1. Go to the mytaxform.com
secure website.
2. Either enter in the employer
code: 13879
OR
Enter in University of Oklahoma
3. Click Login
4. Enter in your SSN. This is
required to identify you. These
are tax forms and require SSNs.
5. Your original PIN is your date of
birth in MMDDYYYY format. For
instance birthdate of
01/01/1950 would be PIN
number of 01011950.

6. You will be prompted to
“Continue”. Click the
button
7. Enter your identifying
information. NOTE: this must
be your legal name on record
for taxes/employment and your
date of birth should be
formatted with slashes, i.e.
01/01/1950.
8. The system will continue to
match your identity based on
your phone and address.
Employee phone numbers and
Home Address in the OUHSC
Self Service portal should be
used. This will help the system
match your records.

9. Choose which form of
validation you prefer. You will
receive a code that you will
need to enter for verification.
10. When you select your option,
the system will prompt you
with the email, cell or landline
number (based on your choice
in step 9) from your profile.
Click the one you want to send
the code to.
11. You should receive your code
and enter it into the One‐Time
Passcode box. Click Continue.

12. After validation, the system will
prompt you to enter your
current PIN (the date of birth
format in step 5) and choose a
new 8 digit PIN. Re‐enter the
new PIN and click Continue.
13. Choose whether the system
remembers the device on which
you are currently logging in.
Never choose remember on a
public/shared device.

15. Once you are logged in, the system will
ask you if you want to receive your tax
forms online. This will allow you to
view/print your W2 and 1095c forms
as soon as they are posted rather than
waiting for a paper copy to be mailed.
This also allows you to come back and
view/print the forms again later (if you
lose them). This access continues even
after you leave the University.
14. If you chose to receive your forms electronically (online), you will be prompted to accept
the disclosures below. Click Accept & Continue.
16. You will need to confirm your contact information for notification when your forms are
available.

17. When your enrollment in electronic forms is complete you will receive this message.
