Embryology – OR Schedule View Trigger List 1. Select Embryology tab. 2. Screen will automatically default to OR Schedule. 3. Select Trigger List radio button. 4. Select From Date and To Date. 5. All patients currently triggered from their respective flowsheets will appear.

Schedule Retrieval 1. Retrievals must be scheduled from trigger list in order to create a gamete sheet. 2. Double click on name of patient desired. 3. The following scheduling screen will appear.

4. Double click on time slot desired on schedule. 5. The following screen will appear with the patient’s phone number, cycle order details, and

scheduling details.

6. Select Doctor of the Day. 7. Select Facility Name. 8. Enter sperm arrival time. 9. Select Procedure. 10. Select Status. 11. Enter notes when applicable. 12. Save.

View Retrieval List 1. Select Embryology Tab. 2. Select Scheduled for Retrieval radio button. 3. Select From Date and To Date. 4. All patients scheduled for retrieval will appear.

Cycle Details Icon 1. Single click on Cycle Details Icon. 2. Cycle Order List will display. 3. Double click on desired cycle to display cycle orders. 4. Single click on desired cycle and select flowsheet.

Gamete Sheet Icon 1. Single click on Gamete Sheet Icon. 2. Gamete sheet list will appear categorized by cycle number and retrieval date. 3. Double click on desired cycle to display respective gamete sheet.

To add Sperm Prep Data 1. Select Embryology Tab. 2. Select Gamete Sheet Icon. 3. Search for Patient. 4. Select Sperm Prep/Insemination Tab. 5. Place cursor on date and select F3. 6. Enter Data. 7. Select OK. 8. Select Save.

To add Gamete Sheet Data 1. Select Embryology Tab. 2. Select Gamete Sheet Icon. 3. Search for Patient. 4. List of gamete sheets will appear. 5. Reminder: A gamete sheet will be created and become available for data entry

upon scheduling a patient for egg retrieval. 6. Double click on desired gamete sheet. 7. Select gamete sheet tab. 8. The following screen will appear.

Box appears when F3 is selected for faster data entry



9. Enter number of oocytes retrieved. 10. Press tab button on keyboard to populate number of oocytes. 11. Each sites gamete sheet is customized. 12. Fields and elements vary dramatically per request. 13. Enter data specific to your gamete sheet. 14. Select Save. 15. Results will be entered and saved on a daily basis.

Shortcut to add Gamete Sheet Data The F3 function may be used to enter data more quickly and efficiently.

1. Click cursor on element date in the example below. 2. Select F3. 3. The following screen will appear.

4. Enter data into customized screen. 5. Select OK. 6. Select Save.

To add a new Gamete Sheet Adding a new gamete sheet may be necessary in the transition from paper charts to eIVF. Sites may elect to back enter data in the computer system.

1. Select Embryology Tab. 2. Select Gamete Sheet Icon. 3. Search for Patient. 4. Select New. 5. The following screen will appear.

6. Select Cycle. 7. Select Facility. 8. Select DOR (Doctor on Record). 9. Select Procedure. 10. Select Day O Date. 11. Save. 12. Refer back to section add gamete sheet data to enter details.

To Create a Status Report 1. Select Embryology Tab. 2. Select Gamete Sheet Icon. 3. Search for Patient. 4. List of gamete sheets will appear. 5. Double click on desired gamete sheet. 6. Select gamete sheet tab. 7. Select Status Report. 8. The following screen will appear.

9. Select Template Category. 10. Select Template Name. 11. Select Open. 12. The following is an example of a Day 3 Embryo Transfer Report and Plan. 13. The document is formatted to populate with pertinent data. 14. Enter more details as necessary. 15. Select Save.
