Criteria Example #1: Identify patients not at goal for HbA1c

The Registry Report in eClinicalWorks EHR can be used to identify appropriate candidates not at treatment goal and gaps in care.

The following demonstrates how to run reports based upon criteria in eClinicalWorks EHR.

To create a Registry Report, select Registry from the left navigation menu and then select the Large R for Registry

Navigate to the Labs / DI /Proc. tab

Click Sel to select the test from the list of available labs

In the pop-up window which appears, change the drop-down from "starts with" to "contains"

- Search for A1c

- Choose the A1c lab test code that your practice uses for ordering labs*

- Select OK to close the pop-up window

In the Attribute grid, set the Lower Limit to 9, leave Upper Limit blank

Check Ignore Fasting Conditions box

Set a Results Date Range

Click Run New

To export report, select Analyze (at bottom right). Select Copy. Then select Open Excel file. Close the blank pop-up box that appears and the Excel file will open

*Factors That May Impact Registry Reports The number of patients appearing on a Registry Re port may be impacted by the clinical data available in the EHR. For example, if lab results are saved in the EHR as a PDF file and not available for use as ‘data’ to be queried. Query results are only based on the specific lab code that is selected. In those cases where there are several different A1c lab options or codes (eg, A1c, HbA1c, Hemoglobin A1c, Wellness A1c) or where lab re sults are received from multiple laboratories, it may be necessary to map each lab’s codes to Logical Observation Identifiers Names and Codes (LOINC) or select each lab's order codes individually to enable all appropriate patients to appear on the Registry Report. As a best practice, all Health Care Professionals (HCPs) should consistently use the same A1c lab code option so that the query results will encompass all patients within the practice that have an A1c >9%.

The selected time period evaluates all patients who had an HbA1c >9% in the data range, regardless of what the HbA1c value is on the date the report is run.

Follow steps in Query #1, to identify patients with HbA1c value > 9%

Select RX Tab

Click SEL and type name of medication in Find box. A list of the medication(s) will display. Select the medication name and dose for which you want to run a report

Click OK after you select the drug

Make sure the search is defaulted to CURRENT MEDICATION AS OF TODAY

Select RUN SUBSET. Or, select Run Subset (NOT) to identify patients with an A1c >9% and not on a specific medication

To export data, select ANALYZE, Copy, open Excel file, close blank pop-up window, and Excel file will open.
