empanelmenr of hospitals and reimbursement ot' medical expenditure incurred by the Haryana

Govemment employees/pensioners and their dependents, as follows;

It is nrede c[ear that the eost of implants wril be reimbursed ,rnly to the exteilr of the costspecif"red in rhe Arrnexure of this policy as amended from time to time or the actual cost.whichever is less' In case the actual cost of impiant is more than the cost specified in the'Annexure of this p$licy, then the excess expencliture will be borne by the patient himself/herself.9' within next 2 months comprehensive package rates for major fifteen procedures will alsobe prescribed. A srfplementary notification in this regard will be issued within next 2 months.10' Sub Lirnits for room tariff depending on the category of employee will be fixed andnptified separ#ly.

efirployees, hcweve]', relaxation in the policy can be ganted in exceptional cases with the priorapprorlal of th'E Hon'ble Chief Minister and Finaace Department. Such relaxation can be grantedin' the eonditio*s $k{ treatment of a very old person, highly sensitive nature of diseases involvingmulti-system dlisord{r and treatments taken in life threatening emergency situation etc.Jqstification fcrr srrc$ expenditure, however, sh.rll have to be first obtained from a Committee ofexp€rts heded by DFHS and two subject specialists posted in panchkula or pGIMS Rohtak.r z . This $olicy will casrc inro force with imrnediatre effect.
