Select the button for the plan you want to change. Then, enter the necessary information or select the appropriate box. When you have made all the changes for that plan, select the button for a different plan to keep, modify, or waive. Review and Checkout When you enrolled, declined, or approved each benefit option, then the button will display the word Completed with a check mark. The Checkout button becomes available when you have completed all required benefit options. Your selections are not finalized until you have clicked on the Checkout button.

IMPORTANT: Once you have clicked on the Checkout button, the system will ask for your signature. If you are on a desktop computer, you may use your mouse to sign. If you are on a laptop, you may use your finger to sign. Then click on SUBMIT, this will complete your open enrollment process. (If you do not click on SUBMIT, after signing, the process is NOT complete.) Confirmation Statement You will receive an email confirming the completion of your online benefits enrollment through MyWorkplace If you make modifications within a category after clicking SUBMIT, you will need to Checkout again and click on SUBMIT again to finalize your most recent requests. This year’s Open Enrollment period will run from May 11th, 2020 through June 12th, 2020 at 5:00 pm. It is mandatory to review your benefits each year. Who is Eligible for insurance?

If you are a full-time employee (working 30 or more hours per week) you are eligible for medical, dental, vision, life, voluntary life and voluntary LTD insurance the 1st of the month following 30 days of continuous employment. You are eligible to enroll in the Fresno Pacific University Employee Benefit Plan, which provides group healthcare coverage that is both affordable and meets the minimum value as required by the Affordable Care Act (ACA). These benefits are available to you and your eligible dependents. Information regarding the ACA and the Health Insurance Marketplace is available at www.healthcare.gov. If you do NOT want a University medical plan, be sure to indicate in MyWorkplace that you are waiving all offered medical insurance. Seasonal and Temporary employees are not eligible to enroll.

Dependent children are covered as follows: Up to age 26 regardless of student status for Medical, Dental & Vision.

Open Enrollment All eligible employees must login to MyWorkplace to change, opt out or enroll in current or new elections. Online enrollment is open from May 11th, 2020 through June 12th, 2020. Elections and/or changes become effective July 1st, 2020. You must visit www.MyWorkplace no later than June 12th, 2020. Otherwise, you will have to wait until next year’s open enrollment to make changes (unless you experience a qualifying event in status per IRS Code 125). To enroll online, you can register by going to www.MyWorkplace.net . If you are enrolling in the Voluntary Life through Met Life, and request an amount over the guarantee issue amount, you will be emailed an Evidence of Insurability (EOI) form to complete; this must be submitted to Met Life directly within 24 hours from receipt via mail, email or fax. Met Life’s contact information will be provided to you in that same email. (The EOI form along with other documents and forms will be available in the Benefits Information Section.)

Even if you are waiving medical, dental and/or vision benefits it is still required that you go online and waive benefits. You may also take this opportunity to update your beneficiary for the life insurance that Fresno Pacific University pays for on your behalf.

Changes Outside of Open Enrollment

Qualified changes must be made in writing within 31 days of a change in status, including: marriage, divorce, legal separation, birth or adoption of a child, change in child’s dependent status, death of spouse, child or other qualified dependent, change in residence due to an employment transfer for you, your spouse, commencement or termination of adoption proceedings, or change in spouse’s employment status. Please contact the Human Resources Department for the applicable forms(s) to complete for qualifying event changes.

Direct Path / Health Advocate – 1.866.253.2273 For medical insurance issues, please contact a Health Care Advocate at Direct Path. Direct Path can assist with things like resolving claim and billing issues, help to find you a contracted doctor and answer questions about your benefits. They can also assist you with finding the right provider/facility should you need services. Contact them prior to scheduling any health care procedure or test. Your health advocate will contact up to three contracted providers/facilities to compare the cost and quality information. That way you are informed and can make the best choice for you. Plus if you choose the provider that is the lower cost option, you may be eligible for a reward of 20% of the plan savings (up to $500). Please see more information noted in this guide.

How much does it cost me? LiveHealth Online is FREE! Take advantage of this great benefit at no cost to you.

How does it work? LiveHealth Online lets you talk face to face with a doctor through your mobile device or a computer with a webcam.

No appointment necessary You don’t have to wait in a waiting room to see a doctor You don’t have to drive anywhere Use LiveHealth Online for common health concerns such as, colds, the flu, fevers, rashes, infections,

allergies and more! It’s faster, easier and more convenient than a visit to an urgent care center A typical virtual visit lasts about 10 minutes Doctors may call in a prescription to a 24 hour pharmacy

How do I sign up? To use a mobile device, search for LiveHealth Online in the App Store or on Google Play. To learn what mobile devices are supported and to get instructions, go to livehealthonline.com and select Frequently Asked Questions under the How it Works tab. Customer Support is available over the phone at 1.855.603.7985.

