1. Code Brown - Introduction

An external emergency is defined as a multi-casualty incident that stretches oroverwhelms the available health resources, e.g. bus crash, aircraft crash, structuralcollapse, explosion.

The provision of emergency health services shall be based on the principle of agraduated response. The response should start at a local level with a gradualescalation to district and state (Western Australian) level as required with time.

2. Administration and Governance

The Warren Health Service, Code Brown Emergency Response Procedure has beendeveloped and issued under the authority of the WACHS-SW Regional Director inaccordance with applicable legal and other requirements.

These Emergency Response Procedures have been formally recommended by theWarren Health Service District Manager, endorsed by the WACHS-SW InlandOperations Manager and approved by the WACHS-SW Regional Director as theoperating procedures to be followed in a Code Brown at Warren Health Service.

3. Distribution and AvailabilityThe current editable version of this Code Brown Emergency Response Procedure isheld by the WACHS Policy Unit. The procedures are available electronically to all staffvia the WA health system wide intranet portal – HealthPoint.

Hard copies of the Warren Health Service Code Brown Emergency Procedures areheld in the Emergency Response Manuals located in the areas described below.

4. Incident Notification

· When a staff member receives any outside call pertaining to an externalemergency, the staff member should complete the “Notification of a MajorIncident” Form (Appendix 15) to gain necessary information:

- Caller’s name, position, location, service and telephone number- Exact location of the incident- Type of Incident (e.g. road, rail, air incident)- Hazards that are present and anticipated- Access road for emergency vehicles to use- Number and severity of casualties (may be a rough estimate)- Emergency services that are already present, and those that are required.

· If the emergency is nearby, the sudden influx of patients may be the first indicationof a mass casualty event.

· Verify the call, by contacting the organisation that the caller is representing.· The information must be immediately conveyed to 000 and the Senior Nurse on Duty

who becomes the Emergency Controller (EC) and the District Manager who becomesthe Local Health Disaster Coordinator (LHDC).

· The Senior Nurse is required to carry the Wi-Fi phone Ext 5235 at all times and takethe role of EC until a more senior staff member arrives.

· If notification of the external disaster comes from the Bunbury Hospital dutyofficer, The “Notification of a Major Incident” Form is to be filled in and the SeniorNurse on Duty must be notified.

· The staff member assuming the role of EC must notify the CNM, DM and SouthWest Executive on-call 1800 744 059 .

· If first notification is received at Warren Health Service, the EC must complete theCETHANE form and phone it to Bunbury on 97221262 and fax it to the BunburyED at 97221019.

5. Activation of Code Brown Emergency ResponseThe Code Brown Emergency Response can be activated by several means:

· By the EC / LHDC with the impending arrival of three or more seriously ill orinjured people. (or less if the hospitals internal situation means that they are lessable to compensate for the external emergency).

· When directed by the Director General (DG) or nominated delegate.· On receipt of credible information relating to mass casualties e.g. St John

Ambulance.· The Regional Health Disaster Coordinator (RHDC) has authority to activate a

regional wide emergency response.

5.1 Stages of Activation

While an emergency response would normally be activated in stages, in an impact event’ these stages may be condensed with stages being activated concurrently.

Phase 1 – Alert (There is a possible Emergency which is being confirmed)

The alert phase is activated when advice of an impending or potential emergency is received or when following the occurrence of an event, it is unclear as to whether a health response is required. During this stage the situation is monitored by the LHDC to determine the likelihood and nature of the health emergency response and resource requirements assessed.

Actions: · The EC liaises with effected services to determine the extent of the health

response required.· The EC alerts own personnel, placing site on Code Brown – Alert.· Follow local escalation process to inform CNM, District Manager, Operations

Manager and RHDC including after hours.· Review Medical and Nursing staff availability.· Review Warren Health Service bed availability and identify patients for discharge

or transfer.· Preparation of the Emergency Department.

Phase 2 – Standby (the emergency is imminent, prepare for a response) Standby is activated when information received is sufficient to warrant preparatory activities in readiness for a response. Actions:

· The EC alerts own personnel, placing site on Code Brown - Standby, hospital begins to prepare resources and receive patients.

· Follow local escalation process to inform CNM, District Manager, Operations Manager and RHDC including after hours.

· Preparations may include standby for the most appropriately located Health Response Team (HRT) (MIMMS Trained).

· It should be recognised that medical and nursing resources at WHS may be best utilised in the ED rather than deploying essential staff to the site of the incident.

· Health Response Team prepares Disaster Response Kits including medications and communication devices such as Satellite phones..

Phase 3 - Response (the emergency exists and a response is required) Response phase is activated when a health emergency management response is required and resources are deployed accordingly. Actions may include:

· following local escalation process to inform CNM, District Manager, Operations Manager and RHDC

· patients are received and / or there is deployment of HRT to the emergency site. It should be recognised that medical and nursing resources at Warren Health Service (WHS) may be best utilised in the hospital rather than deploying essential staff to the site of the incident. An option available is to deploy the Health Commander to the site and HRT who are MIMMS trained are sent from other sites.

