EMA
The EMA sleep apnea appliance is easilyadjusted with a variety of flexible straps to allowfor patient comfort and individualization.
he elastic mandibularadvancement appliance, or EMA,is formed from a dentalimpression and both resembleand fit like a retainer. The majordesign innovation includes theregulation of the amount ofadvancement and freedom ofmandibular motion. The flexibilityof the elastic bands allow forindividualization of your patientstreatment, and the ability for quickand easy adjustments to provideoptimum results.
Bands of varying elasticity movethe lower mandible forward intothe desired position while allowingthe jaw to move in all directions. This positioning of the mandiblecauses a forward movement ofthe tongue, reopening the air flowthrough the oral pharynx (back ofthroat).
The freedom of movement of themandible significantly reduces theoccurrence of temporomandibularjoint dysfunction and pain oftenassociated with similar appliancescurrently onthe market.
The EMAappliancealso offerslong-termstabilitydue to thefull occlusal coverage on botharches.

he Silencer® is a two component, articulated appliance which acts toadvance the mandible through acontrolled range of protrusivemovements to assure that theoptimum advancement is achieved.
The Silencer utilizes acommercially-pure titanium “hinge”which provides for the incrementaladvancement of the mandiblethrough a range of 10mm. It alsoprovides for a 6mm lateralmovement as well as a 1mm verticalmovement of the mandible, which isat the same time being held in thedesired protrusive position. Theinterincisal distance may also beadjusted over a range of 6mm.
All hinges carry a serial number, which allows for tracking andvalidation of the 5 year warranty.

Nocturnal Airway Patency Appliance
he Nocturnal Airway PatencyAppliance, or NAPA, is veryeffective in reducing the signsand symptoms of both OSA andsnoring. In moderate to severeOSA cases, 80% of the patientsreceive good to excellent resultswith the NAPA. In non-OSAsnoring, 95% of those sleepingwith a NAPA find their night timenoise is stopped or greatlyreduced. Most patients reportthey feel alert and energetic theday after wearing the NAPA.
The NAPA prevents OSA and snoring by several mechanisms:• Increasing tonicity of tongue bystretching muscle• Inhibiting movement of musclesof mastication to prevent tongueretraction via the anti-tongue-biting reflex.• Stabilizing mandibularanchorage of geniohyoid muscleto permit protraction of hyoidbone and tongue.• Displacing tongue anteriorly byholding mandible in protrusion.
Important: At least one healthyposterior tooth in each quadrant isnecessary to construct an effectiveNAPA.

octors today are finding avariety of causes relating tosleep disorders that may need tobe corrected before a sleepapnea appliance is used. Some of the key areas are
poor habits like tongue-thrustersand bruxers. A habit appliancemay be needed first to retrain thetongue so other treatment cancontinue more effectively.Bruxism is a poor habit that canbe treated prior to the sleepdisorder or it may affect whichsleep apnea appliance is best.Other doctors, like Dr. Jay
Gerber, prefer to check theneuromuscular system before
any long term treatmentis performed. Short termand long term splintscan be fabricated atJohns Dental and willbegin retraining properjaw position. Many doctors use
Ceph tracings to helpdetermine the bestmandibular position inrelationship to themaxilla. Today, doctorsare finding properalignment can improve

TREATMENT CONSIDERATIONSD
Computer ceph tracings are a good wayto determine the optimal positions of themaxilla and mandible for an improvedairway.
Bruxers tend to collapse their bite,TMJ space and airway by slowlygrinding the occlusal surface. Thiscan be a warning sign for futuresleep disorders.
Dr. Dennis Bailey says one of the firststeps in any dental treatment is tonormalize the tongue function. JohnsDental fabricates several types of habitappliances like this one to retrain atongue thruster.
Dr. Jay Gerber says a neuromuscularevaluation prior to any treatment iscrucial for long-term effectiveness andstability.
This Doctor-supplied stone modelshows the challenges of bruxism.