You will select your state you are located in and answer a few questions. Establishing an account allows you to securely store your personal and health information. Plus you can easily connect with doctors in the future, share your health history and schedule online visits at times that fit your schedule. Once connected, you can talk and interact with the doctor as if you were in a private room.

Optum Rx is Fresno Pacific University’s Prescription Drug Vendor. They work hand in hand with our medical plan. Our copayment amounts will remain the same for this plan year.

Go Mobile with Optum Rx’s App! This is available to assist you with your prescriptions needs on the go. The App is available to members with an iPhone, iPad, or Android smartphone/tablet. For assistance, please call customer service at 800.334.8134. Some of the offered features are as follows:

Fresno Pacific University’s Prescription plan also allows for Mail Order Prescriptions to be sent directly to your home through Optum Home Delivery. This allows you to save money by getting a larger amount of prescriptions at a lower cost.

You will need to complete the patient profile form and mail it in to Optum Home Delivery along with your maintenance prescription(s) written for a 90-day supply from your physician (please see your Human Resources Department for this form).

It generally takes 10-14 business days to receive your order. Once you have submitted your initial order and you need to refill your prescriptions, you may simply log into the website on the back of your ID card. Please be sure to have the Optum Home Delivery prescription number and date of birth of the patient handy. You may also call their customer service team at 800.334.8134.

Fresno Pacific University provides full-time employees with the opportunity to purchase additional Life and AD&D Insurance for themselves as well as their eligible dependents through Met Life. Please be sure to keep your beneficiary information updated in MyWorkplace.

For any amount requested over the Guarantee Issue Amounts listed above, you will need to complete the Evidence of Insurability (EOI) Form. This will be emailed to you via MyWorkplace. If you do not receive the EOI form, please reach out to your Human Resources Department. Please see MyWorkplace for the applicable rates & costs associated with these benefits. IMPORTANT NOTE: If you did not enroll in the Voluntary Life and AD&D benefits when you were first eligible, you may enroll during open enrollment. You will have to complete the Evidence of Insurability (EOI) form for the amounts you elect. You will submit the EOI form directly to Met Life and their underwriting team will make a determination as to whether they grant or deny your request for coverage.

Fresno Pacific University provides benefit eligible employees the opportunity to purchase long-term disability insurance coverage. When you enroll yourself in this benefit, you pay the full cost through payroll deductions. In the event you become disabled from a non work-related injury or sickness and unable to perform your own occupation, disability income benefits are provided as a source of income.

Health Care Reimbursement FSA (Maximum annual election: $2750) Please go to https://intranet.fresno.edu/human-resources/fpu-employee-benefits for a listing of Eligible Medical Expenses, visit Flex Today’s website for any questions at www.FlexToday.com or call their Customer Service number at 1.800.995.5373. This program allows employees to pay for certain IRS-approved medical care expenses not covered by their insurance plan with pre-tax dollars. Some examples include:

Employees may elect to receive an optional debit card to use towards qualified medical expenses. The Med-FSA CARRYOVER allows up to $500 of your Med-FSA balance remaining AFTER the claims runoff per ends to CARRYOVER to the next plan year. If you are an active employee and remain eligible to participate in the Flex Plan, up to $500 of your Med-FSA balance from the prior plan year will CARRYOVER to be added to the top of whatever you have elected for the Med-FSA in the new plan year. The new CARRYOVER does not reduce your election for the new plan year; it will be added to the top of your new year election. The CARRYOVER itself will occur several weeks AFTER the March 31st claims runoff period has ended to allow for any last minute claims.

Use it or lose it rule: As required by the Internal Revenue Service (IRS), an FSA has a “use it or lose it” provision stating that any unused funds at the end of the plan year will be forfeited. When electing an FSA during open enrollment, the employee must specify how much they would like to contribute to their FSA for the year. The goal is choosing an amount that will cover all medical expenses, but that is not so high that the money will be forfeited and wasted. NOTE: Please visit www.fsastore.com for a variety of items you may use your FSA dollars on if you have money at the end of the plan year and need to spend it.

Dependent Care FSA

(Maximum annual election: $5000) The Dependent Care FSA lets employees use pre-tax dollars towards qualified dependent care such as caring for children under age 13 or caring for elders. The annual maximum amount you may contribute to the Dependent Care FSA is $5,000 (or $2,500 if married and filing separately) per calendar year. Examples include:

FITNESS FIRST: EXERCISE AND 

HEALTHY EATING In addition to trimming your waistline, regular exercise 

and nutritious eating will help you feel better, think 

clearly and live a longer, healthier life. 