Phase 4 -Stand Down (the emergency has abated and recovery activities begin)

The stand down stage is activated when the WHS response is no longer required and when the hospital can resume normal business. Actions may include:

· EC informing the site of “stand down” · informing CNM, District Manager, Operations Manager and RHDC · staff deployed to the Emergency Site return to hospital · debriefing occurs – see individual action cards · resources are restocked · reports are written and tabled at the WACHS-SW South West Executive

Emergency and Disaster Sub-Committee · alterations to plans, procedures and processes made as necessary.

7. Resources

Should additional resources be required, the Nursing Coordinator /Hospital Resource Nurse and Director of Emergency Medicine at Bunbury Hospital, in collaboration with the RHDC are to activate the WACHS-SW District Emergency Response or if necessary, request activation of the State Health Emergency Response Plan (SHERP).

8. Local Emergency Operations Centre

The Warren Health Service Local Emergency Operations Centre (LEOC) is to be located in the staff development / meeting room located in Ambulatory Care.

9. Media Enquires · Media enquiries / communication are coordinated by the WACHS-SW Inland

Operations Manager. · Media enquiries are managed by WACHS Communications, available 24hrs/

day on 61454166. · A media area should be established by the LHDC · The St John Ambulance Centre will be the designated location for media.

10. WebEOC

· Log in at https://myeoc.health.wa.gov.au/eoc7/ · WebEOC is a web based Incident Management System that facilitates

response and recovery agencies to access relevant incident information. It allows agencies to communicate and collaborate in real time during special events and disasters.

· WebEOC should be used to capture information pertaining to the incident. This includes capturing disaster patient demographics in the Regional Casualty Tracking board, and use of the Activity Communications Log to detail actions and communications relating to the incident.

11. Health Response Team

· Where the Director General’s Delegate or the RHDC requests HRT to attend the site and provide medical assistance, the WHS LHDC will arrange a team comprised of MIMMS trained staff.

· The LHDC in consultation with the St John Ambulance (SJA) Commander will arrange suitable transport for the team and their equipment to transport them to the Casualty Clearing Post.

· Disaster Response kits are to be in accordance with the Department of Health Pre-hospital Handbook.

12. Call in Procedure for Staff

· on confirmation of a major incident the EC can request available staff to be called in · staff rostered for the next shift are not usually contacted in the first instance · all heads of departments are expected to call in their own staff · staff must remember to bring their identification (ID).

13. Decanting of Patients · Decanting is the discharging or transferring of patients out of the hospital in

order to make beds available for casualties and to free up staff required in other areas of the hospital as part of the Emergency Response. No patients are to be discharged or transferred until authorised by the EC.

· Patients are reviewed by a medical officer and the Inpatient Unit Shift Coordinator to determine transfer or discharge

· Patients considered suitable for discharge, where possible will have prescriptions provided, appropriate discharge letters, instructions and arrangements made. Relatives or friends may be requested to collect patients.

· Patients for transfer must have copies of medical records, medications and belongings made available to go with them. The LHDC and RHDC and / or SHICC will make arrangements for transfer and transport.

14. Hospital Set up for Reception of Casualties

14.1 Emergency Department The emergency department (ED) will be the primary area dealing with the reception, triage, initial treatment, and disposal of major incident victims. The key objective of the ED response is to “do the most for the most”.

Patient Flow

· If accurate information from the scene is available and it appears the numbers may be small, or transport to hospital will mean a spaced out or pulsed arrival, then a strategy of keeping the victims in the ED for further observation and surgical decisions may be possible. However, if a true mass casualty incident occurs then flow through the department must occur and the following should be the default strategy.

· Process for patient flow through the department: o Triage nurses attend both the ambulance door and front door to facilitate

“one point” of triage: - Two or more triage nurses could be used initially - Patients are triaged on arrival using the Australasian Triage Score

(ATS). Score 1, 2 and some 3 are treated in in ED - Patients with ATS Score 4 or 5 and some 3 are treated in the

Ambulatory Care Gym, entry through hospital Main Entrance - Designate patient to appropriate bay with appropriate medical and

nursing staff

· The EC liaises with SJA, RFDS, hospital initiated transport (e.g. volunteer drivers), ward staff and other hospitals for the transfer of patients from the Emergency Department.

· A log of the transfers/discharges is to be maintained by the EC. · The holding and recovery bays in Surgical Services could be used as further

treatment areas if required and staffing available.

Emergency Department Patient Tracking · A whiteboard or log located in the ED area maintained by the Triage Nurse,

assisted by administration staff.

· The boards will have the following fields: o Patient name/number o Time seen o Present patient location o Doctor o Injuries o Planned destination

· The board will be utilised by the medical and nursing staff to track patient movement, provide information for reporting to LHDC / EC and assist planning.

Disaster Documentation, Patient Notes · Yellow files provided, marked in consecutive numbers female DV1, DV2 etc. or

male DV1, DV2 etc. located in the MIMMS cupboard in ED airlock.

