The 79-1151A Agreement and Customer Authorization form and 79-1151B Application form must be completed on a computer and submitted by email. Click on the blue-highlighted form name which will open the form in your browser.

Click the “Save” icon to save a blank copy to your desktop or wherever most easily accessible.

Create your To save yourself time in the future, create a template with your contact information and any Template: other information that will be identical with every submittal.

Save again and use this template for all submittals.

For detailed instructions on how to complete each section of the forms, please reference the “How to Fill out Form […]” under the Getting Started Guides on the Standard Net Energy Metering (NEMS) page of PG&E’s website.

Once all sections of the forms are populated with the project specifics, save the form by going to the “File” menu.

From the menu select “Save As.”

Chose the location where you’d like the form to save. Either to your computer desktop or to the customer’s folder.

Choose a file name.

Select “Save”. Simple as that!

Submit Forms: Submit forms by email to [email protected] .

The subject line of the email should be “STND NEM AA and APP for [Service ID #] [Meter ID #]”

Attach the completed 79-1151A and 79-1151B form in the same editable format as downloaded from the website. Receiving them in this format allows PG&E to automate the data-entry portion of the processing. This will fast-track your PTO.

In addition, attach the customer-signed 79-1151A in the format of a locked e-signed document or signed/scanned pdf.

Lastly, attach the final building permit signed-off by the city/county inspector.
