Alam Maritim Group has set zero accident and zero LTI as its HSE performance target and is relentless in its efforts to achieve this goal. Alam Maritim Group has launched behavioural safety campaign ‘Anda Lalai, Anda Merana’

in the beginning of 2011 as to address one of the main causes of accidents, i.e behaviour towards safety.

“Anda Lalai, Anda Merana” is a safety reminder for us to be constantly on guard in observing HSE standards and best practices. “Lalai in Malay language or careless is one of the factors which lead to accidents and injuries in work place.

In order to safeguard our people, environment, assets and reputation, the management are focusing on strengthening HSE leadership, culture and mind set, as well as enhancing HSE risk management and HSE governance.

Thus, Alam Maritim Group has developed its own house rule called “ALAM 24/7 Rules”. 24 hours and 7 days rules, comprises of 10 mandatory rules to ensure all activities are carried out safely and to prevent unwanted and unintended consequences on human safety or health conditions as well as environment. One day full of HSE activities such as health check, blood donation, HSE talks by reputable speakers and demo on safety equipment by our business partners. The event was held in collaboration with JPAM, BOMBA and Pusat Darah Negara.

HSE ENGAGEMENT SESSION WITH MARINE CREWSDate : 10th October 2012Venue : Suria Cherating Beach Resort

A session between Alam Maritim management and marine crews discussing on ways forward to better achieve good HSE performance. Marine crews were briefed on Job Safety Analysis and Behavior Based Safety tools.

ZERO IS HEROZero is Hero Campaign is a competition among Alam Maritim Group’s fleet of offshore supply vessels (OSVs) based on five selected categories for evaluation. The vessels are divided into two groups, viz. 1) Group A – workboat and workbarge 2) Group B – AHTS, supply boat, crew boat and standby boat. The five categories to be evaluated are Zero LTI, Highest UAUC Contribution, Good Housekeeping, Zero Machinery Breakdown and Zero Complaint from Clients.

En.Mohd Izham Che Ariff presented Alam Maritim Best Practices during the launch of the Marine Offshore Safety Taskforce (MOST). Also present at the ceremony were Datuk Azmi Bin Ahmad and En. Ahmad Hassanudin bin Ahmad Kamaluddin. During the Q&A session, we received very positive and encouraging comments from clients and audience. Our HSE Best Practices are;

General Manager of Alam Hidro (M) Sdn Bhd, En Noor Amran gave speech during SOGT De-Mob exercise and praise all for successfully completing the project without LTI and within scheduled and budget. Safety achievement recognition has been rewarded to all crews involved in the project. LAUNCHING OF ALAM 24/7 RULES

ONBOARD 1MAS

The launching of ALAM 24/7 Rules onboard 1MAS was successfully officiated by ALAM Group Chief Executive Officer/Managing Director Y. Bhg Datuk Haji Azmi Bin Ahmad together with Executive Director En Ahmad Hassanuddin and En Sohaime Abdullah PCSB Head DD Project on 15th May 2012 in conjunction with Management HSE Visit (MHSEV). The visit was done during SOGT pipelay project at Kimanis Sabah.

The team has conducted site HSE walkabout covering all areas onboard 1MAS. All findings were closed accordingly within the given time frame.

MANAGEMENT HSE SITE VISIT (MHSEV) ONBOARD 1MAS

PCSB Development Division, PETRONAS Development and Production (PDP), together with AMRB & TLO conducted another MHSEV onboard 1MAS and BCOT site on 2nd & 3rd Oct. 2012 respectively. The objectives of the MHSEV are to :1. Demonstrate DD/PCSB Management Leadership

and Commitment towards HSE Excellence.2. Conduct engagement and dialogue sessions with

personnel at worksites.3. Identify gaps and provide feedback on strengths and

opportunity for improvement in the implementation of HSE rules and regulations.

INCIDENT SHARING

MEDICAL TREATMENT CASE (MTC) INCIDENTS

BRIEF DESCRIPTION OF INCIDENT:During lifting activities at Bunga Raya A, the IP right foot was caught under the food container (8 feet x 8 feet) as he tried to release entangled tagline on top of suspended food container. Whilst the container was lifted approx 1 feet above the deck, the IP realized half of the tagline was entangled to one of the 4 legged slings and another half of the tagline hanging beside the container within hand reach. The IP instructed the crane operator to stop lifting and hold the load. As the IP was standing near the load, he then attempted to clear the tagline by pulling the hanging portion. While releasing the tagline, the cargo came down unexpectedly and landed on IP right foot. The IP immediately instructed crane operator to lift up and he remove away from the drop zone.

