6 to 7 p.m.:Randi Belisomo, WGN reporter and co-founder of Life Matters Media, will discuss both the process and barriers of “Starting the Conversation.” This presentation will explore how to solicit, consider and communicate the individual values, goals and beliefs that infuse a comprehensive end of life care plan.

Noreen Burke Coussens, aging life care specialist, MBA, CMC, with Elder Guide, Ltd., will review the POLST, its purpose and how best to complete the document including a role playing of the conversation.

7 to 8 p.m.:Mary F. Mulcahy, MD, of the Robert H. Lurie Comprehensive Cancer Center and co-founder of Life Matters Media, will discuss “Advanced Directives an Imperfect Solution.” Participants will be able to identify the different types of directives, the benefits and limitations of each and common pitfalls which make them ineffective.
