114 posters or messaging 114 kids impacted After sending the 5210 brochures home with each child at the 4 YMCA/YWCA childcare sites participating in the grant and hanging the posters up, we got really good feedback from the families about what we were doing. The conversations changed at the sites and focused on making better choices.

30 5-2-1-0 posters or messaging 30 kids impacted As part of the preschool 5210 celebration, the children got sample a variety of fresh fruits and vegetables. One little boy told the class the broccoli is like little trees and when he eats it he feels big and strong like a giant. Needless to say everyone in the class took at least a small bite of the broccoli so that they could feel like a giant too.

66 posters or messaging 66 kids impacted The new equipment that our school age programs received thru this grant enhanced many components of our program. Not only did it increase the variety of physical activities we had available, it also opened up new topics that we could discuss at our daily group meetings. The children became more aware of what we were serving for snack and there were many conversations about healthy vs. not healthy.

Medical Associates Health Care East & West Campuses - Pediatrics Strategies of Success: Strategy 1- Connecting to the Community

● Promote 5-2-1-0 goals at all Well Child visits between 2-1-18 and 6-30-18 by placing posters in the lobbies and exam rooms.

● Moving electronic billboards throughout the Dubuque area with marketing promoting 5-2-1-0 and well-child visits.

Strategy 2- Measuring and Weighing ● Continue to actively weigh and measure all children during physicals and to use Healthy

Habits questionnaire. Strategy 3- Talk with Patients and Families

● Engage patients and families in conversations about 5-2-1-0 and diet, nutrition, exercise, screen time. Distribute Activity Tracker band and log to all patients over the age of 5 while supplies last.

50 posters or messaging 470 kids impacted We are discussing issues of the 5-2-1-0 goals with all children. Those with >85% BMI are receiving activity trackers and instructions. Children seem very interested and excited about the watches/trackers. We will continue to follow through with message and distribute all step counters until gone.

2030 children and families reached directly in Year 1.

Lincoln Elementary School Strategies of Success Strategy 1: Engage community partners to help support healthy eating

● To provide fresh, nutritional food to the children of Lincoln Elementary School (LES) and their families to bridge permanent funding which begins June 2018.

○ Paid to have commercial refrigerator moved to school. Purchased freezer. Had food pantry available during conferences and supplied for 41 families (195 people).

○ Purchased new standup freezer and was able to distribute frozen hams in addition to many other food items purchased from St. Stephen’s at our March 14th food pantry. (13 families/75 people.)

Strategy 2: Partner with and educate families ● Co-promote 5-2-1-0 through posters, flyers and electronic communication to families.

1,400 posters or messaging- fliers with the 5-2-1-0 logo to families for the Feb, March, April and May (2) Food pantries. In addition, information was sent electronically each time to all families, 5 large laminated posters displayed throughout school at all times, and information on our website/Facebook page for Lincoln elementary school. School Board presentation that was televised May 14, 2018. 99 kids impacted- Pantries served approximately 930 students We started the pantry during all three day of parent-teacher conferences in February and served 51 families. The pantries were then offered the second Wednesday of the month with two in May- we served an additional 137 families. The families answered surveys at the conferences indicating their needs and each pantry we offered more of the items they indicated they needed most- fresh produce, milk, eggs, grains, meats. Families were very appreciative and students got very excited on the pantry days. We will start again in the fall when school resumes and now look to start another pantry in one of the other Title One schools next year as well.

Eleanor Roosevelt Middle School Greenhouse Strategies of Success Strategy 1: Limit unhealthy choices while providing healthy choices

● Greenhouse will allow expansion of the produce grown at school, including broadening of different varieties of produce.

Strategy 2: Participate in local initiatives that support healthy eating ● Create an outdoor learning lab across the curriculum to learn about plan production from

seed to harvest. Learn about nutrition and make healthy snacks. Financial literacy through purchasing materials and products to grow, sales at local Farmer’s Market, etc.

Strategy 3: Engage community partners to help support healthy eating • Expand upon current program of school garden club of providing locally grown, organic,

fresh produce to local women’s shelters. Strategy 4: Collaborate with Food and Nutrition Programs

• Involve Food Services to serve produce grown in collaboration with the school lunch program. This allows students to experience fresh, organic, new varieties of produce.

Strategy 5: Collaborate with Food and Nutrition Programs

• Implementation of new policy – Entirely new policy to serve school grown produce with school lunch program according to USDA guidelines.

Strategy 6: Partner with and educate families • Communication through key partners, traditional DCSD newsletter communications to

families regarding programs that relate to the greenhouse school garden. All money was used for purchase of greenhouse. Concrete will be poured mid-June and construction will begin next fall. She talked about how excited the students are about getting the greenhouse. With the weather this spring, they lost many of the seedlings in the raised beds. Having the controlled environment and the ability to extend the growing season and plant other varieties of produce. As part of addressing childhood obesity, teaching students how to grow their own vegetables will help them increase their consumption of more vegetables. Students are more likely to try them if they were part of the growing process. Produce grown will also be available as additional items at lunch for all students to get a chance to sample and add healthier options to their diets.

Community Strategies of Success Strategy 1- To reinforce the need for daily physical activity at least one hour per day.

● 5-2-1-0 was added to the Dubuque YMCA/YWCA sponsored Go the Distance day held at Loras College annually for all DCSD 4th grade students.

Strategy 2- To reinforce the need for healthy foods, snacks and drinks ● Go the Distance day is also sponsored by Hy-Vee grocery stores who supply healthy

snacks for the event. 12 posters or messaging 800 kids impacted Annually the Dubuque Community YMCA/YWCA sponsors Go the Distance day at Loras College and this year, 5-2-1-0 was also introduced at the event using posters, activities and healthy snacks. Also added this year was Dubuque on the Move, for all Dubuque residents.