LTI CASE

BRIEF DESCRIPTION OF INCIDENT:• Duty rigger found leakage on air hose on #2 air compressor discharge valve

which has been running for the last 12 hours.• Duty rigger switched off #2 air compressor then run #1 air compressor

(observed faulty fuel indicator and report to his supervisor) • IP received instruction from the 3rd Engineer to check the portable air

compressor on deck.• IP checked #1 air compressor & found in good working condition. He then

adjusted discharge air pressure back to normal.• IP carried out his routine inspection on #2 air compressor prior to start-up

which include topping up the water in the radiator (radiator has been leaking).• Once the IP opened up the radiator cap, the boiling hot water shoot out through

the filling point and sprayed onto IP’s arms and legs.• IP stepped down from the top of the air compressor and walked on his own to

consult the Medic onboard on his injury.• Medic gave first aid treatment to IP and advised Company Man to send IP for

further medical treatment ashore.• IP was flown by regular- scheduled chopper on same day to Kerteh Airport and

was sent to Clinic Juliana, Kemaman for treatment. The doctor treated the IP and recommended that ALAM seek further treatment for IP at hospital .

• IP admitted at Kuantan Specialist Hospital and was diagnosed as having 1st & 2nd degree burns on several parts of his body.

INITIAL FINDINGS:• Radiator has been leaking on air compressor # 2• 3rd engineer given insufficient information on the problem of deck air

compressor• Air compressor on deck running continuously with no demand • No proper shift hand over / communication between Night Shift Oiler and IP• IP has not been attending the Toolbox Meeting, TBM was attended by 3rd

Engineer• Rigger started the air compressor #1 as a normal practice since Tukau Project

RECOMMENDATIONS:• To participate in the Workover on Paper with all the other party to identify the • equipment to be used for the particular project• To remove both potable air compressor [#1 & #2] on site• Establishment of shift hand-over log/information for all key personnel• Scheduled pre-tour meeting shall be conducted for the both shift crews. • To reevaluate the competency of its barge crew and their assigned tasks. • Proper procedures and awareness of assigned tasks shall be practiced by all

crew• Warning sign to be placed on the hazardous area

OILER SPLASHED BY HOT WATER FROM PORTABLE DECK AIR COMPRESSOR (LTI CASE)

INITIAL FINDINGS:• Unintentionally suspended the food container while clearing • entangled tagline. • Incorrect positioning and securing of taglines• Wrong foot print during clearing the tagline• Failure to identify and analyze all risks• No proper supervision during cargoes handling• Wrong terminology used during crane lifting operation

i.e. stand-by instead of stop.

RECOMMENDATIONS:• All cargoes must be firmly landed on deck for lifting gear and tag lines

inspection.• Minimum 2 taglines attached to strong point at each opposite corners. • No tagline shall be secured at slack 4 legged slings• Maintain safe distance from suspended cargo• Deck cargoes lifting activities and potential risks to be discussed during

toolbox. • Bosun to maintain close safety supervisory roles.• Deck crews shall only use standard crane communication.

IP sustained crush injury at right foot with fractured at big toe and 2nd toe.

DESCRIPTION OF INCIDENT• On 9/10/12 at 1330hrs, 0.8 barrels (127L) of diesel fuel (DO) was

spilled on deck of Setia Teguh and less than 5% went overboard while pumping diesel to the West Leda

• A jumper hose was used to connect the rig hose to the vessel diesel manifold

• The jumper hose burst just after Setia Teguh commenced pumping DO to West Leda and Master stopped the transfer

• Immediately the deck hands cleaned the deck• There were no personal injuries• The deck crew reconnected the rig hose directly to the manifold and

safety completed the bunkering operation

CAUSAL FACTORS• Vessel crew was unfamiliar with the correct use of the TODO

connection. Due to incorrect make up, connection remained closed, creating the backpressure

• Jumper hose was not suitable for hydrocardon transfer

DESCRIPTION OF INCIDENT :

• On 17/10/12 at 1630hrs, IP was sitting on toilet bowl in toilet

room no 302 when suddenly heard cracking sound from toilet

bowl below and IP immediately stand up but was imbalance and

fell on the sharp edge of the broken toilet bowl underneath him

• IP suffered laceration 7cm long and 2mm deep on left side of

his buttocks

CAUSAL FACTORS :

• Screw that toilet bowl to wall was loose due to broken plastic

washer• Gap between bottom of toilet bowl and floor

ACTION TAKEN :