· Content of the file is to include the following items labelled with the appropriate allocate number: o Identification stickers x1 o Name band x1 o MR1 form x1 o Pathology and Medical Imaging request forms x1 each o Additional continuation sheets x2 o Registration form o MR 176 Intravenous fluids o MR170A Medications o MR2 Emergency Department Trauma notes

· The files are only to be used only for the disaster victims and allocated as they

are triaged.

· Name bands are to be attached at point of triage.

· The file is to remain with the patient, hung on the end of the bed during the ED episode and transferred with the patient as required.

14.2 Ambulatory Care Gym – Walking Wounded

· The Walking Wounded Clinic (Green Tag) will be set up in the Ambulatory Care Gym with overflowing and impending discharges to go the meeting room.

· The LHDC is to assign a nurse(s) to this area and arrange for a doctor. · The Clinical Nurse manning the Walking Wounded Clinic is to obtain supplies as

per Section 24 and liaise with the EC for admissions, transfers and resources. · Patients presenting are to be triaged as per the Australian Triage Scale using the

MR1 for documentation.

14.3 Distressed Relatives Area

The Main reception / Waiting area is to be set up for distressed relatives and staffed by a mental health nurse and/or social worker who are to:

· ensure the setting up of tables, chairs and a coffee / tea dispenser

· maintain a register with the details of relatives which includes: full name, address, contact telephone number (including mobiles) and details of the person they are waiting for

· call in other mental health staff as needed. Keep list of names, time in/out for LHDC

· liaise with the LHDC to inform Police and Australian Red Cross of identification of victims/injured

· liaise with Department of Communities (Great Southern Office) for shelter, personal needs, crisis counselling

· liaise with LHDC within 24 hours after disaster to arrange hospital staff Critical Incident Stress Debriefing. Work in conjunction with Employee Assistance Program (EAP).

Australian Red Cross volunteers (may be contacted by Department of Communities). 15. Compliance

Failure to comply with this procedure may constitute a breach of the WA Health Code of Conduct (Code). The Code is part of the Employment Policy Framework issued pursuant to section 26 of the Health Services Act 2016 (HSA) and is binding on all WACHS staff which for this purpose includes trainees, students, volunteers, researchers, contractors for service (including all visiting health professionals and agency staff) and persons delivering training or education within WACHS. WACHS staff are reminded that compliance with all policies is mandatory.

16. Evaluation Monitoring of compliance with this document is to be carried out by Warren Clinical Nurse Manager every 5 years.

25.1 First Responder - Action Card 1: First Responder

First Responder: A staff member who has been alerted to an external emergency.

Responsibilities : - Take measures to record relevant information. - Take measures to ensure that the Clinical Nurse Manger Ext 5235 or A/H the General

25.2 Action Card 2: Emergency Controller

Emergency Controller: After hours the shift coordinator has delegated responsibility until the Clinical Nurse Manger arrives on-site. Shift Coordinator Ext 5236

Responsibilities: Alert and control the health response to an external emergency.

8. Instruct Communications Officer to call in personnel on Emergency Code Brown contact list and to send a group page to all contacts advising ‘Code Brown Standby’.

9. Liaise with ED, Inpatient wards and Ambulatory Care to ensure that there is adequate equipment and supplies to deal with expected casualties.

Identify designated areas for reception of disaster casualties and instruct staff to prepare these areas:

· ED Ambulance entry for Priority 1 and 2 · Main Foyer entry to receive, assess and direct Priority 3 (Walking

Wounded) to Ambulatory Care Gymnasium for management.

11. Allocate appropriate staffing to ED and Ambulatory Care Gymnasium for reception of patients including VMPs.

12. Instruct the Admin Coordinator to allocate a dedicated Clerical Officer to be located in Main Reception to collect data from presenting relatives and also to monitor the Ambulatory Care phone for enquiries from relatives.

8. Where notified by a relevant authority that a health response is not required, carry out Code Brown all clear actions.

Responsibilities: Coordinates Emergency Department response in collaboration with the Emergency Controller for an external emergency.

1. Convene all key ED stakeholders and provide a CETHANE on the type, scope and location of the external emergency. Disseminate Action Cards.

Liaise with the senior doctor about triage and resource requirements. Set up triage points manned with triage nurse(s):

· ATS score 1, 2 and some 3 presentations at the Ambulance airlock. · ATS score 4, 5 and some 3 presentations via the Main Entry to be

directed to Ambulatory Care gymnasium.

3. Request the Emergency Controller to recall medical, nursing and clerical staff. Allocate duties when arrived.

4. Ensure resources for the required health response are adequate.eg supplies, pharmacy Escalate immediately to the Emergency Controller if there are deficits.

5. Conduct a review of Emergency Department attendees with the senior ED Doctor and discharge as appropriate.

6. Provide surge capacity status to the Emergency Controller.

7. Allocate staff to manage presentations.

8. Communicate half hourly updates to the Emergency Controller

9. Coordinate triage, registration, resuscitation and on-going care.