Start Sensibly 

Don’t begin your exercise program too ambitiously. The 

key to success is to start slowly and increase the 

difficulty of your workouts as you become more fit. 

Those who overdo it often experience muscle soreness, 

become discouraged and quit. Rather than trying to run 

three miles on your first day, begin by running a mile 

and increasing your distance as your fitness level 

improves. Most importantly, remember that feeling 

dizzy or ill is your body’s way of telling you that you are 

working too hard. If this happens, take a break or stop 

your workout for the day. 

Find the Right Pace 

Exercise should be fairly comfortable for you. Your pace 

should be just below the point at which you start to 

breathe quickly. Exercising at this pace produces two 

desirable results: it mobilizes fat burning and helps you 

develop endurance. This means that for maximum fat 

burning, longer, slower exercise is more beneficial than 

short, strenuous workouts. If you are reasonably fit and 

exercising at the proper pace, you should burn between 

400 and 600 calories per hour during any aerobic 

exercise. This includes riding a stationary bicycle, 

walking or running on a treadmill or using a stair 

climber. 

Counting Calories Means Trimming the Fat 

The media is full of varying reports on how to lose or 

maintain weight. It’s no wonder that you may be 

confused about what foods to eat and what to avoid.  

Most experts agree that eating a well‐balanced diet 

low in fat is the key to losing weight. Since fat contains 

more than twice the calories of carbohydrates or 

protein, high‐fat food equates to higher calories. While 

lowering your fat intake is important, it is also 

important to monitor your overall caloric intake. Your 

ideal caloric intake depends on your age, body size and 

level of activity. Generally, women ages 23 to 50 need 

an average of 2,000 calories per day, while men in the 

same age group require about 2,400 calories per day.

VOLUNTEER WORK 

In addition to boosting your social well‐being, 

volunteering can make you feel physically stronger, 

increase mental sharpness, aid in reducing stress and 

even help you live longer. Despite these proven positive 

effects, only 1 in 4 Americans regularly volunteer, 

according to the Bureau of Labor Statistics. If you’re 

curious about the specific benefits of volunteering or 

how to get started, read on.  

Health Benefits of Volunteering 

As previously mentioned, volunteering has proven 

health benefits. Here are just a few of those benefits: 

Increased mental and physical activity—

Participating in volunteer activities gives you the 

opportunity to move and think in different ways 

than you’re used to doing at home or at work. 

And, there’s a good chance that you’ll be on the 

move while volunteering, which increases your 

overall physical activity level, too.  

Reduced stress levels—Giving back to those in 

need is said to enhance one’s sense of 

belonging, meaning and appreciation, which are 

all elements that can help reduce your stress 

levels.  

Increased social well‐being—By going out and 

volunteering in your community, you’re 

broadening your social circle, forging new 

relationships and giving back to those who are 

in need. All of these activities can increase your 

social well‐being, a component that plays a 

large role in your overall health.  

Volunteers may work full time, part time or be retired. 

They may volunteer with one organization a few hours 

a week, or with multiple organizations several hours a 

week. No matter what amount of time that is 

contributed, studies show that volunteers have an 

overall healthier, happier outlook on life. 

How to Get Started 

Many organizations can benefit from your volunteer 

time. A good place to start looking is within your 

community. Pick one that complements your talents, 

interests, schedule and physical abilities. When you’re 

thinking about what you’d like to do as a volunteer, it 

may be helpful to think about what’s motivating you. 

Some volunteers are motivated by the following: 

Helping others 

Learning about the community 

Meeting new and interesting people 

Working in a team environment 

Feeling like they made a difference 

Gaining valuable life experiences 

Acquiring new skills

VOLUNTEER WORK 

  Once you’ve determined what’s motivating you to 

volunteer, you’ll be able to narrow down your search for 

local volunteer programs. For example, if you enjoy 

helping others, it may be beneficial to try to volunteer at 

a hospital or clinic. If you want to learn more about your 

community, volunteer on a community council or 

committee.  

You can browse online listings or a newspaper to 

identify volunteer programs. To ensure a successful 

outcome for both you and the volunteer program, ask 

yourself the following questions: 

When can I begin? 

How much time can I commit? 

How often do I want to participate? 

What skills can I offer? 

What skills can I gain? 

What would I not want to do as a volunteer? 

What community issue do I want to affect 

through my volunteer service? 

What group or population do I want to serve?