• Inspect all toilet bowls on board marina Star 35510 and change

plastic washer to steel washer

• To explore the possibility of installing support under toilet bow to

close gap between bottom of toilet bowl to floor

TOP 5 RISKS FOR PASSENGER• Pinching hazard due to movement of the doors• Transfer by personnel basket• Struck by objects due to vessel movement• Slip, trip & fall while using the gangway/stairs• Scalding by hot water/ food due to vessel movement

SAFETY TIPS• Customize the Job Safety Analysis (JSA’s) to include hazards due to adverse weather conditions and necessary precautions to observe• Report ALL potential hazard in the UCUX form regularly• Good Housekeeping is of utmost importance

FOLLOW UP ACTION• Reinforce expectations on timely and accurate incident reporting• Provide all crew with formal training TODO dry break coupling• Alert all vessels to ensure all hydrocarbon transfer hoses are certified

and fit for service• Include checking of all transfer hoses as per ISM Plan Maintenance

System• Add to receiving facility fuel transfer checklist pre-operation review

with vessel of site specific fuel transfer equipment (TODO valve operation) and rig-up procedure

FOLLOW UP ACTION• Evaluate standard and condition of fuel transfer kits across vessels

and receiving facilities in EMEPMI operations• Revise vessel Oil Bunkering and Transfer Operation practices of Sea

Procedure and Checklist; 1. Agreed pressure and flow between the transfer parties2. Working pressure parameter during product transfer3. Chief Engineer to check the hose lay out of any oil transfer onboard4. Confirmation on correct TODO dry connect coupling make –

up including valve in open position prior to initiating transfer (ie: pressure test), if applicable

Setinggi-tinggi syukur ke hadirat Allah SWT dengan limpah kurnia dan inayahNya saya diberi

kesempatan untuk menulis di ruangan yang sangat bermakna ini. InsyaAllah pada kesempatan kali pertama ini saya akan memulakan bicara berkenaan aspek keselamatan menurut kaca mata Islam.  Islam merupakan agama yang sangat menitikberatkan persoalan keselamatan. Ini dilihat dari kata dasar ‘Islam’ itu sendiri yang berasal dari perkataan ‘salam’ yang membawa maksud ‘selamat/sejahtera’. Maka di sini bolehlah kita mendefinisikan bahawa Islam merupakan agama yang menitikberatkan soal keselamatan baik ketika di dunia, mahupun ketika di akhirat. Ini dikuatkan lagi dengan firman Allah SWT dalam surah Al-Baqarah ayat 195 yang bermaksud: “Dan janganlah sekali-kali kamu memcampakkan dirimu kedalam kebinasaan. Dan berbuat baiklah (sesama makhluk), sesungguhnya Allah mengasihi pada orang-orang yang berbuat baik.”Menurut agama Islam, mereka yang melakukan perbuatan atau kerja yang tidak mementingkan aspek keselamatan adalah haram. Mungkin ada yang tertanya-tanya kenapakah Islam meletakkan hukum haram serta berdosa bagi mereka yang tidak berhati-hati semasa melakukan sesuatu pekerjaan, sedangkan diri kita adalah milik kita sendiri dan kita yang menanggung akan segala akibatnya.

Sahabat yang dikasihi sekalian, inilah masalahnya apabila kita tidak meluaskan lagi skop pemikiran kita dan mengambil mudah urusan yang penting. Memang diri ini milik kita. Namun harus kita ingat, segala anggota, pancaindera dan tubuh badan ini milik Tuhan yang diamanahkan kepada kita untuk menjaganya dengan baik. Kita juga mempunyai keluarga seperti ibu ayah, pasangan hidup, anak-anak dan saudara mara yang sentiasa berharap keselamatan dan kepulangan kita kembali ke sisi mereka. Akibat dari mengabaikan keselamatan, pergi kita dalam keadaan sempurna, namun kembalinya kita ke sisi keluarga dalam keadaan sebaliknya.  Ingatlah wahai sahabat-sahabatku, sesungguhnya setiap anggota tubuh badan kita ini nilainya sangat tinggi. Tidak ada amaun yang boleh diletakkan walaupun hanya pada sebatang jari. Lebih-lebih lagi nyawa yang pasti hanya ada kesempatan sekali saja seumur hidup untuk kita menjaganya. Oleh itu, sama-samalah kita memupuk kesedaran akan betapa pentingnya menjaga keselamatan terutama ketika bekerja. Jadikan kerja kita sebagai satu ibadah yang mampu menghubungkan dengan baik antara dunia dan akhirat yang kita tuju. Bekerja itu tidak salah, malah ianya wajib atas tujuan mencari nafkah, tetapi salah mengamalkan dasar ketika bekerja yang akan membuatkan seseorang itu bersalah.
